TNBC Patienten profitieren von einer neoadjuvanten Chemotherapie (NACT) mit Carboplatin → Die pCR Raten waren noch höher bei Patienten mit BRCA 1/2 Mutationen (pCR 65%) und mit HRD-Score high (pCR 63%)

Rationale I



Bei etwa 70% der Patienten mit TNBC lässt sich ein homologer rekombinanter Mangel (HRD) nachweisen→ 67% haben einen hohen HRD-Score und 30% haben eine tBRCA Mutation

Es besteht eine hohe Korrelation zwischen Tumor und Keimbahn BRCA 1/2 Mutationen

Bestimmung der Rate pathologischer Komplettremissionen (pCR= ypT0/is ypN0) nach neoadjuvanter Behandlung mit Olaparib und Paclitaxel gefolgt von Epirubicin und Cyclophosphamid (POEC) bei Patienten mit primärem Brustkrebs und HR defizienten Patienten



Sekundäre Zielkriterien

Bestimmung der pCR Raten (ypT0/is, ypN0) in den stratifizierten Subgruppen

Raten von ypT0 ypN0; ypT0 ypN0/+; ypT0/is ypN0/+; ypT (jede) ypN0) mit PO→EC und Vergleich mit PCb→EC

Bestimmung der pCR Rate bei HRD high mit und ohne tBRCA Mutation

Bestimmung der Ansprechrate des Brusttumors und der axillären Lymphknoten

Rate der brusterhaltenden Therapie

Toxizität und Compliance

Histologisch gesichertes, primäres Mammakarzinom (uni- oderbilateral).

Triple-negativ oder HR+/Her2- (zentral bestätigt)

tBRCA/gBRCA 1/2 positiv und/oder HRD positiv

cT2 - cT4a-d

cT1c wenn cN+ oder pNSLN+ oder TNBC oder Ki67>20%

Komplettes Staging innerhalb von 3 Monaten vor Randomisierung

Normales EKG und LVEF

Unauffällige Hämatologie und klinische Chemie
