Please print this form, complete it as you evaluate the files. When the evaluation is completed to your satisfaction and the tier has met the requirements for the award please forward this Evaluation Form, the flies, and the completed Bronze Award Fly Transmittal Form to a member of the Evaluation Review Panel for verification and award.
