• Thirty-one Improvement Teams from Long Term Care facilities across the country enrolled in the Collaborative, and made a one year commitment to implementing best practices and change concepts reduce falls and injury from falls by 40% by May 2009. This Collaborative consisted of four face to face in person learning sessions.

by March 2011. Team used virtual technology to meet for four learning sessions.
