On May 7, 2008, the State Board of Education adopted the FORT as a requirement for anyone applying on and after July 1, 2009, for issuance of:

Elementary Education K-6 (#013) or Integrated early childhood/Special Education Nursery –K and

Elementary 1-3 (#113)

Effective September 1, 2013, the FORT or a comparable test, as adopted by the State Board of Education became a requirement for issuance of:

Comprehensive Special Education (#165) Remedial Reading and Remedial Language Arts (#102) Reading and Language Arts Consultant (#097)

Required for a first-time endorsement, a cross-endorsement, or reissuance of a lapsed certificate with one of these endorsements Slide 3

The local or regional board of education that employs a certified individual who holds an initial, provisional or professional educator certificate with an early childhood nursery through grade three or an elementary endorsement in a position requiring such an endorsement in kindergarten to grade three, inclusive, shall require each such certified individual to take a survey on reading instruction, developed by the Department of Education that is based on the reading instruction examination approved by the State Board of Education on April 1, 2009, or a comparable reading instruction examination with minimum standards that are equivalent to the examination approved by the State Board of Education on April 1, 2009. The department shall design such survey in a manner that identifies the strengths and weaknesses of such certified individuals in reading instruction practices and knowledge on an individual, school and district level. Such survey shall be administered at no financial cost to such certified individual and in a manner that protects the anonymity of such certified individual. The results of such survey shall not be included as part of any summative ratings for performance evaluations, conducted pursuant to section 10-151b, as amended by this act, and not be subject to disclosure under the Freedom of Information Act, as defined in section 1-200. Such results shall be used for the purpose of improving reading instruction by developing student learning objectives and teacher practice goals that will be included in the professional development conducted pursuant to section 10-148b, as amended by this act, for such certified individuals. Who must take it?

Any teacher holding a certificate endorsed in one of the following areas and serving in a position requiring such an endorsement:

◦ Integrated ECE/Sped. Nursery –K and Elementary 1-3 (#113)

◦ Elementary education (#001, 002, 003, 004, 005, 008, 013, or 305)

◦ Bilingual elementary education (#009, 902)

Teaching in Grades Kindergarten, 1, 2 or 3

Commences in the 2014-15 school year and biennially thereafter

Why/Purpose?Identify the strengths and weaknesses in reading instruction practices and knowledge

Improving reading instruction by developing student learning objectives and teacher practice goals that will be included in the professional development

Circular Letter to Superintendents disseminated June 4, 2014

Appointment of a District Coordinator / Liaison to the CSDE by June 13, 2014

Dedicated CSDE webpage where district liaisons will be able to access and/or download the following:

◦ All statewide communications regarding the survey◦ List of identified teachers required to take the survey based on

Certification and Staff File Data◦ Authorization codes associated with the individual teacher’s Educator

Identification Number (EIN) and ◦ District results from the survey administration windows

CSDE to identify teachers required to complete the survey based on Certification and Staff File Data

District liaisons may download from the dedicated CSDE website the list of identified teachers required to take the survey District must review and ensure the accuracy of the teacher list including

◦ Assignment Data (Elementary, Kindergarten, Bilingual Elementary)◦ Grade Level Data (K, 1, 2 or 3)◦ Check current year staff file data to ensure accuracy of the teachers’

assignment or grade level, and make change in the ED163 Staff File on or before June 30. The Staff File is scheduled to close at the end of the day on June 30 and will not reopen until end of August with the new Educator Data System (EDS). Changes to next year’s assignment and grade level data, addition of new hires or returning teachers, will need to be made in August/September in the new EDS.

CSDE recommends using the circular letter or other district letter to notify teachers now (and not wait until the fall)

Teachers who took and passed the actual Foundations of Reading TEST will be exempt from the 2014-15 survey

Teachers who participated on the CSDE item review panel will be exempt

Computer-delivered format via a Pearson website link

Two windows for survey completion during 2014-15 – (1) September 15 to November 14 and

(2) January 5 to February 6 (window may be expanded and/or another added)

To be completed on school/district site ◦ Recommend centralized school or district locations such as the

media centers or computer labs where the surveys may be taken

85 multiple choice items/180 minutes allotted including a 15 minute tutorial (one sitting only)

If a teacher cannot complete the survey, the survey will be “timed out” and may not be completed at a different time; district liaison will need to report problem to CSDE Slide 11

Results provided to the teacher immediately upon completion of the survey to the teacher’s survey account (created at the log-in stage) and may be printed directly from the account

Following survey administration window, the results will be posted to the dedicated CSDE website and district liaisons may download results data

District liaison and leadership will need to plan for analysis of survey results data in order to determine professional development needs

Reported for each teacher based on the number and % of items correct in the following 3 Survey Subareas and 9 Objectives

District responsibility to analyze survey results to identify areas of reading instruction that data may indicate for professional development support

Consider including teachers not required to take the survey

◦ TESOL, Special Education, etc., Teachers who were exempt, Reading and Instructional Specialists
