BEFORE HOSPITALIZATION

The patient was feeling well according to his father, his health was good. The mother of the patient believes in Herbal Medicines. The patient loves to play with his toys.

DURING HOSPITALIZATION

The patients parents sees their child as a total ill person because he cannot do anymore the things he usually does like playing with his toys. The patient rely his present condition with the help of the therapeutic personnel and by following the prescribed medications.

BEFORE HOSPITALIZATION

According to the parent of the patient, he has a good appetite and drinks his milk frequently.

DURING HOSPITALIZATION

The appetite of the patient decreases . He is on a

BEFORE HOSPITALIZATION

Patient has no problem regarding his bowel movement. He defecates 2-3 times a day. Urinates frequently with a light yellowish color. No discomforts stated regarding his bowel and urinary pattern.

DURING HOSPITALIZATION

No apparent changes occurred.

BEFORE HOSPITALIZATION

He loves to play with his toys. According to his mother, he often plays with his relatives.

DURING HOSPITALIZATION

The patient was restricted to play so much as it might lead to an asthma attack.

BEFORE HOSPITALIZATION

The patient sleeps 8-9 hours, no problem regarding his sleeping pattern and takes a nap regularly.

DURING HOSPITALIZATION

The patient has no regular sleep because of intervention when nurses or doctors come in for check ups.

BEFORE HOSPITALIZATION

He is normal in terms of his cognitive abilities. He can easily comprehend on things. In terms of his perceptual patterns, he has no problems with his senses.

DURING HOSPITALIZATION

He was normal as before in his cognitive and perceptual pattern. He obeys simple commands.

The patient lives with his mother and father. He has a good relationship towards the people around him, though at times he’s a bit shy to other people.

DURING HOSPITALIZATION

Family members of the patient become worried when the illness occurred.

He does not fully identify his situations having stress. At his age, he still has a playful mind and he doesn’t mind the stressors in life.

DURING HOSPITALIZATION

BEFORE HOSPITALIZATION

Religion is important to the family; it is their way of having time together. Patient is a Roman Catholic. Parents also believes in “hilot” and faith healer.

DURING HOSPITALIZATION

According to the patients mother, the illness that brought there child to the hospital increases their faith.
