Finger reattachment

DefinitionFinger reattachment (or replacement) is defined as reattachment of the part that has beencompletely amputated

Purpose

Replantation refers to reattachment of a completely severed part meaning there is no physicalconnection between the part that has been cut off and the person Reattachment can besurgically performed for the finger and such other detached body parts as the hand or arm

DemographicsGood candidates for this procedure include persons with thumb or multiple digit amputation(A-CeAmputationhtml) Injury to multiple digits is an important patient selection criterionsince in some cases the least damaged digits may be shifted to the least injured or most usefulstump Patient exclusion is neither clear-cut nor absolute Generally severe crushing oravulsing (tearing away) injuries to the fingers may make replantation difficult but venous graftsmay help replace injured blood vessels Additionally older persons may have arteriosclerosisthat frequently impairs function in blood vessels especially in small vessels Special effortsmay be made to replant fingers if the persons livelihood (such as professional musicalperformance) depends on absolute finger control

DescriptionTo increase efficiency the replantation team splits into two sub-teams One sub-team in theoperating room (La-PaOperating-Roomhtml) cleans the amputated finger with sterilesolutions places it on ice and identifies and tags (with special surgical clips) nerves and bloodvessels Dead or damaged tissue is surgically removed with a procedure called debridement(Ce-FiDebridementhtml) The emergency room (ER) sub-team will assess the patient duringa physical exam with x rays of the injured area blood analysis and cardiac (heart) monitoringThe patient is given fluids intravenously (IV) a tetanus (knowledgeTetanushtml) injectionand antibiotics (A-CeAntibioticshtml) Usually most finger reattachments are performedwith a local anesthetic such as bupivacaine and a nerve block to numb the affected armMaintaining a warm body temperature can enhance blood flow to the affected limb

The surgical procedure consists of several stages The bone in the amputated finger must beshortened and fixed which means that the bone end is trimmed After this process the bone isstabilized with special sutures called K-wires and fixed pins are placed in the bone after drillinga space to insert them This process connects the two amputated bone fragments After bonestabilization and fixation the extensor and flexor tendons are repaired This step is vital sincearteries veins and nerves should never be surgically connected under tension Next thesurgeon must repair (suture) cut-off tendons arteries veins and nerves Healthy arteries andveins are sutured together without tension A vein graft is used for blood vessels that cannot bereattached

Nerve repair for finger reattachment is not difficult Since the reattached bone parts are shorterthan the original length nerves can be reattached without tension A microscope(knowledgeMicroscopehtml) is used for magnified visualization of finger nerves duringreattachment When the severed ends of the nerve cannot be reattached a primary nerve graftis performed Finally it is vital superficial veins on the affected finger (dorsal veins) to coverwith a skin flap to prevent death of the venous vessels The skin over the surgical field is looselysutured with a few sutures Any damaged tissue that may die (necrotic tissue) is removed No

tension should be placed on the skin fields during closure of the wound Wounds are coveredwith small strips of gauze (knowledgeGauzehtml) impregnated with petrolatum The upperextremity (knowledgeUpper_limbhtml) is immobilized and compression hand dressing andplaster splints are arranged to prevent slipping and movement of the affected arm

DiagnosisPreparationThe diagnosis is easily made by visual inspection since the finger(s) must be completelydetached from the hand The reattachment procedure is complex and involves the expertise andskill of a highly trained surgeon There are several important factors necessary to successfulreplantation including special instrumentation and transportation of the amputated fingerSurgical loupes (binocular-type eyepieces used by surgeons to magnify small structures duringsurgery) are necessary for this procedure Instruments should be at least 39 in (10 cm) long toallow for proper positioning in the surgeons hands Special clips are used to help suture bloodvessels together The best method of saving and transporting the amputated finger is to wrap itwith moistened cloth (Ringers lactate solution or saline solution) and place it on ice Generallythe tissues will survive for about six hours without cooling If the part is cooled tissue survivaltime is approximately 12 hours Fingers have the best outcome for transportation survival sincedigits (fingers) do not have a large percentage of muscle tissue

AftercarePostoperative care is vital for successful finger reattachment The hand is wrapped in a bulkycompression dressing and usually elevated If arterial flow (knowledgeBlood_flowhtml) isimpaired then the hand should be lowered since this maneuver will promote blood flow fromthe heart to the reattached finger If venous outflow is slow the hand must be elevatedMedications to increase blood flow (peripheral vasodilators) and an anticoagulant(knowledgeAnticoagulanthtml) (heparin) are used A tranquilizer may be given to reduceunnecessary blood vessel movement (vasospasm) that can occur due to anxiety Carefulexamination of the reattached digit(s) is necessary The surgeon frequently monitors color thecapacity of blood vessels capillary refill and warmth to monitor replant progress The YSItelethermometer monitors the digital (finger) temperature with small surface probes Skintemperature falling below 86degF (30degC) indicates poor blood perfusion

To save a detached finger for reattachment surgery it should be wrapped ina moist paper towel and put on ice (A) First the surgeon will reattach theblood vessels and nerves of the finger (B) The bone may be repaired withwires (C) and tendons are repaired (D) Skin and muscle wounds are also

(poor blood and oxygen delivery to the affected area) of the replant The cause of poor bloodcirculation must be investigated and corrected if possible The patients room should be warmand bed rest for two to three days is recommended Patients must refrain from smoking andtake antibiotics for one week after surgery Follow-up consultations are necessary for continuedwound care and rehabilitation

RisksThe experienced surgeon can estimate the likelihood complications based on the nature of theinjury Replantations that are risky such as those with circulatory perfusion problems havelower success rates Generally the most difficult replantations are those that involve childrenunder 10 injuries caused by a ring catching in machinery and crush-and-tear injuriesManagement of the difficult replant typically includes intravenous heparin to prevent clotting ofthe blood and providing a continuous nerve block in either the median or ulnar nerve

(depending on which fingers are reattached) A nerve block will cause vasodilatation orexpansion of the blood vessel Vasodilatation will increase blood flow bringing with it freshoxygenated blood Further evaluation should include checking the patients dressing forconstriction (ie if the dressing was placed too snugly and is constricting local blood vessels)

Normal resultsNormal results may not seem encouraging It must be considered that this is a major traumaand a highly complicated and intricate surgical repair Generally a normal result usuallyincludes good nerve recovery approximately 50 of normal for active range of joint motioncold intolerance (usually reversed in about two years) and acceptable cosmetic results

Morbidity and mortality ratesThere are about 10000 cases of job-related amputations in the United States each year 94 ofthese involve fingers Few statistics are available for the out-come of replantations

AlternativesThe only alternative to this procedure is to lose the finger(s) entirely and manage the remaininghand injury

Please help me with advice on post operative care and what to expect Four days ago thethumb of my dominant hand was severed between the nail and the top joint It wasreattached at St Marys Hospital San Francisco by Dr Charles Lee in a two-houroperation Several nerves and blood vessels were reattached but the tendons are left to berepaired later The reattached tip is stabilized with two pins that stick out from the top witha 08cm length of crushed bone left in place for the body to heal The blood supply andsensation of the fingertip when scratched appear to be normal I did not have much painand am not on pain medication What should I watch out for in the next weeks and whatcan I expect going forward

I work at a bank and about 3 weeks ago the vault door shut on my fingers Its not just anydoor either its 5 tons of steel and concrete so obviously the damage was pretty extensiveIt did not completely amputate my fingers but partially amputated 3 of them The bones inmy index and middle finger were completely crushed and the sin was badly torn so both ofthe were dangling off The middle finger was handing on by a tendon(knowledgeTendonhtml) and some nerves and the index finger was hanging on by someskin and the main artery The skin was torn on my ring finger but the bones werent brokenand my pinky nail was ripped off The doctors werent sure if the could save my middlefinger because my main artery was severed and it was very pale Anyways after8 hours ofsurgery they managed to reattach everything and I should have nearly full function inabout a year Its truly amazing what they can do

If all of the skin has been torn off and the nerves are also gone and the only thing left is thebones of the finger does the finger have to be amputated

I had my right index finger was crushed and basically amputated The finger was stillattached The blood vessels nerves and tendons were severed I had surgery 2 years agoand my finger was successfully repaired I have about 70 regained motion I do have theintolerance of cold and one of the blood vessels was not abled to be saved My question ismy finger is still numb in places and I still have daily pain in the finger Does the pain evergo away I am very luckey and thankful that I still have my finger I would still like to getmore range of motion less pain A medical professional said I should seek a hand surgeonand get another opinion and surgeryI worry that this might do irrepairable damage
