Genomic biomarker: a measurable DNA or RNA characteristic that is an indicator of normal biologic processes, pathogenic processes and/or response to therapeutic or other intervention.
Pharmacogenomics (PGx): the investigation of variations of DNA and RNA characteristics as related to drug response.

Son alteraciones somáticas (aparecen en el tejido tumoral) caracterizadas como:
• mutaciones en el ADN, • cambios en el número de copias de un gen, • reordenamientos cromosómicos, • alteraciones epigenéticas.
Se identifican en el material genético obtenido de las células tumorales.

Erbitux 5 mg/ml solución para perfusión
En pacientes con cáncer colorrectal metastásico, cetuximab se utiliza en monoterapia o en combinación con quimioterapia (ver sección 5.1).
Debe verificarse que el gen KRAS sea de tipo nativo antes de la primera perfusión de cetuximab.
Es importante que un laboratorio con experiencia emplee un método validado (ver secciones 4.4 y 5.1).

Son alteraciones germinales (aparecen en todas las célulasdel organismo) y son del tipo:

Se identifican en el material genético obtenido de las célulasnucleadas de sangre periférica, saliva, etc.

CPT11 / Irinotecan
Es un compuesto semisintético derivado de la campotecina. Se obtiene de la corteza y las hojas del árbol de origen chino Camptotheca acuminatay su acción antineoplásica se debe a la inhibición de la enzima topoisomerasa I.
Indicaciones terapéuticas:1.- En cáncer colorectal avanzado:
- en combinación con 5-FU/LV como 1ª linea de tratamiento (FOLFIRI).
- en combinacion o en monoterapia en 2ª o subsiguientes líneas de tratamiento.
2.- En otros tipos de tumores sólidos
- Irinotecan es un profármaco que se convierte en una molécula con actividad citotóxica: SN-38.
- SN-38 is se elimina gracias a la acción de un enzima hepático (UGT1A1) que lo inactiva (SN-38G)y facilita su excreción por via biliar.
- Toxicidad asociada: diarrea y neutropenia

Conclusions
• * 390 mg/m2 in patients *1/*1 * 340 mg/m2 in patients *1/*28 …are safe doses when patients undergoing FOLFIRI regimen
are stratified by UGT1A1 genotype.
• The standard dose of 180 mg/m2 for irinotecan is significantly lower than the dose that can be tolerated by patients with a UGT1A1 *1/*1 or *1/*28 genotype.
• Further phase II and phase III studies are needed
Pharmacogenetic trials with cheap drugs like irinotecan may cut spending in times of economic recession.
