Interpretation of changes in the pulmonary vasculature has been largely dependent on observations of chest roentgenograms in daily practlce. Simon proposes an objective quantitative approach for the interpretation of pulmonary vascular markings on chest X-rays , but this method fails to re f1ect real vascular distribution of the lower lung, since coun- tlng was done on the lateral field rather than of the entire area due to incomplete visualization of the vascular mark- ings behind the heart on an orthovoltage PA chest. We propose a modified method of counting the pulmonary vessels that re f1ects more accurately the real condition by using high-penetratlon X-rays. The material in our study con- sisted of 200 normal adult chest films ofmen and 100 ofwomen , aged 20- 60 years). We counted the number of vessels in the lung fields using Simon 's and our modlfied method. There were 6 to 9 vessels counted in the upper zone by both methods . However , in the lower zone thére were 7 to 10 vessele according to Simon’ s method and 10 to 13 according to our method. 80th methods showed more vessels in the lower zone than in the upper zone. The number ofvessels was greater in the lower zone than in the upper in 98.5% by our method in contrast to 57% bySimon’ s method. The number ofvessels was greater in the left lung than in the right (p<0.05). Statistically there were no significant differences between the number of vessels according to sex , age. and weight. but a significant difference was noted according to height . Considering the erect position of the plain PA chest roentgenogram having normal physiologic caudalization of vascular distribution. we believe our modified method is more appropriate than Simon's.
