Facilities Incident Notification Reporting Requirements All employees are required to report immediately any incidents, accidents, or major equipment failures that have no redundancy and that have a direct or indirect effect on science or the site as soon as they become known. This includes injuries. This process is designed to: Promote employee safety throughout LBNL Minimize disruptions to the laboratory community Maximize response time for failing equipment Aid Facilities Management to meet reporting requirements We need to take care of each other 2.
