to Fight AIDS, Tuberculosis and Malaria)などをはじめとする各種国際機関の努力により、マラリアの患者数と死亡者数は減少し(図 1)、2014 年以降の死亡者報告数は一桁になった 6)。ラオス政府と WHO は2030 年までに同国からマラリアを排除(elimination)することを目標に掲げ、長期残効型薬剤浸潤蚊帳の無料配布、並びに早期診断と早期治療に取り組んでいる。 ラオスのマラリア診断は、病院では主に末梢血薄層塗抹標本をギムザ染色したものを顕微鏡下で観察する方法(顕微鏡検査)が用いられ、ヘルスセンターと呼ばれる簡易の診療所では、イムノクロマト法を応用した迅速診断テスト(Rapid Diagnostic Test:
リア原虫(122 検体)の K13 遺伝子変異の有無を調べた結果、20%(24/122)に変異が観察された。これはパスツール研究所国際ネットワークにより実施された世界 59 カ国のアルテミシニン耐性マラリア分布状況を明らかにするための研究成果として New
England Journal of Medicine 誌に報告された 10)。 その後、われわれはラオス国内のアルテミシニン耐性マラリアの分布状況をより広範囲かつ継時的に把握するためのモニタリング方法を構築した。はじめに 2013 年から 2014 年にマラリア患者数が多かった南部 5 県の中から、特に患者数が多かった 16 郡
またマラリア感染に関するインタビュー調査結果とPCR 法による DNA 解析結果を用いて多変量解析を実施したところ、無症候性キャリアーとなるリスク因子は、成人男性であること、並びに過去に 3 回以上マラリア罹患経験があることであった 15)。またマラリア感染のリスク因子は、成人男性であること、特に農業従事者や兵士であることであった。一般的には森林での作業に従事するとマラリアに感染すると言われており、成人男性や兵士は、女性や子供より森林での作業に従事する機会が多いためと推察される。近年はこのようなハイリスク集団への防蚊対策として、一人用の薬剤浸潤蚊帳やハンモック、並びに虫除けスプレーやローションなどの配布も行われている。 近年 WHO では、無症候性キャリアーを含む低寄生率感染者への対策の一つとして、流行地域住民全員を対象とした抗マラリア薬の集団投与(Mass Drug
