Betty Carlisle made a motion, Sarah Stokes seconded, and the Board voted unanimously to approve the minutes of the May 21, 2012 Board meeting. Santresa Weldon, CO087817, submitted a refund request. Betty Carlisle made a motion to refund the $30 initial application fee, Norma Banks seconded and the Borad voted unanimously to refund the $30 initial application fee. Marguerite Everson submitted an inquiry to the Board seeking the approval to have an open air canapé to provide haircuts in the event a hurricane destroys the salon or if the salon is without power for days. The Board advised that she be referred to the Cosmetology Board rules regarding salons. Rivertown School of Beauty, Barber, Skin Care and Nails submitted a request that the Board speak to acceptance of a high school diploma. The Board advised that staff is to accept high school diplomas at face value, only looking further if the diploma is challenged. LAKA USA, LLC submitted a letter seeking approval for a boutique-style retail spa kiosk. The Board advised that LAKA USA, LLC be referred to the Cosmetology Board rules regarding salons. Chris submitted an inquiry to the Board regarding specialty school licensing. The Board viewed the inquiry as informational only. The Board reviewed a report regarding the types of Board Approved Continuing Education Providers. The Board has 31 approved CE Providers. APPOINTMENTS

S.S.B.S. did not appear before the Board to discuss the pending cosmetology school application.

R.S.O.B.B.S.C.&N. appeared before the Board to discuss their pending cosmetology school application. The Board reviewed the facility plans and documents presented. The Board recommended approval to schedule an initial school inspection.

T.H.A.I. appeared before the Board to discuss their pending cosmetology school application. The Board reviewed the facility plans and documents presented. The Board recommended approval to schedule an initial school inspection.

E.B.S. appeared before the Board to discuss their pending cosmetology school application. The Board reviewed the facility plans and documents presented. The Board recommended approval to schedule an initial school inspection.

D.I.O.E.A.C. did not appear before the Board to discuss their pending cosmetology school application. The school is adding “Inc.” to their name. The Board recommended approval for the school application.

A motion was made by Betty Carlisle, seconded by Sara Scott, and the Board voted unanimously to enter Executive Session in accordance with O.C.G.A. §43-1-2(k) and 43-1-9(h) to deliberate on applications and enforcement matters and to receive information on applications and investigative reports. Voting in favor of the motion were Norma Banks, DRee Church-Krohn, and Penny Johnson.

EXECUTIVE SESSION

M.H. (Esthetician Reciprocity Application) – The Board recommended to table and request educational documents from the United Kingdom. The Board further suggested she reinstate her Arizona license. B.K. (Hair Designer Reciprocity Application) – The Board recommended to request certified documentation of training hours on the school’s letterhead. Kalen S. Wheeler, ES004690, requested to have the probation status lifted from her license. The Board recommended to approve the request and lift probation status from license. Zeta R. Carter, CO112630, requested to have the probation status lifted from her license. The Board recommended to deny the request stating probation cannot be lifted until a letter from the court is received stating that probation has been terminated. J.D. (Master Cosmetology Reinstatement Application) – The Board recommended to reinstate upon receipt of completion of 15 hours CE. A.H. (Master Cosmetology Renewal Application) – The Board recommended to rescind previous vote and ratify renewal.

Shawana Smith-Lacy, CO073356, requested a waiver of CE requirements for renewal. The Board recommended to deny the waiver. Tammie Taylor, COA005896, requested an additional renewal of her cosmetology apprentice license. The Board recommended to deny the request. C.R.W. (Master Cosmetology Reciprocity Application) – The Board recommended she sit for the examination. A.R.J-A. (Master Cosmetology Reciprocity Application) – The Board recommended she sit for the examination. T.J.T. (Cosmetology Apprentice Renewal Application) – The Board recommended approval for renewal stating that apprenticeship must be complete in 2 years and she cannot practice the profession without a valid cosmetology license. Z.A.L. (Initial Hair Designer Application) – The Board recommended approval. B.T.C. (Cosmetology Apprentice Renewal Application) – The Board recommended approval for renewal stating that apprenticeship must be complete in 2 years and she cannot practice the profession without a valid cosmetology license. Following Executive Session, Cynthia Stein declared that the Board was in Open Session for the purposes of voting on the business items considered in Executive Session. Betty Carlisle made a motion to approve the recommendations made in Executive Session.
