Subcontractor Oneida Total Integrated Enterprises (OTIE) is a teaming subcontractor with CH2M HILL on U.S. Environmental Protection Agency (EPA) Region 6 Projects. OTIE personnel, along with CH2M HILL personnel, will be supporting the soil sampling task for this mobilization. Mobilization Prior to mobilization the following supplies and equipment should be procured.
Two or three stainless steel hand augers capable of advancing to depths of 5 feet bgs
Two or three stainless steel trowels
A handheld Global Positioning System (GPS) enabled device
Photoionization Detector (PID) and calibration gases
Sample containers
Labels/tags for sample containers
Custody seals
Packing/shipping tape
Latex gloves
Several 5-gallon buckets for decontamination of the hand auger
Alconox phosphate-free soap and/or Liquinox to decontaminate the hand auger
Scrub brushes for removing gross contamination

Prior to mobilization the Field Team Leader (FTL) will confirm that the following items have been ordered and shipped to the site or CH2M HILL/DFW office: sampling containers, trip blanks, chain of custody (CoC) documents, custody seals, sample tags, and sample labels. Further, submission of applicable health, safety, and quality paperwork for CH2M HILL and OTIE employees should also be completed.
Field Monitoring and Sample Collection The GPS equipment will be checked prior to sampling to confirm it turns on and is working accurately. Sample containers will be checked for broken or missing containers. Each sample location will be verified using the coordinates listed by each Sample ID listed on Table 1 and Figure 1, and the location will be verified by using the GPS unit.
The field team will collect the soil samples using a stainless steel hand auger or a stainless steel trowel. Prior to collection, the sample will be placed into a decontaminated stainless steel bowl and homogenized. Any visual staining or presence of chemical odors, will be recorded in the field log book. The surface soil sampling procedures using hand auger are provided in the FSP, Appendix A-Standard Operating Procedures (CH2M HILL, 2013). Similarly, after collecting the soil sample the hole will be backfilled following the procedures outlined in the FSP, Appendix A-Standard Operating Procedures (CH2M HILL, 2013). Sample container size, preservatives, and holding times are provided in Table 2.

Decontamination and Waste Management Procedures The decontamination procedures of hand augers/trowels and waste management procedures will be followed as outlined in the Field Sampling Plan (CH2M HILL, 2013).
Project Organization/Staff Contacts Coordination and communications for the field event will primarily be coordinated through the Project Manager (PM), John Knott/DFW. Technical questions regarding the scope of work or other concerns that could potentially affect quality on the project should be directed to the PM. The longitude and latitude of sample points is provided in Table 3.
Communications and Quality Assurance/Quality Control Communication among the project team is critical for the success of this project. Communication includes the following:
Daily communication with project team
Daily e-mails and/or phone calls that summarize pertinent sample information and progress to John Ynfante, the Project Chemist, and PM – as appropriate.
Field notes, forms, and logbooks will be used by the FTL to verify that the information uploaded to the project database is accurate and complete at the end of the sampling event.

Deviations from the scope of work will be communicated in e-mails to the PM and documented in the logbooks.
Community Relations During the course of field activities, the project team may encounter pedestrians, workers, or possibly media officials who inquire about the nature of the work being performed. The project team is responsible for directing any inquiries to the PM. Field team staff are not allowed to discuss the nature of the work with non-project team personnel.

Schedule Sampling activities will start on October 20, 2014, and are anticipated to be completed by October 24, 2014.
Sample Handling and Analysis Sample handling involves the field-related considerations concerning field sample documentation, nomenclature, packaging, shipping, and chain-of-custody (COC). Sample container, preservation, and holding times are provided in Table 2. The FTL will be the immediate contact for sample tracking with support from John Ynfante/HOU. The FTL will coordinate with the CLP/EPA laboratories and the field team. The FTL will notify Myra Perez/Christy Warren/Mike Fertitta (EPA Laboratory contacts), and Margaret Jarosz/ Myra Perez (CLP Laboratory contacts) of all shipments sent to their laboratory and provide them with the number of coolers shipped and the tracking numbers of the shipment. Each day that samples are shipped to a CLP laboratory, the Scribe Export file for CLP must be uploaded on the SMO Portal. If any samples are shipped to CLP laboratories for Saturday delivery, Margaret Jarosz must be notified by 3:00 p.m. on Friday afternoon. Also, on the FedEx form, “overnight priority” should be checked. If necessary, check “For Saturday Delivery,” and apply Saturday delivery stickers to coolers.
The laboratory will send the Project Chemist a copy of the laboratory log-in sheets and any corrective action forms. The Project Chemist will receive the laboratory data packages and the Electronic Data Deliverables.
Sample Shipping Each day soil samples will be delivered to the following FedEx location in Alexandria, Louisiana for overnight shipping:

Health and Safety Work will be performed using Level D personnel protection (steel toe boots, nitrile gloves, reflective vests, and hard hat where required). Hard hats are required when working near the roadway. Snake chaps will be worn, if needed, in the areas that are not mowed or are wooded.
A hospital route map, driving directions, and additional health and safety information are included in the health and safety plan (HASP). In the event of an emergency, injured personnel should be taken to the following medical center:
Winn Parish Medical Center 301 West Boundary Street Winnfield, LA 71483 (318) 648‐3000 The FTL, who also serves as the Site Safety Coordinator (SSC), will conduct a daily health and safety meeting before beginning work to discuss the work items, worker responsibilities, and familiarize workers with the HASP. Each team member is responsible for reading, understanding, and abiding by the health and safety requirements documented in the HASP. The HASP will be signed by each field team member prior to conducting work. The Field team will refer to the HASP for tasks to be completed as part of this effort.
The HASP, activity hazard analysis (AHA), safe behavior observations (SBO), and pre task safety plans (PSTP) should be approved by the Site Health and Safety Manager, Mark Orman, and then printed and transported to the site. The AHAs for the soil sampling task using a hand auger is provided in Table 4. The expectation is for each team member to complete and submit an SBO once every week.
Send SBO file(s) via e‐mail to the ESBG SBO e‐mail box ([email protected]) and to the PM. Copies should also be saved to the SharePoint Site (link provided below).
