Crea+ng the ligaments

1. Volar wrist ligaments.

To create a light +nge of blue for the wax mix the white wax with blue. Using rolled and flaFened wax create the following extrinsic ligaments using the illustra+ons as a guide.

a. Radioscaphocapitate ligament

b. Long radiolunate ligament

c. Short radiolunate ligament

d. Ulnolunate ligament

e. Ulnocapitate ligament

f. Ulnotriquetral ligament

Appreciate the space of Poirier between the two chevrons.

You can make the ligaments appear more real using finer sculp+ng tools.

2. Dorsal wrist ligaments

Using rolled and flaFened wax create the following extrinsic ligaments using the illustra+ons as a guide.

a) Dorsal radiocarpal ligament

b) Dorsal intercarpal ligament

3. Beak ligament of thumb.

Create a ligament between the ‘trapezium and beak’ of first metacarpal.

4. Radial and Ulnar collateral ligament of thumb and other finger MCP joints.

Appreciate the orienta+on of these ligaments with the proximal volar aFachment.

5. Radial and ulnar collateral ligaments of the interphalangeal joints.

Appreciate the difference in shape of the metacarpal head and the head of the proximal phalanx change. This is the reason for the change in length of the collateral ligaments with MCP flexion and the reason for the intrinsic posi+on of the hand.

6. Create the volar plates for the MCP and IP joints.

Prac+cal session 2:

Crea+ng small muscles of the hand

1. To create the colour of muscles, mix the white wax with red. Using the picture, create the dorsal interossei. These are bipennate muscles taking origin from both the metacarpals. They are aFached to the base of the proximal phalanx and the extensor tendon. AS you have not completed the extensor tendon you can complete the distal part later.

2. Create the palmar interossei. These are unipennate muscles which take origin from one metacarpal only.

3. Create the thenar and hypothenar muscles.

Make adductor pollicis no+ng its origin from the third metacarpal and create its aponeurosis which goes over the ulnar collateral ligament of the thumb. Remember how this can be the cause for the Stener lesion.

Remember that abductor pollicis brevis and flexor pollicis brevis take origin from the flexor re9naculum, so you can make them a<er you make the flexor re9naculum on the second day.

Make the muscles look more real by crea+ng the fibres using sculp+ng tools.

Prac+cal Sessions 3:

Crea+ng the Extensor tendons

You will find premade strips of wax which you can use for crea+ng your tendons. Alterna+vely, you can make your own tendon strips.

Remember the compartments of extensor tendons and try and make them in order.

Make them up to the MCP joints and create the sagiFal bands. Create the juncturae tendinum between them. Appreciate the arrangements between EIP and EDC and EDM and EDC.

Using flat sheets of prefabricated wax create the extensor re+naculum.

Now complete the distal extensor mechanism crea+ng the central slips and lateral bands. You can now connect your interossei to the extensor mechanism.

Try and create the radial artery in the anatomical snuff box, as it passes between the two heads of first dorsal interosseous muscle.

Now is the 9me to make the inter-metacarpal ligaments. Turn the hand and create them between the metacarpal heads. Remember that they are volar to the interossei.

Crea+ng the flexor tendons.

Using the pre-made tendons, create FDS and FDPs of the fingers. Make note of the chiasma of the FDS and how the FDP tendons pass through them.

Create FPL for the thumb.

Now create the pulleys. Create A1 pulley, oblique pulley and A2 pulley for thumb. Create A1-A5 and the cruciate pulleys for the fingers.

Median, ulnar nerves

Prac+cal session 5

Create the median and ulnar nerves and the digital nerve branches. Try and create the motor nerve branches to thenar muscles.

You should create the deep branch of the ulnar nerve which supplies the interossei, the ulnar two lumbricals and adductor pollicis muscles. You may have to li_ off some of the already created structures to place the deep branch underneath them.

Also try and create the dorsal branch of the ulnar nerve which is given off proximal to the wrist.

Create the arteries to the hand. Note how the radial artery passes into the palm between the two heads of dorsal interossei. Complete the superficial and deep palmar arches and the digital vessels. Appreciate that the thumb and radial side of index finger are supplied off from the deep palmar arch.

Now complete the flexor re+naculum and the rest of the thenar muscles.

You can create palmaris brevis on the hypothenar side.

Create the dorsal veins and superficial radial nerve branches. The dorsal venous arch has a variable paFern and you can make any paFern of your choice. Proximally the vein join up to form cephalic and basilic veins.
