Generic Diflucan (Forcan Tablets), a synthetic triazole antifungal agent is indicated for the treatment of Vaginal candidiasis, Oropharyngeal and esophageal candidiasis, Cryptococcal meningitis. Generic Diflucan is also effective for the treatment of Candida urinary tract infections, peritonitis, and systemic Candida infections including candidemia, disseminated candidiasis, and pneumonia.
Generic Generic Diflucan (Forcan Tablets), is used for the treatment of:1. Genital candidiasisVaginal candidiasis (vaginal yeast infections due to Candida)Candida balanitis2. Mucosal candidiasis:Oropharyngeal and esophageal candidiasisBrochopulmonary infectionsCandiduriaMucocutaneous candidiasischronic oral atrophic candidiasis (denture sore mouth)
3. Tinea pedis, tinea corporis, tinea cruris, tinea versicolor and dermal Candida infections. Forcan is not indicated for nail infections and tinea capitis.4. Systemic candidiasis Candidaemia Disseminated candidiasis Peritonitis Candidal infections of Endocardium, pulmonary and urinary tracts Candidal infections in patients with malignancy, in intensive care units or those receiving cytotoxic or immunosuppressive therapy may be treated
5. Cryptococcosis:Cryptococcal meningi:s and infections of other sites (e.g. pulmonary, cutaneous). Normal hosts and patients with AIDS, organ transplants or other causes of immunosuppression may be treated. Generic Diflucan can be used as maintenance therapy to prevent relapse of cryptococcal disease in patients with AIDS.6. Prophylaxis: Generic Diflucan is also used to decrease the incidence of candidiasis in patients undergoing bone marrow transplantation who receive cytotoxic chemotherapy and/or radiation therapy.
The recommended dosage for the treatment of various indications is given below:➔Single Dose:➔Vaginal candidiasis: The recommended dosage of Generic Diflucan for vaginal candidiasis is 150 mg as a single oral dose.➔Multiple Dose: In general, a loading dose of twice the daily dose is recommended on the first day of therapy to result in plasma concentrations close to steady-state by the second day of therapy.
➔Oropharyngeal candidiasis: The recommended dosage of Generic Diflucan for oropharyngeal candidiasis is 200 mg on the first day, followed by 100 mg once daily.➔Esophageal candidiasis: The recommended dosage of Generic Diflucan for esophageal candidiasis is 200 mg on the first day, followed by 100 mg once daily. Doses up to 400 mg/day may be used, depending on the patient's response to therapy.
Recommended Adult Dosage ➔Systemic Candida Iinfections: In the case of systemic Candida infections including candidemia, disseminated candidiasis, and pneumonia, optimal therapeutic dosage and duration of therapy have not been established and dosages of upto 400 mg daily have been used.➔Cryptococcal Meningitis: The recommended dosage for treatment of acute cryptococcal meningitis is 400 mg on the first day, followed by 200 mg once daily.
➔Prophylaxis in patients undergoing bone marrow transplantation: The recommended Generic Diflucan daily dosage for the prevention of candidiasis in patients undergoing bone marrow transplantation is 400 mg, once daily.
ContraindicationsGeneric Diflucan (Fluconazole Tablets) is contraindicated in patients with the following conditions:
➔Hypersensitivity to Fluconazole or any inactive component of this medication.➔ Caution should be used in prescribing Forcan to patients with hypersensitivity to other azoles.
In Patients Receiving a Single Dose for Vaginal Candidiasis: The most common side effects reported in the patients who received 150 mg single dose fluconazole for vaginitis were headache (13%), nausea (7%), and abdominal pain (6%). Other side effects reported with an incidence equal to or greater than 1% included diarrhea (3%), dyspepsia (1%), dizziness (1%), and taste perversion (1%). Most of the reported side effects were mild to moderate in severity.
Side Effects of Generic Diflucan TabletsIn Patients Receiving Multiple Doses for Other Infections:The following treatment related clinical side effects occurred at an incidence of 1% or greater in patients receiving Generic Diflucan (Forcan Tablets) for 7 or more days in clinical trials: nausea 3.7%, headache 1.9%, skin rash 1.8%, vomiting 1.7%, abdominal pain 1.7%, and diarrhea 1.5%.
➔Simultaneous use of fluconazole and erythromycin has the potential to increase the risk of cardiotoxicity (prolonged QT interval, torsade de pointes) and consequently sudden heart death.➔Hepatic injury: Generic Diflucan should be administered with caution to patients with liver dysfunction.➔Anaphylaxis: In rare cases, anaphylaxis has been reported.
➔Dermatologic: Patients have rarely developed exfoliative skin disorders during treatment with Generic Diflucan. Patients who develop rashes during treatment with Generic Diflucan should be monitored closely and the drug discontinued if lesions progress.➔Use in Pregnancy: A rare pattern of distinct congenital anomalies in infants exposed in utero to high dose maternal fluconazole (400-800 mg/day) during most or all of the first trimester has been observed.
There have been reports of overdose with Fluconazole accompanied by hallucination and paranoid behavior. Very high doses of fluconazole, can possibly cause decreased motility and respiration, ptosis, lacrimation, salivation, urinary incontinence, loss of righting reflex, and cyanosis.
In the event of overdose, symptomatic treatment (with supportive measures and gastric lavage if clinically indicated) should be instituted. Fluconazole is largely excreted in urine. A three-hour hemodialysis session decreases plasma levels by approximately 50%.
