GENERIC DRUG VOUCHER PROGRAM A COST-SAVING ALTERNATIVE The rising cost of medication is a concern for health insurance members, employers and physicians. As one response to this concern, The ODS Companies (ODS) has developed the Generic Drug Voucher Program. The voucher program is designed to promote the use of safe and effective medications that are not available as samples at the doctor’s office. For participating employer groups, the Generic Drug Voucher Program allows members with prescription coverage through ODS to receive an initial 30-day supply of a selected generic drug at a one-time $0 copayment. The voucher can be found on page 2 of this document and is also available at www.odscompanies.com through your myODS online account, at participating employer group intranet sites or by mail when requested from ODS. AT YOUR PROVIDER’S OFFICE Members are encouraged to take a voucher to their provider appointments to discuss if any of the voucher medications would be appropriate as either a new medication or an alternative to a medication they are currently taking. If the member’s physician agrees that one of the medications listed on the voucher would be appropriate, the member should obtain a new prescription from his or her physician to present at the pharmacy with the voucher. AT YOUR PHARMACY The preferred generic voucher form must be accompanied by a new prescription. Your pharmacy will use the information on the voucher form to process the prescription at a one-time $0 copayment. Vouchers are valid for only one drug at a time. The $0 copayment applies only once to the selected medication; any subsequent refills will be subject to the copayment and prescription benefit guidelines for your program. Voucher forms are only available to members and dependents of employer groups who participate in this program. Please keep in mind that your physician can best assess the appropriateness of a medication available through the voucher program for your individual treatment.
