Social security and prevention at work
Since the first occupational accident insurance system was introduced in Germany 125 years ago the prevention of work-related accidents and diseases has been an intrinsic part of social security
Today, many social security schemes are actively involved in prevention efforts at the workplace, in particular social accident insurance
Their detailed knowledge of accident and disease causes and impacts is crucial for the design and evaluation of prevention measures

As health is a person’s most precious possession, the preservation of the health of mankind is a key objective for social security. After all, healthy people are also the basis for productive and cost-effective business activities, successful prevention serves to preserve human capital, to control costs for all actors involved and to avoid preventable human suffering. Health protection has become one of the hottest issues on social policy agendas. Considering international commitments to realize global improvements in health levels, not least to help combat poverty, this is not surprising: Despite increased life expectancy, there is a growing concern that new health problems will emerge: For example obesity in Europe continues to increase; an estimated 500 million people may die worldwide as a result of tobacco use in the years to come and 35 million people already die each year from cardiovascular diseases, cancer, diabetes and chronic respiratory diseases. At the same time, increases in the costs of medical care are accelerating in view of new medical technologies, increased expectations by the insured and demographic changes. Furthermore, affordable health care remains inaccessible to the majority of the world’s population. As one measure of the challenge, it is estimated that 100 million people a year fall into poverty because of inadequate access to affordable medical treatment.

ConclusionsWith a dramatically changing world of work the demands on prevention are increasing, as workers’ health increasingly is influenced by both occupational and non-occupational factorsPrevention measures must take a multidimensional and more individual view focussing on both health and employability, incl. factors like competencies, lifestyle and social environmentMore holistic and integrated approaches are needed, where classical prevention measures are supplemented by health promotion and return to work programmesIncreasing collaboration and coordination across different social security branches and organizations vis-à-vis the individual and the workplace is requiredPrevention structures and practices at the workplace need to be reviewed and new competencies developed
