Needs and Target AudienceBased on its review of participant activity evaluations from the 2008 conference, as well as recent articles in the medical literature, and alsorecent Pediatric Anesthesiology regional and national meetings, the planningcommittee has identified the need in anesthesiologists and CRNA’s for:• Discussion on the use of new devices and techniques for regional analgesia
in labor• Discussion of new thought and innovations in post operative analgesia in
obstetrics• Discussion of development of chronic pain following cesarean section and
ways to avoid development of plasticity and chronic pain• Discussion of ethical dilemmas and anesthetic challenges in hemorrhagic
parturients in Jehovah Witness patients and the use of cell saver• Applying evidence-based medicine to everyday practice of obstetric
anesthesia• Discussion of lessons learned from real world obstetric cases• Discussion of ASA Closed Claims Analysis – Medicolegal issues and lessons
learned from these cases• Discussion of post-operative pain management and new regional
anesthesia techniques in pediatrics• Discussion of out of operating room anesthesia for pediatric patients• Discussion of malignant hyperthermia in pediatrics• Discussion of anesthesia for the ex-premature infant• Discussion of common post-anesthesia care issues in pediatrics
The goals of this conference are to make the participant aware of the evolvingpractices in obstetric anesthesia, and to address the introduction of newdevices and techniques for labor analgesia, selected aspects of postpartumcare, i.e., use of new long-acting epidural opioid and prevention of chronic painresulting in social and medical issues. The conference will also highlight thehotly debated “pay for performance” topics such as intraoperative control oftemperature, glucose homeostasis and the use of “pencil point” spinal needles.The conference will also highlight medical issues in obstetrics such as management of “Pres Syndrome,” difficult airway, anaphylaxis and substanceabuse in pregnancy, and to discuss maternal mortality, resuscitation and ethicalissues such as informed consent, patient safety, quality improvement and professional liability in obstetric anesthesia. This conference will also includeseparate topics in pediatric anesthesiology, to make the participant aware ofnew developments in pediatric pain management, regional anesthesia, andsedation techniques for diagnostic and therapeutic procedures outside theoperating room. There will be ample opportunity to interact with expert pediatric anesthesiologists during panel discussions, and during a special “Ask theExperts” panel on pediatric anesthesia.
Educational Methods and ObjectivesThrough the use of lectures, presentations, panel discussions, and a syllabus,the participants should be able to explain and discuss:• Evolving practice of using combined-spinal epidural in all laboring patients• Awareness of increasing incidence of obesity resulting in new design of
operating rooms• Regulatory issues: JCAHO requirements to prevent neuraxial infection,
maintenance of normothermia and glucose homeostasis in all patients• Lessons learned from quality assurance, real patient complications• New thoughts and new techniques to manage hemorrhage• Lessons learned from obstetric anesthesia malpractice claims• Analysis of maternal mortality and new thoughts on maternal resuscitation• Real world issues in obstetric anesthesia with practical solutions• Evolving practice of sedation and anesthesia for out-of-operating room
procedures in pediatrics• New techniques for pain relief and regional anesthesia in pediatrics• Current anesthetic approaches to the pediatric patient with an upper
respiratory infection• Update on malignant hyperthermia incidence and management in pediatrics• Common post anesthesia care problems in pediatrics• Current issues in pediatric anesthesia

Needs and Target AudienceThe planning committee reviewed comments from previous workshop participants which indicate the value of the workshop. The committee alsoreviewed recent regional and national scientific and educational meetingcontent, and pediatric anesthesia published medical literature, to determinethe need for increased education and hands-on training in the pediatric difficult airway.
One source of data demonstrating the need for improved performance inthe management of the difficult airway (DA) is the American Society ofAnesthesiologists (ASA) Closed Claims Project database, a standardizedcollection of case summaries derived from professional liability insurancecompanies closed claims files. The analyzed data published in summaryarticles provide a unique tool not only for reducing malpractice liabilityexposure for the anesthesia practitioner, but also for improving patient safety and health outcomes. Pediatric anesthesia patients are also includedin this database. Emerging data also shows adverse outcomes followingextubation of difficult airways, which is another area of focus for futureimprovement in patient safety.
In recent decades, video techniques have been employed in the majority ofintubations because of several distinct advantages provided. The anatomyis magnified, recognition of the anatomical structures and abnormalitiescomes earlier, and manipulation of airway devices is facilitated. Whenassistance is required, the operator and assistant can coordinate theirmovements because each sees exactly the same image on the video monitor. As a result, video techniques have become the method of choice in teaching. This is another area in which we are trying to focus on improvement in patient safety.
Given the significant potential adverse outcomes, this workshop will provideDA management training, as well as provide a refresher course for theanesthesia practitioner. National experts in airway management of bothadult and pediatric patients will be presenting practical updates in smallinteractive groups regarding use of proven and new airway devices in themanagement of difficult and failed airway.
Educational Methods and ObjectivesThrough the use of lectures, presentations, a syllabus, and hands-onworkshop sessions, the participant should be able to explain and discuss:• Use of various educational tools, simulator, and methodology used in
• Use of proven and new supraglottic devices, fiberoptic scope, andvideo laryngoscopes in managing difficult airway
• Quality assurance analysis of adverse cardiac and neurological outcomes following failed intubation
• Airway laser surgery in pediatrics• Advanced airway techniques in pediatrics, including fiberoptic
intubation, supraglottic airways, and other devices specifically for pediatric patients
