girl scouts of greater chicago and northwest indiana SITE USE AGREEMENT This Site Use Agreement ("Agreement") is made as of 3/27/2015 3:58:00 PM by and between Girl Scouts of Greater Chicago and Northwest Indiana, Inc. ("GSGCNWI") and ("Group Coordinator"). GSGCNWI and Group Coordinator agree to the terms as follows: 1. Group Coordinator will have the limited and nonexclusive right to use the certain GSGCNWI property ("Property") as set forth in the attached Reservation Sheet for a certain group activity and for no other purpose. 2. Group Coordinator will pay GSGCNWI all expenses related to the group's use of the Property, including the charges set forth in the Reservation Sheet. 3. Before GSGCNWI will reserve the Property for Group Coordinator, it must receive a 50% deposit of the reservation fee along w i th the signed Agreement. GSGCNWI will not reserve the Property unless the deposit and signed Agreement are returned to GSGCNWI within a reasonable time as determined by GSGCNWI. Modifications to the reservation after the signed agreement and deposit have been received will be at the sole discretion of GSGCNWI. 4. The deposit paid by Group Coordinator as required by Section 3 of this Agreement is nonrefundable. Provided that however, should Group Coordinator need to reschedule the group activity, GSGCNWI may at its sole discretion apply the deposit paid by Group Coordinator to charges due and owing for a new reservation. 5. Fifteen (15) days prior to the reservation date, Group Coordinator must pay the remaining 50% of the reservation fee to GSGCNWI. A credit card must be provided with final payment to be used in the event of minor damages. 6. During the check-in process, GSGCNWI will assign a GSGCNWI Representative to the group. 7. Group Coordinator further agrees, represents, and warrants as follows: a. Before the group activity, Group Coordinator will obtain executed parental consent forms as applicable, including the Activity Permission and Emergency Medical From as provided in Volunteer Essentials. b. At the time of reservations, Group Coordinator will notify GSGCNWI's Property Registrar of any special accommodations the group may need. c. Upon arrival on the Property, Group Coordinator will provide the designated GSGCNWI Representative with the most current, group participation roster. d. Group Coordinator acknowledges receipt of the current Volunteer Essentials, Outdoor Essentials, applicable Property Guide, Safety Activity Checkpoints and Procedures for Arrival and Departure (collectively herein "Guides"). Group Coordinator has read and understands all obligations set forth in the Guides. Further, Group Coordinator represents that a group participant who has participated in the applicable training as provided in the Guides will be present on the Property during the duration of the group activity. e. Group Coordinator will communicate all rules and guidelines posted on the Property or in the Guides to each group participant. All group participants, including Group Coordinator will follow all rules and guidelines at all times while on the Property. f. Group Coordinator will make certain the group includes a physician, nurse, or EMT, or First Aider as defined in Volunteer Essentials (each individually "Medical Professional"). The Medical Professional will be present on the Property at all times during the duration of the group activity to administer the necessary medical treatment to group participants, including child CPR. Group Coordinator will provide GSGCNWI with a copy of the Medical Professional's current certification fifteen (15) days prior to the reservation date. GROUP WILL NOT BE PERMITTED TO USE THE PROPERTY UNLESS GROUP'S MEDICAL PROFESSIONAL IS ON THE PROPERTY AT ALL TIMES DURING THE RESERVATION. g. In the event of a medical emergency while on the Property, Group Coordinator will immediately contact emergency medical services (911) to respond. After contacting 911, Group Coordinator will immediately notify the GSGCNWI Representative by telephone to report the medical emergency. h. Group Coordinator will immediately notify the GSGCNWI Representative by telephone to report any damage to the Property occurring while on the Property. i. As soon as possible, but no later than 24 hours from the occurrence of any injury to person or property damage while on the Property, Group Coordinator will submit a completed Accident/Incident Report Form to the GSGCNWI Representative. j. Group Coordinator will be present on the Property at all times and will provide all necessary supervision to prevent any and all personal injury and loss or damage to the Property. Should a group medical emergency require Group Coordinator to leave the Property, Group Coordinator will designate a back-up person to be present on the Property at all times to provide the required supervision. Coordinator will immediately notify the GSGCNWI Representative by telephone of the designated back-up person. k. Group Coordinator will arrange and make certain that a vehicle other than a bus is available at the Property to transport a group participant who needs medical treatment. Additionally, Group Coordinator will confirm and require that the owner and driver of such vehicle maintains general liability, collision, and comprehensive automobile insurance covering, without limitation, (i) injuries, death, and property damage resulting from an accident, and (ii) collisions. I. The number of expected guests will not exceed the reservation nor the posted occupancy capacity on the Property, m. Group Coordinator will properly assign lodging to each group participant as provided in Safety Activity Checkpoints. n. Group participants and any other person providing services or support to the Group Coordinator while on the Property will comply with all applicable laws, rules and regulations. o. No group participant will possess any alcoholic beverages, illegal drugs, tobacco products, weapons (including firearms), fireworks, explosives, animals, or any item which may present a risk to person or property while on the Property. Notwithstanding the foregoing, a service animal will be permitted on the Property as required to aid a group participant with a disability. Prior to the group activity, Group Coordinator must notify the GSGCNWI's Property Registrar of any necessary service animal. p. No group participant shall bring an ATV, dirt bike or non-licensed recreation vehicle on the Property. q. Group Coordinator will complete and submit the Camp Property Check-In & Out Form to the GSGCNWI Representative. r. Before departure, Group Coordinator will make certain the reserved area is clean, leaving it in good condition, ready for use by the next guest. s. Unless program leadership is being provided by GSGCNWI as part of this Agreement, Group Coordinator will plan the group's camping experience and provide the group's program activities. 8. Group Coordinator shall indemnify GSGCNWI, its officers, directors, employees and agents from any and ail third party claims, disputes, costs, expenses, liabilities SAMPLE
