4. Memorandum of Agreement (MOA)

4. Biodata of Officers and Management Staff ( With latest 2 x 2 colored Pictures)

5. Notarized GA/BOD/Officers Resolution authorizing the President/Chairperson of the organization to make representations for the availment of financial assistance

from DOLE

10. Brief Descriptions of implemented socio-economic projects

11. Certification of Good Moral Standing of the Org. & credibility and capability of its Officers & staff from LGU Official.

DOLE REGISTRATION REQUIREMENTS ( 3 Sets )

1. Application for Registration ( Form No. 3-WA )

2. Name of the Assn. and of the Officers with their respective addresses;

3. Minutes of the Organizational Meeting;

4. List of members who participated the organizational meeting

5. Financial Report ( for Org. existing for 1 year or more only ) or Certification for less than 1 year of existence;

6. CBL with the attached names & signatures of ratifying members ;

7. Minutes of the ratification/adoption of the CBL ( Not required if done simultaneously with the Orgl. Meeting )

8. Registration Fee of Php 70.00 .

9. All documents must be duly certified true and correct by the assn. secretary/treasurer and attested by its President/Chairperson
