This is a story about different groups coming together to change lives.

Let me tell you about how NHS Halton CCG, Halton Council and Community Integrated Care transformed a traditional dementia care home into a center of excellence…

Background to St Luke’s

Successful home for many years

Recent years began to experience occupancy

issues.

Hands-off relationship between the home and

the Local Authority

Losses for many years, becoming untenable…

…and then in 2012

NHS reforms – introduction of the CCG and integration

Worked together to create a true Centre of Excellence

David Lyon, CCG Clinical Lead and specialists became

involved

NHS Halton CCG Clinical Specialists work in the service, a

transformation of its living environment and a focus on

providing true relationship-centred care - delivered by

highly trained staff.
