1.緒 言
小児がんには成人にみられないユニークな生物学
的特性を示すものが多く,小児がんの領域から,が
ん研究の進歩に大きく貢献する発見が少なからず報
告されている。そのような意味で,本誌にも小児が
ん研究の最近の動向の一端を紹介することは意義が
あると思われる。小児がん患者の中で白血病・悪性
リンパ腫の症例は約半数を占め,血液腫瘍は小児が
んの代表といえる。本稿では小児期特有の白血病の
中でも,とくに最近新知見が数多く報告され,疾患
の本態と発症機序の解明が進みつつある,ダウン症
における骨髄性白血病について概説する。
2.ダウン症児の白血病
ダウン症児では白血病発症のリスクが高いことが
よく知られており,非ダウン症児に比較して約10~
20倍の発症率とされている1)~3)。白血病のタイプは
発症年齢によって異なり,年長児では非ダウン症児
と同様に急性リンパ性白血病 acute lymphoblastic
leukemia(ALL)が多くを占めるが,2歳以下のダ
ウン症児では急性骨髄性白血病 acute myelogenous
leukemia(AML)が多くを占める。この中でもとくに
急性巨核芽球性白血病 acute megakaryoblastic leu-
kemia(AMKL)[FAB(French-American-British)分
類の M7]が多くを占め,1歳児に高いピークを示
す(図1)。AMKL は非ダウン症児には稀な型の白
血病であり,ダウン症児における AMKL の発生率
は非ダウン症児の約500倍とされている1),2)。また以
下に述べるように,ダウン症児の AMKL は非ダウ
ン症児における AMKL とは異なる種々の生物学的
特徴がある。1)cytosine arabinoside などの化学
療法剤に対する感受性が高いため,治療が著効しや
すく治癒率が高い。2)白血病発症に先行する長い
骨髄異形成の時期が多くの症例でみられる4)。ダウ
ン症児における骨髄異形成は末梢血の汎血球減少症
(とくに血小板減少)と形態異常を示す巨核球の骨髄
における増殖によって特徴づけられる。3)新生児
期に AMKL と区別し難い血液異常を呈するも,無
治療で自然に治癒する病態がある。自然治癒という
特有の経過から,この病態は類白血病反応をはじめ
とする様々な名称で呼ばれてきた。現在では欧米に
て一過性骨髄増殖症 transient myeloproliferative
disease(TMD),我 が 国 で は transient abnormal
myelopoiesis(TAM)という名称が広く用いられて
いるが,クローン性解析の研究から,この疾患は単
クローン性の疾患,すなわち腫瘍性疾患であること
が示されており5),一過性白血病なる名称も提唱さ
れている。本稿ではこの用語を用いることとする。
