OFFICE HOURS Thursday, March 26 1:00 pm till 8:30 pm. Friday, March 27through Sunday, from 7:30 AM – 15 minutes after the last class. The office maybe closed during lunch and dinner breaks, and will reopen 30 minutes before thestart of the next session.

POST ENTRIES will be accepted up to the end of the session prior to theclass you wish to enter. To ensure correct reporting of results, pleasecomplete rider change & scratch forms before the respective classsession.

ENTRIES must be submitted on the official entry form and bepostmarked no later than March 11, 2020 for inclusion in the showprogram. There is no post entry charges for the show. Entries mustbe paid in full when submitting your entry; post dated checks wil l notbe accepted. An open check may be left in the show office whenchecking in to cover post entries and other incidentals. Entries willnot be taken over the phone or by fax. Entr ies wil l not be acceptedwithout payment.

FARM AND BUSINESS NAMES can be used only if it is the owner appearingon the horse’s registration papers. In that case, the farm or business must be abusiness member of AHA and a copy of that card enclosed with the entry.

HORSES ENTERED IN AMATEUR OWNER/JUNIOR OWNER CLASSES mustbe registered in the name of the competitor or a member of the competitor’sfamily, as defined by USEF GR123. Horses registered in afarm/ranch/syndicate/partnership/corporation name may be shown in ownerclasses provided the family is the sole owner of the farm or entity as defined byUSEF GR141. A contract or bill of sale cannot be accepted as ownership foramateur owner and junior owner classes.

REFUND POLICY: Management agrees to refund entry and stabling fees onany horse not fit to show provided written notice is given on or before March 18,2020. After March 18 and prior to Thursday, March 26th at 7 pm, withdrawalmust be accompanied by a veterinarian’s certificate. In such cases, a refund ofclass entry fees will be given less a $20 office charge per horse. Stall andtack/ready room fees will not be refunded. There will be no refunds afterThursday March 27th at 7 pm.

ENTRY FORM MUST

I be signed by the exhibitor, rider, driver, handler, coach (if applicable) andtrainer. In the case of a rider under the age of 18, his/her parent orguardian must sign on the minor‘s behalf. All entries must have thetrainer line signed, as that is the adult person who has care andcustody of the horse at the show.

I include the address, phone number and email of owner and trainer.

I include current USEF Amateur and AHA competition membership numbersfor each owner, trainer, rider, handler, lessee, agent and coach.

INCLUDE WITH YOUR ENTRY

I a copy of registration papers for each horse, including contract or bill of sale ifhorse is not in exhibitor’s name. (Horses being purchased may only beshown under new owner’s name if a copy of the contract is included.) Toshow in AOTR, AAOTR, and JOTR classes the horse must be registered inname of owner or member of family.

I a copy of USEF Competing Equestrian Amateur membership cards and AHACompetition membership cards for each owner, trainer, rider, driver,handler and coach. The online USEF membership card will show SafeSport Training completion.

STABLING INFORMATION

ARRIVAL AND DEPARTURE: Stall fees are for arrivals Thursday March 26, at 8AM and departures completed on Sunday, March 29. Exhibitors may set up and bedstalls in assigned stabling areas on Wednesday, March 25 at no extra charge.Horses may not be brought in until Thursday at 8 AM. Early arrival of horses isavailable by contacting show manager, Wendy Irvin, 360-794-8960, to allow thefairgrounds to open the appropriate stalls. Early arrival is $25 per day per horse stall.

BEDDING will be supplied by show management. A $30 shavings and shavingsremoval fee will be charged per horse. This procedure is more cost effective asopposed to purchasing bagged shavings.

FIRE MARSHALL RULES: By order of the Fire Marshall, all aisles between stallsmust remain open. All materials used for decorative purposes must be treated withflame retardant; this includes stall drapes, even those made of cordura. Please beprepared to present evidence of this to the Snohomish County Fire Department.The Fire Marshall will inspect the show grounds. Authorized electrical appliances forgrooming purposes may be used. The electrical system may not be altered ortampered with, under any circumstances. If electrical cords are used, they must beof the heavy duty, outdoor type. SMOKING IS NOT ALLOWED IN THE ARENASOR BARN AREAS. Shavings may not be used in aisles or barn walkways as theyare a fire hazard.

SHARPS CONTAINERS have been placed throughout the barns. Please use them.Competition management will fine any individuals $300 ($100 USEF & $200 facility)for improper disposal of needles or other sharp disposable instruments.

STABLING ASSIGNMENTS: One stall per horse, except mares with foals. Stallassignment made upon consideration of sponsorships, volunteer status, andcontinued support in exhibiting at our competition. No stabling changes withoutconsent of barn manager. The decision of management is final. Exhibitors wishingto be stabled together must submit their entries together in the same envelope. Ifyou have special requests or considerations, please submit them with your entry. Allrequests will be considered, but no promises are implied. Stabling assignments canbe viewed online at www.northwestheritage.org on Tuesday March 24. 2020.

STALL AND BARN CLEAN UP: Exhibitors are responsible for cleanup anddisposal of materials other than bedding used in stalls. Exhibitors must removedecorations following the show and prior to departure. This includes any materialsplaced in aisles or outside barns or materials used for decorative purposes. Thecost of clean-up will be $40 per hour and $5 per yard dumping fee for cleanupand disposal of materials other than actual stall bedding

STALL CONSTRUCTION: Wood/metal with wood/metal dividers. Half or full doors.

STALL AND BARN DAMAGE: Stalls may not be structurally altered in any way.This means no removal of fronts or tear down of sides to make larger areas. Donot put nails, screws or other types of hole producing devices into stall walls. Theuse of staples, tacks or tape to attach decorations to walls, buildings, stalls, etc. isprohibited. Horse hooves are not to be painted on unprotected barn floor andaisles. Exhibitors are financially responsible for any damage to stalls or barns andwill be charged accordingly. Barns and stalls will be inspected for damage andalterations.

GENERAL RULES AND INFORMATION

EVERY CLASS OFFERED HEREIN WHICH IS COVERED BY THE RULES AND SPECIFICATIONS OF THECURRENT USEF RULE BOOK WILL BE CONDUCTED AND JUDGED IN ACCORDANCE. TO EXHIBIT IN THEAMATEUR CLASSES A USEF AMATEUR MEMBERSHIP IS REQUIRED.

USEF Vaccination Rule (GR 845) eff. 12/1/15: All horses on the grounds must have current vaccinations for flu and rhino. ExhibitorsMUST have proof of vaccination in their possession for each of their horses at the show.

ABUSE, CRUELTY, CONDUCT: USEF GR839, AR105, and AHA Article 703 will be strictly enforced.

AATR SELECT, CHOICE & ELITE, AND JTR SELECT CLASSES: Riders will be allowed to “level up” at this show. A rider may enter their level (see yourlevel at www.arabianhorses.org) and a level up from that. For example, a Select rider in Western could also enter the Elite class. Riders may also enter theAAOTR class if they meet the USEF owner definition. Juniors, who are Select riders, may enter JTR Select classes as well as JTR and, if owners by USEFdefinition the JOTR.

ADMISSION TO THE ARENA: No person may enter the space set aside for the judging or hold any communication with the judge or examiningveterinarian without permission of the steward or show management, who will arrange an appointment with the judge at the proper place and time.

AGE OF HORSE AND RIDER: For horse show purposes a horse is considered to be 1 year of age on the first day of January following actual foalingdate. A junior horse is 5 years old and under. Junior exhibitors are those who have not reached their 19th birthday by Dec 1 of the previous year.

AHA MEMBERSHIP STATEMENT: EACH COMPETITOR (THIS INCLUDES AT LEAST ONE RECORDED OWNER AND ALL TRAINERS,DRIVERS, RIDERS, HALTER HANDLERS, COACHES AND THE PERSON(S) SIGNING THE ENTRY BLANK) IN AN ARABIAN HORSEASSOCIATION RECOGNIZED ARABIAN DIVISION, AT THE TIME OF ENTRY INTO A COMPETITION, MUST SUBMIT A COPY OF A VALID AHAMEMBERSHIP/COMPETITION CARD, JOIN AHA OR PAY AN AHA SINGLE EVENT MEMBERSHIP FEE PER COMPETITOR. WHEN AN AGENTSIGNS FOR THE OWNER AT AN AHA RECOGNIZED COMPETITION, BOTH THE OWNER AND AGENT MUST BE MEMBERS OF AHA OR PAYTHE AHA SINGLE EVENT MEMBERSHIP. SINGLE EVENT MEMBERSHIP IS NOT ACCEPTABLE AT AHA REGIONAL OR NATIONALCOMPETITIONS. EXHIBITORS IN WALK-TROT CLASSES ARE EXEMPT FROM AHA MEMBERSHIP REQUIREMENTS AT QUALIFYINGCOMPETITIONS, BUT MUST HAVE AN AHA MEMBERSHIP WITH COMPETITION CARD AT AHA REGIONAL AND NATIONAL COMPETITIONS.EXHIBITORS IN LEAD LINE CLASSES OR PARENTS/GUARDIANS SIGNING FOR MINORS ARE EXEMPT FROM AHA MEMBERSHIPREQUIREMENTS. (BOD 5/12)

ARENAS: Arena footing is sandy loam or sand. Measurements are: Main arena—120’ x 248’, covered warm up—100’ x 200’, front sand ring—128’ x 254’,NE sand ring—130’ x 200’, sand ring between barns 105 and 106—73’ x 164’, as well as several sand lunge rings.

AWARDS: Junior exhibitor classes will be awarded ribbons to eight places. All other classes will have ribbons to six places.

AMATEUR OWNER “FAMILY” DEFINITION: Horses entered in an amateur owner and junior owner classes must be registered in the name of thecompetitor or a member of the competitor’s family as defined by USEF Amateur Owner classes provided the family is the sole owner of the farm or entityas defined by USEF rules). The sale of a horse does not eliminate this registration requirement, Contracts of Sale or Bill of Sale will not be accepted forowner classes. Leased horses may not be shown by the lessee in amateur/owner classes. Combined ownership is allowed in amateur/owner classes onlywhen these owners are members of the same family.

USEF ANATEUR CERTIFICATION FOR EC AMATEUR MEMBERS: EC amateur members, competing in amateur classes, must complete andsign a USEF membership application verifying they meet USEF amateur requirements. There is no charge.

BIKES, MOPEDS AND ROLLERBLADES: Close calls between children, cars and horses have caused concerns. Please keep in mind this is a horseshow facility, not a playground. Please supervise children when they are riding bikes and leave rollerblades at home. NW Heritage and the EEP will not beresponsible for injuries or damage caused by such activities.

BREEDING/IN-HAND SCORING SYSTEM: COMP 815. CLASS PROCEDURE FOR COMPARATIVE JUDGING IN BREEDING IN-HAND (Res. 20-13) 1.Class Procedure for AHA Recognized shows other than Regionals and Nationals ; a. Judging begins when the first horse enters the ring. From the momentthe horse enters the ring its movement is being evaluated. The horse shall enter at a trot, and at the handler’s discretion may either proceed directly alongthe rail, or trot a counter-clockwise circle at the in-gate end of the arena and then proceed along the rail to the far end of the arena. Upon each horsereaching the designated position at the far end of the arena they are to remain relaxed. The judge(s) will then ask for the next horse to enter the ring. Whenall horses have entered the ring the horses shall be asked to walk collectively counter-clockwise and/or clockwise past each (and all) judge(s). They shallwalk in a relaxed manner on a loose lead with the handler at the side of his horse, whips down. The lead line must maintain a discernible drape and thehandler must not place his hand on the chain or close enough to in any way restrict the natural head and neck motion. The handler may not unnecessarilyimpede the forward motion of the horse while at the walk. Following the cessation of the walk all horses shall relax within the ring and only the horse “ondeck” shall be prepared to show. Each horse shall be led to the judge(s) at the walk and presented for judging. In order to satisfy a proper presentation tothe judge, horses should stand correctly, bearing weight on all four feet (not stretched), in a quiet and deliberate manner. Following the evaluation by thejudge(s), horses shall walk away from the judge(s), and then strike a trot before turning left and proceeding counter-clockwise down the rail. They shallresume their relaxed position at the end of the arena. b. Handlers are expected to keep a reasonable clearance between horses and judges should modifythe alignment of horses to achieve that clearance. Further judging will not commence until the Call Judge approves the placement and spacing of allhorses, c. After the last horse is presented, the adjudication process is complete and horses will not be further compared. 2. That in the event that anycomparative judging is used, in a one (1) judge system, judge shall give oral reasons for their decisions after the card is handed in. In the event ofconcurrent shows the two (2) judges shall alternate classes in giving reasons.

CHANGE OF HANDLER/RIDER: It is the obligation of the exhibitor to notify the show office of any change in rider, driver, handler or cancellation of entry.

CLASS CONFLICTS: Exhibitors are cautioned against entering the same horse in successive classes. Due to full schedules, in-gates will not be heldfor riders with conflicting schedules. No refunds will be given for class conflicts.

CLASS SIZE: Three entries constitute a class. Management reserves the right to cancel or combine classes that have not filled, to acceptentries for unfilled classes at regular entry fees, and to reject an entry if in the best interest of the show.

DISCIPLINE: Exhibitors are warned that any act of discourtesy or disobedience to the officials on the part of the owner, trainer, groom, or riderwill disqualify the horse from adjudication. The show committee reserves the right to remove a horse, owner, rider or groom from the show without beingliable for compensation or damages. It is a violation of USEF rules to commit any act or make remarks during the show that are considered offensive ormade with the intent to influence or cast aspersions on the judging, and exhibitors may be barred from the show for this offense.

DOGS: Dogs are not allowed within the fairgrounds fences, barns or arenas. They may be in the RV areas.

FARRIER: There is no farrier scheduled to be on the grounds.

FEED: Feed is available from a number of area retailers. Check at the office for delivery services.

FOOD: Food and beverages will be available from concessions on the grounds during show hours.

LUNGEING: Lunging arenas will be provided, and will be marked. Lungeing is not allowed in the main ring, the covered outdoor ring, the front sand ring,or the northeast sand ring.

MOTORIZED VEHICLES: Minors who do not have a valid driver’s license which allows them to operate a motorized vehicle in the state in which theyreside will not be permitted to operate a motorized vehicle of any kind including, but not limited to, golf carts, motorcycles, scooters or farm utility vehicleson the competition grounds. Minors with a valid temporary license may operate the above described vehicles as long as they are accompanied by an adultwith a valid driver’s license. The parent(s), legal guardian(s), or individual who signs the entry blank as a parent or guardian of a minor operating amotorized vehicle in violation of this rule are solely responsible for any damages, claims, losses or actions resulting from the operation.Violations of this rule will b e cause for sanctions against the parent(s), guardian(s), and/or trainer(s) responsible for the child committing the offense.Penalties may include exclusion of the child, parent(s), guardian(s) and/or trainer(s) from the show grounds for the remainder of the competition andcharges being filed against any of the above individuals in accordance with Chapter 6. Wheelchairs and other mobility assistance devices for those withdisabilities are exempt from this rule. Proof of Insurance must be available for golf carts, scooters, and motorcycles.

PARKING: Parking is not allowed in the fenced fairgrounds area. Trucks and trailers must be removed immediately after unloading. Only emergencyvehicles, vet and farrier will be allowed in the barn area. A fire lane must be maintained at all times.

PERSONS WITH DISABILITIES: If you are disabled, please write a brief explanation of your disability and any special needs that you may have. Pleaseinclude this with your entries

PROTECTIVE HEADGEAR: It is compulsory for all junior riders in hunter seat equitation classes, to wear properly fastened protective headgear whichmeets or exceeds ASTM (American Society for Testing and Materials) / SEI (Safety Equipment Institute) standards for equestrian use and carries the SEItag. It must be properly fitted with harness secured. Show committee must bar riders without protective headgear from entering the ring for classes inwhich protective headgear is required.

RETURNED CHECKS: There will be a $50 fee per incident for any check dishonored for any reason. It is the policy of this organization to pursue allavenues to make collection on checks. In the event a check is returned by the bank after the completion of the show, it will be handled in accordance withUSEF GR913.3.

RV/CAMPER HOOK-UPS: There are limited hook-up spaces located in RV 1 north of the Equestrian Park, and 10 hook-up spaces with septic serviceslocated east of the Equestrian Park, available on a first come first serve basis. Please pay for RV on Official Entry Blank. Day fees: $35 with electrical,water, and septic, $30 with electrical and water, and $20 for trailer camper with no hookup. Any RV taking up more than one space will be chargedfor a double space.

STALLIONS: Arabian stallions may be shown by ladies and junior exhibitors in any class in this show except showmanship, lead line and walk-trot. HalfArabian and Anglo Arabian stallions registered with AHA or which have been issued an AHA Certificate of Registration issued by the Canadian PartbredArabian Register are permitted to compete in AHA performance classes for Half Arabian and Anglo Arabian horses.

TBA Classes: Any class on the AHA Class List or recognized by AHA maybe designated a TBA class. (Exception: dressage, cutting and reined cowhorse. Sport horse classes dependent on section.)

TRAIL SCHOOLING: Schooling on the trail course will be held Thursday afternoon/evening (entry acknowledgment will include times). Pay $5 at the gate(cash only) for a ten minute individual work. One round per horse until all who wish to go have gone. Time permitting, extra rounds may be purchased.

There is a $5 entry fee to compete for the high point. The rules are easy:

1st Place – 6 points, 2nd Place 5 points, etc.

We are going to have the Silent Auction again!

We have ATR Jackpot classes for both Arabian and Half Arabian Hunter and WesternPleasure. The entry fee is $30. $15 of each entry goes to the Jackpot. The class will beplaced as usual to provide qualifying points. The Jackpot will be split 1st 50%, 2nd 30%and 3rd 20%.

AHA ENTRY AGREEMENT I have read the rules concerning competitions as printed in the Arabian Horse Association (AHA) Handbook / Directory and Competition Prize List and agree to be bound by and subject to those Rules.

AHA ASSUMPTION OF RISK, RELEASE AND INDEMNIFICATION This document waives very important legal rights. Read it carefully before signing.

In consideration for AHA permitting me to participate in this Competition, and by signing the entry blank, I agree as follows: I AGREE that I choose to participate voluntarily in this Competition, as a rider, driver, handler, lessee, owner, agent, coach, trainer, junior exhibitor, or as a parent or guardian of a junior exhibitor. I AM FULLY AWARE AND ACKNOWLEDGE THAT HORSE SPORTS AND PARTICIPATION IN THIS COMPETITION INVOLVE SERIOUS RISK OF HARM INCLUDING, BUT NOT LIMITED TO, RISKS OF ACCIDENT, SERIOUS BODILY INJURY, INCLUDING DEATH, BROKEN BONES, HEAD INJURIES, TRAUMA, PAIN, AND SUFFERING, AND PROPERTY DAMAGE. I ASSUME ALL RISKS OF HARM TO ME, MY HORSE OR MY PROPERTY. I AGREE for myself, my heirs, executors, administrators, successors and assigns to release AHA, the Competition, the facilities leased by the Competition and the owner(s) of the facilities, and all of their respective officers, officials, directors, employees, agents, personnel, volunteers, affiliated organizations and insurers (collectively, the "Released Parties") from any and all claims for damage, loss, or injury to myself, other persons, horses or other property belonging to me to the fullest extent permitted by law that arises out of or relates in any way to the Competition and my participation in the Competition INCLUDING, BUT NOT LIMITED TO, DAMAGES, LOSS, OR INJURY RESULTING FROM ANY ACTS, FAILURE TO ACT, NEGLIGENCE OR NEGLECT OF OTHER ENTRANTS, THE RELEASED PARTIES, THEIR CONTRACTORS OR INVITEES, as well as for theft, vandalism, fire, other casualty damage, or damage arising out of any defects in the premises. I AGREE to indemnify and hold harmless (that is pay all losses, damages, attorneys fees and costs of) the Released Parties from and against any and all claims, demands, penalties, actions, losses, costs, damages, injuries, liabilities and obligations (including attorneys fees) of whatsoever kind and nature, which may be asserted against or incurred by any of them as a result of (1) my participation in the Competition or (2) any act, failure to act, or neglect (a) by me, my agents, employees, riders, handlers, trainers, coaches, drivers, contractors or invitees, or (b) by any animal owned or exhibited by me or in my custody or control. I AGREE and represent that I am qualified and eligible to enter and/or participate in the Competition, and every horse I am entering is qualified and eligible as entered. I AGREE to accept AS FINAL any decision of AHA, the Show Commission or Show Officials concerning my qualification or the qualification of my horse to enter the Competition or any results of the Competition, except to the extent that the Rules of AHA, the Competition, CEF or USA Equestrian permit a protest or hearing of such decisions. Should a hearing be requested, I agree to accept AS FINAL the decision of the particular hearing body. I agree to release, hold harmless and not to sue AHA, the Competition Sponsor, their officers, directors, employees, volunteers or members concerning any decision of AHA, the Competition, its Show Commission, Show Officials or any hearing body that relates to my qualifications or my horse(s)’ qualifications to enter the Competition or any results of the Competition. I AGREE that AHA has the sole right to control, sell, supervise or give away (or assign to others the right to do so) the exclusive rights to broadcast, televise, reproduce, transmit and disseminate all or part of this event, and I agree that AHA may use or assign, in any way AHA sees fit, photographs, films, videos, audios, cablecasts, or other likenesses of me and my horse taken during the course of the Competition for the promotion, coverage or benefit of the Competition or AHA. Those likenesses shall not be used to advertise a product and they may not be used in such a way which implies endorsement of any company, product, product category or service. I hereby expressly and irrevocably waive and release any rights in connection with such use, including any claim to compensation, invasion of privacy, right of publicity, or to misappropriation. By signing below as a parent or guardian of a junior exhibitor, I consent to the child’s participation and agree to all of the above provisions, and further agree to assume all of the obligations of this AHA Assumption of Risk, Release and Indemnification personally and on behalf of the child. This AHA Assumption of Risk, Release and Indemnification is governed by the Laws of the State of Colorado and is intended to be interpreted as broadly as possible. I agree that exclusive jurisdiction and venue (place) for any legal action against AHA, its officers, directors, employees, volunteers or agents shall be in the local district courts or the federal court of the State of Colorado. If any part of this agreement is determined to be unenforceable, all other parts shall remain effect.

Federation entry agreement By entering a Federation-licensed Competition and signing this entry blank as the Owner, Lessee, Trainer, Manager, Agent, Coach, Driver, Rider, Handler, Vaulter or Longeur and on behalf of myself and my principals, representatives, employees and agents, I agree that I am subject to the Bylaws and Rules of The United States Equestrian Federation, Inc. (the “Federation”) and the local rules of this Competition. I agree to be bound by the Bylaws and Rules of the Federation and of the competition. I will accept as final the decision of the Hearing Committee on any question arising under the Rules, and agree to release and hold harmless the competition, the Federation, their officials, directors and employees for any action taken under the Rules. I represent that I am eligible to enter and/or participate under the Rules, and every horse I am entering is eligible as entered. I also agree that as a condition of and in consideration of acceptance of entry, the Federation and/or the Competition may use or assign photographs, videos, audios, cable - casts, broadcasts, internet, film, new media or other likenesses of me and my horse taken during the course of the competition for the promotion, coverage or benefit of the competition, sport, or the Federation. Those likenesses shall not be used to advertise a product and they may not be used in such a way as to jeopardize amateur status. I hereby expressly and irrevocably waive and release any rights in connection with such use, including any claim to compensation, invasion of privacy, right of publicity, or to misappropriation. The construction and application of Federation rules are governed by the laws of the State of New York, and any action instituted against the Federation must be filed in New York State. See GR908.4. I am fully aware and acknowledge that horse sports and the Competition involve inherent dangerous risks of accident, loss, and serious bodily injury including broken bones, head injuries, trauma, pain, suffering, or death. (“Harm”). I AGREE to hold harmless and release the Federation and the Competition from all claims for money damages or otherwise for any Harm to me or my horse and for any Harm of any nature caused by me or my horse to others, even if the Harm arises or results, directly or indirectly, from the negligence of the Federation or the Competition. I AGREE to expressly assume all risks of Harm to me or my horse, including Harm resulting from the negligence of the Federation or the Competition. I AGREE to indemnify (that is, to pay any losses, damages, or costs incurred by) the Federation and the Competition and to hold them harmless with respect to claims for Harm to me or my horse, and for claims made by others for any Harm caused by me or my horse while at the Competition. I have read the Federation Rules about protective equipment, including GR801 and, if applicable, EV114, and I understand that I am entitled to wear protective equipment without penalty, and I acknowledge that the Federation strongly encourages me to do so while WARNING that no protective equipment can guard against all injuries. If I am a parent or guardian of a junior exhibitor, I consent to the child’s participation and AGREE to all of the above provisions and AGREE to assume all of the obligations of this Release on the child’s behalf I represent that I have the requisite training, coaching and abilities to safely compete in this competition. I AGREE that if I am injured at this competition, the medical personnel treating my injuries may provide information on my injury and treatment to the Federation on the official USEF accident/injury report form.
