3. Description of the observationElectrical cable is crossing on the walkway at the ground level, one of the our worker walked the path, he had tripped by cable . But some how he managed from fall and no harm happened to him.

4. Root Cause

Cable laid on the walkway

5. Corrective & Permanent Action Cable should be Properly routed away from the walk way
