Resource and Patient Management System (RPMS) Application Services

RPMS is a decentralized integrated solution specifically for management of cl inical and administrative information in IHS, Tribal, and Urban (I/T/U) healthcare facil ities. Flexible hardware configurations, over 100 software applications, and network communication components combine to create a comprehensive clinical, financial, and administrative solution – a solution that can stand alone or function in concert with other components as needed.

OIT performs all of the necessary software development and enhancement activities involved in updating RPMS software. The work is performed by a combination of OIT Federal and contracted developers following SDLC/EPLC compliant software development stages: 1. Initiation Phase2. Concept3. Planning4. Requirements Analysis5. Design6. Development7. Test8 Implementation Planning

I/T/U health care facil ities vary greatly in size, services provided, and in a number of other factors. For this reason it is difficult to provide a single set of recommendations for hardware and network infrastructure that will fit all sites. configurations from two RS 6000 computers for large facil ities to one Intel-based Windows NT computer for small facil ities. Softwa re Applica tions: RPMS applica tions opera te individua lly and a s an integra ted suite. Using

the HL7 protocol, RPMS applications can be interfaced with a variety of commercial off-the-shelf (COTS) software products. Networking Connectivity: RPMS informa tion can be exchanged over a loca l a rea network (LAN)

within a single facil ity a wide area network (WAN) with other facil ities and the Internet with other

1.2 Resource and Patient Management System (RPMS) Application Services

Ambulatory Outpatient Clinic

NOTE ; b. [FMEO - Implication - Many sites are currently paying the same amount as hospitals. Creation of this package implies that some sites will not have to pay as much as hospitals.

An ambulatory care facil ity supports day-to-day activities specific to improving patient care. There are two primary services required in order to support an ambulatory clinic including administrative and clinical services. The administrative services focus on the day-to-day program management and executive administration of the overall facil ity. Administrative services include business office, central supply, financial management, information technology, office support, quality management, and telemedicine services. The clinical service is the point of initial contact, or entry level, for patients seeking medical attention. Patients may/may not have scheduled appointments during normal/extended clinic hours 5 to 7 days a week at an ambulatory care facil ity. Clinical services include care provided by experienced l icensed providers in specialized areas including nursing, primary care, nutrition, surgery, audiology, dental, & eye care. In most cases, ambulatory clinical services also offer ancil lary support services including imaging, laboratory and pharmacy. All ambulatory sites include health information management (HIM) services that specifically target patient treatment records, e.g. obtaining, securing, protecting, preserving, and util izing (HIM) information. HIM management services also coordinate records preparation transcribing of records and assemble the records from

In order to support an ambulatory clinic, health information technology (HIT) is essential for managing the day-to-day activities. IHS/OIT/DIT provides a full range of HIT services that help the administrative and clinical services meet the everyday demands that are required when caring for Native American and Alaska Native patients. This includes services focused on new software development utilizing Enterprise Performance Life Cycle (EPLC) standards to ensure successful project initiation, software requirements, technical design, development, testing, certification, and release management activities. Our software applications provides an infrastructure foundation to ensure that the administrative and clinically designed applications maintain a seamless transaction between the underlying operating systems and application codes. In addition to infrastructure, we offer interface support allowing RPMS applications to communicate with other applications, including those that are/are not supported by IHS (i.e. COTS). When investing in the HIT services at IHS, customers also have the option of utilizing services that support workflow integration, installation, software deployment support, Access to generally available OIT offered training and training materials, helpdesk and user support specifically designed for administrative and clinical end users. 1.1 Resource and Patient Management System (RPMS) Application Services

Inpatient hospital

An inpatient hospital facil ity provides clinical care services including evaluation, stabil ization, treatment, and disposition of patients who require emergency services, and/or additional 24 hour care. Inpatient facil ities will include experienced and licensed health care providers specialized in emergency medicine and inpatient care including nursing, obstetrics/gynecology, surgery, anesthesiology, and respiratory care. Inpatient hospital facil ities also provide ancil lary support services including imaging, laboratory, and pharmacy services. In addition to the inpatient hospital services, these facil ities are also tied to an established ambulatory outpatient clinic. An ambulatory care facil ity supports day-to-day activities specific to improving patient care. There are two primary services required in order to support an ambulatory clinic including administrative and clinical services. The administrative services focus on the day-to-day program management and executive administration of the overall facil ity. Administrative services include business office, central supply, financial management, information technology, office support, quality management, and telemedicine services. The clinical service is the point of initial contact, or entry level, for patients seeking medical attention. Once inpatient hospital facil ity has implemented our HIT services, additional enhancements to improve the software usabil ity, maintain accreditation/legal standards, and address unforeseen issues will be managed with our O&M (Operations and Management) development and patch management services. IHS/OIT/DIT also provides access to LISTSERV groups so that staff can participate in community discussions with other ambulatory sites using our HIT solutions, learn/share best practices, and receive updated information pertaining to new releases or changes that will be delivered in the future. In addition to the LISTSERV forums, we also host an RPMS Feedback webpage that end users can submit directly back to OIT/DIT project teams for additional enhancement request, questions, and general comments as needed. With our HIT services comes the need for additional platform and database support. For platform support, our teams provide additional support services that help ensure proper backups, disk space, journaling, mirroring, projected upgrade needs, volume and operations requirements plus address issues involving user accounts, printer support, data transmission, system failures. The RPMS database services also coordinate with the Division of Program Statistics (DPS) and other content providers on the necessity for and/or implementation of additions, modifications, and deletions to the contents of IHS-SCS. Last but not least, there is additional options for data analytics and quality measurement services.

NON EHR Sites- While most ambulatory outpatient and inpatient facil ities will have behavioral health support services, there are situations in which customers may need a separate facil ity to host more sensitive behavioral health care needs. Due to various federal, state, and local rules/regulations, sites wishing to provide alcohol and substance abuse services will need to have a health information technology system that is separate from their regular outpatient and inpatient care facil ities. Sites hosting alcohol and substance abuse treatment services will focus health care needs specifically on administrative, cl inical, mental health, and social service activities designed to improve the mental wellbeing of their patients. The administrative services include case management analysis supporting the behavioral health department who is responsible for providing treatment focused on the emotional, social, and psychological problems of their cl ients. The clinical services include l icensed providers specialized in alcohol and substance abuse services. The mental health services are designed to serve individual with efforts focused on problems of alcohol and substance abuse. Lastly, social services provide treatment for emotional, social, and psychological problems of their patients util izing knowledge of and contacts with federal, state, and local agencies to improve patient care.

The RPMS Purchased and Refferred Support Service is designed to combine all of the requisite RPMS clinical, administrative, and infrastructure applications with the additional value-added support necessary for IHS Purchased and Referred Care Programs (formerly Contract Health Programs) to run the RPMS successfully based on facil ity size and complexity A detailed depiction of the service follows below. OIT provides the operational maintenance and support to ensure that all existing application software is fully functional and operational.

In order to support a purchased and referred care only facil ity, health information technology (HIT) is essential for managing the day-to-day activities. IHS/OIT/DIT provides a full range of HIT services that help the administrative and clinical services meet the everyday demands that are required when caring for Native American and Alaska Native patients. This includes services focused on new software development util izing Enterprise Performance Life Cycle (EPLC) standards to ensure successful project initiation, software requirements, technical design, development, testing, certification, and release management activities. Our software applications provides an infrastructure foundation to ensure that the administrative and clinically designed applications maintain a seamless transaction between the underlying operating systems and application codes. In addition to infrastructure, we offer interface support allowing RPMS applications to communicate with other applications, including those that are/are not supported by IHS (i.e. COTS). When investing in the HIT services at IHS, customers also have the option of util izing services that support workflow integration, installation, software deployment support, specialized training and training materials, helpdesk and user support specifically designed for administrative and clinical end users. Once a purchased and referred care only facil ity has implemented our HIT services, additional enhancements to improve the software usabil ity, maintain accreditation/legal standards, and address unforeseen issues will be managed with our O&M (Operations and Management) development and patch management services. IHS/OIT/DIT also provides access to LISTSERV groups so that staff can participate in community discussions with other ambulatory sites using our HIT solutions, learn/share best practices, and receive updated information pertaining to new releases or changes that will be delivered in the future. In addition to the LISTSERV forums, we also host an RPMS Feedback webpage that end users can submit directly back to OIT/DIT project teams for additional enhancement request, questions, and general comments as needed. With our HIT services comes the need for additional platform and database support. For platform support, our teams provide additional support services that help ensure proper backups, disk space, journaling, mirroring, projected upgrade needs, volume and operations requirements plus address issues involving user accounts, printer support, data transmission, system failures. The RPMS database services also coordinate with the Division of Program Statistics (DPS) and other content providers on the necessity for and/or implementation of additions, modifications, and deletions to the contents of IHS-SCS. Last but not least, there is additional options for data analytics and quality measurement services.

For this reason it is difficult to provide a single set of recommendations for hardware and network infrastructure that will fit all sites. However, the following general guidelines apply: • Hardware: The RPMS suite runs on mid-range to personal computer hardware platforms. Typical configurations from two RS 6000 computers for large facil ities to one Intel-based Windows NT computer for small facil ities. • Software Applications: RPMS applications operate individually and as an integrated suite. Using the HL7 protocol, RPMS applications can be interfaced with a variety of commercial off-the-shelf (COTS) software products. • Networking Connectivity: RPMS information can be exchanged over a local area network (LAN) within a single facil ity, a wide area network (WAN) with other facil ities, and the Internet with other providers and medical researchers.

Under the Meaningful Use 2015 certification initiative, users from inside the Indian Health Service (IHS) D1 domain (Federal) and outside the IHS D1 domain (Tribal, Urban) would need access to the Resource and Patient Management System (RPMS) Network Services: Master Patient Index (MPI), Health Information Exchange (HIE), RPMS Direct Secure Messaging (RPMS Direct), and Personal Health Record (PHR). Currently HIE and MPI applications are set to utilize the Indian Health Service (IHS) D1 Active Directory (AD) domain to authenticate internal users. However, there will be subset of external users, most notably from Tribal/Urban clinics, who are prohibited by security policy from having their accounts added to the D1 Domain. SAS enables a single set of authentication credentials for tribal and urban users to access IHS HIE (future =RPMS Direct) applications. It is enabled through an open source single sign on tool and an active directory instance (E1).

For E1 users, i.e. Tribal/Urban users not on the D1 AD, this provides authentication (Username and Password) to login to MPI and HIE applications, and Self Service Password Reset (SSPR) services. Via these services, users are provided basic authentication services and related provisioning and technical support.

It is intended to eliminate and reduce the use of fax services, couriers, and postal mails that involve the inherent risks of information being misplaced, compromised, or accessed by unauthorized users. The ePrescribing Support & Maintenance Service is bundled with applications specific to RPMS Pharmacy Package, IHS Electronic Health Record and Intersystems Ensemble productions. This service, in conjuntion with the Surescript eRx network, allows a licensed clinical provider, authorized to write prescription medication, to electronically transmit electronic prescriptions from the certified RPMS Electronic Health Record to pharmacies participating on the Surescripts network (including but not limited to Walgreens, Walmarts, Targets, CVS, etc.).

AIX - the V7000 and SVC are critical hardware components of storage that are used in a variety of production systems. This hardware has a Software component that is required to operate. The components are not dedicated to any singular production application. Both items are required for the continued operability, maintenance, support and security risk mitigation for over 100 production level servers. Nor are these components severable.

The production systems that are supported by this equipment include:

OIT provides consultation on IBM AIX licensing, cost comparisons, and compliance advice.

The software procurement , distribution, and licensing service provides for IBM AIX software procurement needs for OIT. We are responsible for securing the products and keeping the maintenance agreements current over the useful life of the products. We work closely with the Divisino of Aquisitinos and Procurment to negotiate and execute software contracts for use by OIT and business partners and stakeholders. IBM AIX software is procured at an enterprise level - RPMS provides all of the software updates and support to the IBM/AIX platform. . OIT provides support for software updates under warranty.

-- Maintaince support and warranty RPMS provides all of the software updates and hardware support to the IBM/AIX platform. Either IBM or contractors provide support for software updates and hardware repair/replacement directly.

RPMS can run on either Windows or AIX operating systems. Areas and facilities running RPMS on Windows machines pay for those licenses through an enterprise contract with a Microsoft reseller. However, the support for AIX systems is provided through a national contract managed by the OIT.

the health information system that supports the delivery of healthcare services at hundreds of IHS Federal, Tribal, and Urban (I/T/U) operated hospitals and healthcare clinics across the country serving American Indian and Alaska Natives. RPMS, designed specifically for Indian health facilities, includes over 50 software, database, network, and hardware products, all of which are critical to the daily operations of these facilities as well as to regulatory compliance for IHS as a whole. The RPMS applications operate as part of an integrated solution to meet the breadth of standard clinical, administrative, and financial healthcare management requirements. The RPMS suite is continually enhanced to meet the changing needs of the Indian Health community and current compliance requirements. RPMS applications can also be interfaced with a variety of commercial off-the-shelf software products.

RPMS Software Application Suite

RPMS has a broad range of software that supports the whole person. Examples would include unsolved problems escalated up from local (Tier 1) and Area Office (Tier 2) helpdesks, troubleshooting errors in IHS-developed software and the maintenance of OIT-managed servers, e-mail and other messaging services, and WAN equipment and circuits. IHS Area Offices and many IHS sites maintain an IT Help Desk. If a problem is determined to require Tier 3 support, the associated Area IT office is responsible for escalating to OIT for resolution.

HEAT Self-Service

HEAT Self-Service (HSS) is a web-based application that enables you to submit service requests, create incident reports, and get status updates on your tickets without requiring the installation of software or special permissions.Any IHS employee who has a D1 Active Directory account can access HSS and perform the following tasks, without requesting assistance from Help Desk staff:• Submit common service requests.• Submit a new ticket and receive an auto-confirmation of an issue ticket via e-mail.• View your tickets, check the status, and edit to add information if needed.• View and subscribe to current hot issues.• View the history of an issue ticket.

LISTSERV Email Groups

The IHS LISTSERV is an email messaging system that allows all IHS audiences (patients, tribes, employees, contractors, providers, etc.) the opportunity to effectively communicate and collaborate on Indian Health and IHS related topics. Over one hundred lists are available on the IHS LISTSERV with more than 10,000 list subscribers. Each LISTSERV is maintained and moderated by an IHS OIT Subject Matter Expert. Many of the RPMS applications have a LISTSERV dedicated to the users of those applications where individuals can collaborate on user support issues and share their experiences and expertise with the software. Each LISTSERV has a searchable archive that is available to subscribers where all previous LISTSERV entries are accessible. IHS Data Dental System (namespace ADE)

A legacy RPMS application with legacy roll and scroll user navigation capabilities. The IHS Electronic Dental Record is a multi-package solution incorporating the RPMS application suite, the 3rd party Dentrix Enterprise dental practice management software, the 3rd party Dexis Imaging application.

3rd Party Dental Software

IHS provides support for integration of RPMS with the Dentrix Enterprise and with Dexis Imaging as a significant component of IHS’s Electronic Dental Record (EDR) software application package. Dentrix Enterprise is a commercial off-the-shelf (COTS) dental clinical and practice management software application that integrates with RPMS or can be used as a stand-alone application for tribal/urban programs not using RPMS. The EDR will provide all the necessary clinical and dental practice management functionality needed by I/T/U dental programs.

Dentrix

Dentrix Enterprise is a dental practice management application for use in dental clinics, health centers, and a variety of other health care environments with dental health facilities. Dentrix Enterprise provides a centralized database and centralized billing, insurance, scheduling, collections, and reporting capabilities including federally required Uniform Data System (UDS) reports. It supports multi-site clinics and multiple workstations with security features that provide real time access to network usage statistics through one main server. According to the National Network for Oral Health Access (NNOHA), Dentrix Enterprise is a leading EDR in use at a large number of community health centers. The RPMS application name space of the IHS EDR is (BADE).

RPMS DIRECT Messaging service

Provides sites and patients continuous access to RPMS DIRECT Messaging (RPMS DIRECT, namespace BRDM), a standards-based, secure, web-based messaging system which complies with the Office of the National Coordinator for Health Information Technology (ONC) guidelines. The secure messaging provided by RPMS DIRECT is certified according to the 2014 version ONC requirements for Certified EHR Technology. RPMS DIRECT is intended for the exchange of patients’ health information between healthcare providers who regularly transmit and/or receive Protected Health Information/Personally Identifiable Information (PHI/PII), and for communication between patients and their health care teams. RPMS DIRECT users include healthcare providers, healthcare organizations, message agents, patients, and patient representatives. RPMS DIRECT is the name of the secure email system and is separate from a user’s other personal email accounts. RPMS DIRECT should only be used for health-related messages. Messages can be sent to and received from other trusted DIRECT addresses only. Provider staff can access RPMS DIRECT within the Electronic Health Record (EHR). Patients can access RPMS DIRECT within the PHR application through the My Messages menu.

MPI - Master Patient Index

The Master Patient Index is an RPMS application that identifies patients across separate clinical, financial and administrative systems and is needed for information exchange to consolidate the patient list from the various multi-site RPMS databases. With the implementation of MPI, each I/T/U facility record for a patient will also belong to an enterprise record for that patient created by the MPI. The MPI generates a unique patient ID (called an EUID) for each enterprise record usable by the HIE and eHealth Exchange to exchange information between I/T/U facilities and with external entities.

HIE - Health Information Exchange

The Health Information Exchange allows users to securely access, retrieve and move electronic healthcare information between among disparate healthcare systems or region. In context, HIE allows RPMS I/T/U users to access clinical healthcare documents from across multiple other I/T/U facilities using EUID’s created by the MPI.

eHealth Exchange

The eHealth Exchange is a public-private alliance of government and private organizations that facilitates the exchange of health information over secure, trusted connections (or health information exchange) between participating organizations. eHealth Exchange is formerly known as the Nationwide Health Information Network (NwHIN). These organizations include federal agencies, public and private Health Information Organizations (HIO's), health systems, and other healthcare related organizations. The eHealth Exchange allows: exchange of health information between participating organizations, standardized information exchange protocols, health information search and request features between organizations, and health record update alerts. As implemented at IHS, implementation of eHealth Exchange extends the HIE capability, allowing I/T/U facilities to exchange health data exchange with other external organizations over the eHealth Exchange national health information network.

Personal Health Record (PHR)

The RPMS Personal Health Record is an online tool that provides patients with direct access to their medical record information through the website address http://phr.ihs.gov . The purpose of the PHR is to increase a patient’s engagement in their healthcare and to improve health communications with the health care team.PHR is a centralized web application that runs separately from the core RPMS application suite but can integrate with certain modules of RPMS (see below). No remote installation is necessary for PHR to run. Information in the interface comes from the RPMS HIE (or from another HIE application). Facilities desiring to run the PHR must onboard to the IHS MPI and IHS HIE, each of which is dependent on the presence of the 2014 Certified EHR, Patient Registration and PCC (HIE). If the site already has IHS MPI and HIE, etc., then no additional configuration or "installation of PHR" will be needed.

ePrescribing

The ePrescribing application provides IHS, Tribal, and Urban (I/T/U) sites using the IHS certified RPMS/EHR software with the ability to transmit electronic prescriptions to over 60,000 private pharmacies that subscribe to Surescripts. The Indian Health Service and other ePrescribing vendors across the nation have adopted ePrescribing to improve prescription workflows, enhance medication reconciliation, and help to ensure patient safety. National data has shown an increase in prescriber adoption along with statistics to support that ePrescribing reduces healthcare costs, improves patient safety, and increases efficiency when it comes to processing prescriptions.

Behavioral Health

Serious behavioral health issues such as substance use disorders, mental health disorders, suicide, violence, and behavior-related chronic diseases have a profound impact on the health of American Indian/Alaska Native (AI/AN)individuals, families, and communities. Through its programs, the IHS Division of Behavioral Health addresses these issues and follows the current Indian health care system focus on integrated behavioral health and primary care treatment that respects the balance, wellness, and resilience of AI/AN people.
