My childcare position will be considered open and available until the following criteria have been met: A signed
contract indicating a start date, and payment in full of the registration fee and first payment. Positions will not be held
for more than two weeks beyond the signed contract date. No money will be refunded in case of default by the
parent(s). Fees are per child and are non-negotiable. There is a one month mandatory notice which must be given
otherwise the full amount is due for child care period normally scheduled. This notice must be in writing.

Late Charges: ALL PAYMENS ARE DUE IN ADVANCE BY ON THE 1st & 15th OF EACH MONTH.
There is a $100 late fee if care is not paid for in advance. You will have a grace period of 5 days making it the 5th and 20th.
A fee of $10 per month credit card charge if paying by card. If card is declined there will be a $25 processing fee. If you are on “auto pay” You can expect your card to be charged up to 5 days before the 1st or 15th to allow
for processing.
Paid Holidays and absences: The following are paid holidays when they fall on a day regular work week M-F:
Veteran's Day, Memorial Day, Labor Day, Thanksgiving Day, the day after Thanksgiving, New Year’s Eve Day, New Year’s
Day, Christmas Eve, Christmas day and the day after. The Fourth of July or the day in observance and the day after. These
are the ONLY days the center is closed. Some of these holidays, depending on the year we are not closed. You will be
notified beforehand.
Charges for a child's absence for an illness or vacation are part of your set fee, which does not change.
If you are a Best Beginnings (state pay) family, you are required to pay your copay in advance, and you MUST attend a
minimum of 4 days per week. Must attend more then 6 hours per day unless your child is a school age child.

CONTRACT
Parent/Guardian to pay one time Registration and supplies fee of $100. Tuition is due on the 1st and the 15th of every month in
advance. A $100 LATE FEE WILL APPLY AFTER THE 5TH AND 20TH OF EACH MONTH. It is your responsibility to pay
before a vacation or if the 5th or 20th fall on a weekend. Please see rate sheet for rate chosen. The fee will be added on your next bill.
There will be a credit card service fee added to your bill of $10 per transaction if you choose to pay with a card. If your card is
declined for any reason you will be charged an additional processing fee of $25. If you are on “auto pay” You can expect your
card to be charged up to 5 days before the 1st or 15th to allow for processing. Credit card # are destroyed after the 1st use then
the # is saved internally on the Quick Books system.

Parent/Guardian will fill out the immunization record and have signed by the child’s Physician. Parent will keep the record updated as
needed.
Parent will fill out all necessary forms and keep them updated yearly. The Special Needs Heath Care plan (must be signed by a doctor
if this form is required), OTC form, food program forms and emergency contact form must be renewed yearly. It is the Parents
responsibility to inform KK of any special needs a child might have.
Parent will be responsible for all medical expenses for any injury /accident occurring while child has attended Kid Kountry.
Parent/Guardian will inform provider at least one week in advance if child will be absent due to holidays or vacation.
Parent/Guardian will let provider know by 9 am if a child will not be arriving for the day. If there is not any communication on this
you will run the risk of no care provided that day as we count our ratios. You will also be charged a $10 additional no call no show for
a day scheduled if we transport your child to/from school. No child can attend after 12 pm with out consent from management.
Parent will pay the full amount for the month including holidays. We charge a monthly rate and will charge the full month rate.
There is a $1.00 per minute late charge if your child is not picked up by 6pm. This is payable upon arrival. Your late fee must be paid
before your child can return. If this is a repeat offence the charge will be doubled, and you will risk losing care.
Kid Kountry will charge a 10% yearly fee which comes to .83% monthly on all accounts 30 days past due. Kid Kountry also uses CPI
collection agency that will collect accounts past 30 days. Kid Kountry is not obligated to tell you when this happens. Your account
will be automatically turned over after 30 days past due.
The Parent/Guardian will inform Kid Kountry of any change in address, phone number, employment status of either parent, or changes
in emergency numbers.
The Parent/Guardian will inform Kid Kountry if anyone other than themselves is picking up their child for that day. We will ask for
identification.
The Parent/Guardian will provide any necessities (diapers, medicine, etc.) and provide a clean set of clothes to be kept at Kid Kountry.
We usually put noticed when your child has 10 diapers left.
The Parent/Guardian will sign child in and out every day using the Procare system with their own ID not someone else’s. Each
person/adult needs to have their own # on the system. Additionally, parent or guardian will need to sign the State pay sheets when
requested.
Parent will keep child at home or find someone else to care for child if child has fever of 101 degrees or higher, diarrhea or any other
signs of communicable illnesses. Parent will pay for any medical treatment for illness or injuries occurring in the childcare. No
medication can be administered without written consent from parents or guardians. If child has lice or bed bugs a parent is required to
let the center know. Kid Kountry CDC will not be held liable for cases of bed bugs or lice or any other forms of communicable
illnesses or infectious disease.
We believe that parents are the most significant adult in a child’s life. We encourage parent involvement and open communication.

I give permission for Kid Kountry to pick my child up from school. I understand that Kid Kountry has many schools to transport to and may occasionally be late to pick up my child.
The following policies are in place for the school age program:
Morning transportation is optional, and you do NOT need to call if your child will not be coming in the morning. Your child MUST be here by 7:15 am for breakfast and the vans leave the parking lot at 7:30am. You MUST call before 2pm if your child will not be attending the afterschool program. We have a no call no show fee of $10 per day that will be added to your bill. The purpose for this policy is to eliminate unnecessary driving and waiting at schools where children don’t show. It also makes us late to other schools as our teachers cannot leave a school where a child is unaccounted for. Please inform your child’s school that they will be attending Kid country CDC.
1. For medical exemption purposes, a physician is a person licensed to practice medicine in any jurisdiction of the U.S. or Canada. This does not include chiropractic or naturopathic doctors, nurse practitioners or physician assistants.
2. In Section II, please include vaccine doses with month, day and year for each administered dose. Immunization dates, as specified in the administrative rules, are necessary. Please sign and date the form.
3. If the child is completing a vaccine series, a Conditional Attendance form can be used. The physician or health department will determine the date of each dose to
be administered and put the schedule on the Conditional Attendance form. Please sign the Conditional Attendance form, and return to the school or child care
facility.
4. Immunization forms can be obtained directly from the local health department or the Montana Immunization Program at www.immmunization.mt.gov.
School and Child Care Official
1. Prior to attending, all students and child care facility attendees must have either a) the required immunizations and documentation or b) have completed the appropriate exemption or conditional attendance documentation. This includes transfer students.
2. Documentation must meet the criteria of the Administrative Rules of Montana. This is limited to other school health records and certain documents from health departments and physicians.
3. Transferring information from supporting documentation to this form must be done by a school or child care official. The school or child care official must then
sign and date the form (Section II) and attach the supporting documentation.
4. Conditional Attendance form, once completed and attached to this document, allows attendance so long as immunization continues as scheduled.
5. School Transfer Students.
There is no transfer period allowed. Transfer students must provide adequate documentation of immunization PRIOR to attending school.
a) Transferring In: Students who transfer into Montana from out of state must have their immunization information recorded on this form (See number 2 above regarding acceptable documentation.) Students must meet Montana immunization requirements.
b) Transferring Out: If students transfer out of your school, a copy of this record should be maintained for one year following the transfer. The Montana law
requires schools to forward the original Certificate of Immunization to the school to which students transfer.
c) Homeless Students: All homeless students must be immediately enrolled in a Montana school to ensure compliance with the McKinney-Vento Act. Students should be assigned a liaison who can assist them in obtaining either appropriate documentation of immunization or in obtaining the required immunizations.
Parent 1. Montana law requires immunization information be recorded on this document for persons to attend Montana schools, preschools and child care facilities. 2. ONLY school, child care and health officials can complete this form. School and child care officials need documentation from physicians or health departments as
described by the Administrative Rules of Montana (examples: A completed Montana Certificate of Immunization; A signed Immunization record card). It is the
parent’s responsibility to provide these documents to the school or child care facility.
3. Religious exemption and conditional attendance may be used in accordance with the Immunization Law and Administrative rules. The Religious Exemption may be used in school settings and must be renewed annually. Religious exemption for child care only applies to Haemophilus influenzae type b (Hib), and must be renewed
annually.
4. Montana law prohibits children from attending any Montana school or child care facility prior to meeting immunization requirements.
5. If your child transfers to another Montana school, a copy of this completed form will allow your child to enter that school. However, the original Certificate of Immunization must be provided to the new school within 30 days of transfer in order for the child to attend.
