Community Hospital has treated more than 160 open-heart surgery patients since the program’s inceptionin February 2007. Each of these patients hasbenefited from the excellent care of our surgeons,nurses, and other professionals in our new state-of-the-art facilities.

As chair of the Board of Trustees for the past twoyears, I have been privileged to closely observe thisand other programs of Community Hospital and havecome to realize that our staff and volunteers expect ahigher level of performance from themselves than istypically found in similar organizations. In 2007, thisobservation was clearly evident in a hospital-wideinitiative known as Exceeding Expectations. Developedby front-line employees, Exceeding Expectationsencourages every employee at every level to committo a set of principles that result in superior treatmentof our patients within a safe and compassionateenvironment. A commitment to the program meansthat hospital employees will place patients’ needs andthose of their families above any other. It means thateveryone involved with the hospital has dedicatedthemselves to doing their jobs at the highest level ofperformance. In October, some 2,100 individualsparticipated in the organization-wide launch of theprogram and the excitement it created was palpable.

This annual report focuses on the ExceedingExpectations initiative. The board is very proud ofevery employee who has committed to the program,and I am confident that those who use hospitalservices in the months to come will notice thiswonderful spirit.

Community Hospital is all about relationships.With those words, the employees of Community Hospital of the Monterey Peninsula embarked on a

program in 2007 to raise the bar on relationships — our relationships with each other, with the

community, and, most important, with our patients.

Called Exceeding Expectations, the program sets 10 standards for all of our employees, at all levels,

to embrace. It is a pledge not only to continue our culture of excellence but, as the name says, to

exceed expectations.

The standards encompass areas that help shape every patient’s experiences, from customer service

to communication, teamwork to training, accountability to financial responsibility. They were developed

by a grassroots group of employees from a cross-section of our organization — people striving to

provide the kind of caring, safe, healing environment they would want their

own families to find.

“Working at a hospital is a unique job,” says one employee. “In an instant,

you or a loved one can be on the receiving end of care. The only way that

we can feel confident and comfortable about receiving care is to hold

ourselves and those around us accountable in providing care.”

Community Hospital has made a long-term commitment to the

ideals of Exceeding Expectations, integrating them into hiring

decisions, employee evaluations, training and education programs,

and more. Though the program is in its early stages, its effects

already are being felt. On the following pages, you’ll read a few stories

about ways that we have been working to meet the standards. They are snapshots of day-to-day

interactions, illustrating moments large and small in which employees exceeded expectations — and

continued to build the relationships that we so value.

“It was heartburn, I was sure of it.”

It was just heartburn, I was sure of it. But it had kept me up for two nights.

When I called in sick for the second day in a row, my co-worker said, “You

need to see a doctor; you should probably come in.”

She meant to Community Hospital, where I’ve worked for 23 years,

registering patients.

I had worked in the Emergency department for a long time, and I had seen

a lot of people come in with heart attacks. I just knew I wasn’t one of them.

I never thought that I was having a heart attack — I was 57 and healthy. I

was having heartburn.

Then they did an EKG. A nurse I didn’t know was very kind and comforting,

and Dr. (Neal) Harris came in and I felt safe. Then he said, “You’re having a

heart attack.” I ended up having triple bypass surgery and was hospitalized

for 17 days.

Family members have been patients, but I had never been a patient. I was

very scared, but staff members reassured me that I would be all right and

constantly told me what was happening and why. The Emergency

department, cath lab, Intensive Care Unit, cardiac team, the physicians,

and staff worked together like a finely oiled machine. They are a

phenomenal team!

My level of respect has increased by volumes for the nursing staff and all

the departments that cared for me. I have always known how hard the

Emergency nurses work, but I had no clue what intensive one-on-one care

the ICU nurses give their patients. I knew what the cath lab was, but I really

didn’t know how they work together with the cardiologists. Cardiopulmonary

Wellness at Hartnell Professional Center is a fabulous team; they’re happy,

they’re motivating, and they helped me regain confidence in myself

physically.

I believe that my being an employee at Community Hospital made no

difference in the level of care I received. I don’t think it would have mattered

if I had been Jane Doe off the street. They take great care of you.

I wrote a thank-you letter to try to express my appreciation and gratitude

for all that was done for me and for having a heart program right here on

the Peninsula.

Mr. Watson had been homeless for more than 30 years, finding a place

to sleep wherever he could. Most recently, that was his van, parked

somewhere in Salinas.

His life had taken a wrong turn after his mother died and he had trouble

coping with the grief. He had few friends, and was estranged from most

of his family. He got by picking up construction jobs now and then, until

pain in his back became more than he could bear and he came to

Community Hospital.

When X-rays taken in the Emergency department showed something

suspicious, he was admitted and taken to a room on Terrace West.

At first, it was a tough transition from the streets. Mr. Watson wasn’t

used to being confined; his world had been the whole world. He didn’t

trust us much. Too often, he had been let down or abandoned. He was in

pain and afraid. Then there was the shock of his diagnosis: He had lung

cancer and it had spread to multiple sites.

We faced big challenges caring for him. Nurses and Pharmacy staff tried

different approaches to control his pain. Sometimes Mr. Watson would

agree with the plan; sometimes he wouldn’t. Palliative care staff spent a

lot of time with him, listening while he talked about his regrets, mostly

guilty feelings that he hadn’t done much with his life.

We tended to all the day-to-day comforts that he had been missing for

years: good food, clean clothes, and lots of warm showers. And

everyone treated him with kindness and respect.

Decisions were difficult; Mr. Watson chose not to receive any “active”

treatment, but to focus instead on comfort. He had kept in touch with

one of his brothers, and the staff offered to help him to reconcile with all of

his siblings one last time. All five of his brothers came to see him; it was

the first time they had all been together in 45 years. With our support, they

made amends, expressed forgiveness, and shared their love.

Mr. Watson had one last request, to see a good friend from Dorothy’s

Kitchen, which serves food to the poor and homeless in Salinas. One of

our social workers found the friend, and they shared one last, good visit.

Mr. Watson was a patient on Terrace West for 26 days, until he died.

By then he was like family.

The woman on the other end of the line was panicked. Her husband, a

hospice patient, had been home from the hospital for only a couple of

days and she was struggling to make him comfortable, to ease his pain,

to simply take care of him. She had called Hospice of the Central Coast

for help.She said she didn’t know what to do anymore. Then she abruptly

hung up.The woman had given me her name, but I didn’t recognize it. I found

her telephone number in the files and called back. But over and over,

the line was busy.Instead of waiting for this distraught woman to realize that her phone

apparently was off the hook, I found the couple’s address and headed to

their home. I introduced myself, told them I was a nurse from hospice,

and said I was there to see how I could help.

Her husband was clearly in pain and short of breath. His medications

were mixed up on the bedside table. I sorted through them, gave him

the right one to help him relax, helped him find a more comfortable

position, and within a short time, his pain decreased and he was

breathing easier. I sat with him for a little while, holding his hand and

reassuring him that things were under control.

When he was calm and feeling better, I spent some time with his wife.

I told her about some steps she could take to keep her husband

comfortable, and reviewed his medications and how they should

be used.I recommended bringing in a hospital bed to make him more

comfortable. I explained that the adjustable bed would make it easier

for him to rest without getting short of breath. And if they put it in the

living room, he could enjoy his favorite view.

Before I left, I told the woman she had done the right thing to call for

help. Sometimes it’s not the patients who are having the hardest time,

PUBLIC SHARING OF DATA FOR COMPARISONAn increasing amount of data about clinical practices, outcomes,and patient experience is being collected, and much of it is beingmade public. At Community Hospital, we provide such informationnot only when it’s required, but also when we think it will improvecare and inform consumers.

“Sharing information related to patient care processes andoutcomes is vitally important in maintaining our community’s trust,”says Dr. Steven Packer, president and CEO.

The hospital takes part in many data-gathering efforts, including therecently launched Hospital Consumer Assessment of HealthcareProviders and Systems, also known as HCAHPS. It is the firstnationwide survey of patients’ perception of care.

“Understanding a patient’s experience of their hospital care is anecessary ingredient to improving our performance,” Packer says.“We are using the information from the survey to evaluate the carewe provide and find ways to ensure we are addressing all of ourpatients’ care needs.”

EVIDENCE-BASED DECISION MAKINGCommunity Hospital strives to provide healthcare with ahuman touch, but we also want to ensure that we are usingbest practices. When clinical decisions are guided by science,results improve.

“In the Intensive Care Unit, we use the evidence in theliterature to choose practices that prevent complications and advance treatment,” says Dr. Georgina Heal, ICU medical director.

For example, patients with breathing tubes for more than fivedays have an increased chance of developing pneumonia. Oneproven way to reduce the chance is by elevating the head ofthe bed to help prevent bacteria in the mouth from going tothe lungs, so that is standard practice.

“In terms of treatment, we also use evidence-basedprotocols,” Heal says. “Where evidence shows, for example,that one antibiotic is better than another, we administer that.Or for heart disease, certain drugs have been shown todecrease mortality after a heart attack.”

Recently, Community Hospital introduced the use of rapidresponse teams.

HOW DO PATIENTS RATE THE HOSPITAL OVERALL?Percentage of patients who gave their hospital arating of 9 or 10 on a scale of 0 to 10 , with 10being the highest.

“This evidence-based practice provides early and timely intervention whenever patients show early signs of clinicaldeterioration,” Packer says. “This activity, like so many we’ve been focusing on, is designed to provide optimaltreatment and improve outcomes.”

PATIENT SAFETY IS A SYSTEM PRIORITYA key way to prevent human errors in a hospital is to make sure systems are the best they can be. Among ourlatest initiatives are a computerized physician order entry system (CPOE) and smart-pump technology for IVmedication delivery.

Through CPOE, physicians enter their orders for medication, tests, and treatment into a computer system ratherthan hand-writing them in a paper chart. The orders instantly go into the patient record and to departments suchas Pharmacy, Laboratory Services, and Respiratory Care.

“The system cuts down on the number of steps where an error can occur between the physician writing an orderand the nurse or other clinical staff member carrying out that order,” says Dr. Michael Smith, who helped in thehospital’s adoption effort. “In the past, there was more interpretation involved. A clinical staff member would haveto read the doctor’s handwriting, and understand not just what was written but the implications. That’s gone away.We now have a very clear pathway between what the physician writes electronically and what the clinical staffmember is asked to do.”

Community Hospital is among the earliest users of CPOE. Only 5 percent of hospitals nationwide have fullyimplemented it. Most hospitals consider implementation a success when at least half of their orders arecomputerized within two years. At Community Hospital, more than 85 percent were computerized within 8 monthsof the system’s introduction.

Smart-pump technology is also aimed at reducing errors, using computers to ensure that patients get the rightmedication at the right dose.

“A smart infusion pump, programmed withpatient and drug parameters, is able torecognize programming errors,miscalculations of medication doses, andinaccurately prescribed infusion ratesbefore the medicine reaches the patient,”says Therese Beauclair, clinical pharmacist,assistant director of Pharmacy, and amember of the team that selected andimplemented the system. “It warnsclinicians of potential medication problems,ultimately preventing medication errors.With these smart pumps, CommunityHospital has achieved the next level ofsafety for our patients.”

FINANCIAL INFORMATIONIn 2007, healthcare spending in the United States reached $2.3 trillion, and it continues to increase at the fastestrate in history. We know these costs are a constant concern for all our patients and their families. We arecommitted to carefully managing the hospital’s resources and your costs, and have made that commitment afundamental part of our Exceeding Expectations program. Our efforts help us keep our average charge per stayamong the lowest of comparable hospitals in Northern California, as the chart on this page shows.

An ongoing challenge is government’s failure to pay its fair share. Did you know that through programs such asMedicare, Medi-Cal, CHAMPUS, and Workers’ Compensation the government pays, on average, only about half ofwhat it actually costs Community Hospital to provide healthcare? If government paid its full share of costs, wecould cut our charges to everyone by 36 percent.

In spite of that obstacle, we continue to provide what our patients and community expect: the best possible care,using the latest technology, at the lowest cost possible. On the following pages, you will find a financial snapshotof how we do it.

Community Hospital was built on philanthropy — the land on which it sits today, for example, was donated byS.F.B. Morse and his Del Monte Properties Company. That spirit of giving continues to nourish the hospital, asgenerous donors make contributions for a particular program, a specific building project, or, simply, the hospital’soverall mission.

BETTY PASSMORE BASS

When Betty Passmore Bass moved to Carmel fromLa Jolla a decade ago, she brought with her anappreciation for art and a dedication to charitablegiving. She found an opportunity to exerciseboth after a hip replacement at CommunityHospital of the Monterey Peninsula.

Bass, who came to Community Hospital forsurgery in 2004, was so impressed with thecare she received at the hospital and,

afterward, during recovery at Community Hospital’sWestland House, that she decided to make a special gift.

“Westland is a wonderful place,” Bass says. “The nurses there are so caring and thoughtful.”

Working with Debbie Howitt, senior development officer of Community Hospital Foundation,Bass settled on sponsoring a soothing work of art for a space set aside in the hospitalEmergency department waiting area.

The sculpture, set into a wall of the waiting area, features a trio of monoliths with watercascading down the faces into a pool at the base. The design, by artist Archie Held,appealed to Bass, who holds a master’s degree in art history and collects modern art.“Debbie recommended it,” Bass says. “She thought I would like it, and I did.”

Once the sculpture was in place, Bass stopped by and admired how it had been incorporatedinto the space. “It makes the room more restful,” she says.

A longtime patron of the arts, Bass was active in philanthropy in the San Diego area prior tomoving to Carmel and had given several gift annuities to museums. She continued her legacyof giving with a series of donations to Community Hospital through charitable gift annuities.

The water feature in the Emergency department has proved to be something more personal, adonation that spoke both to her appreciation for the work the hospital does and her love of art.

“It gives a special satisfaction,” Bass says.

Lola Meredith anticipated a more activevolunteer schedule once she retired, andshe knew exactly where she would spendmost of it: Community Hospital of theMonterey Peninsula.

Meredith, of Pacific Grove, once recoveredfrom a heart attack at Community Hospital. Hermother had gone there for cancer treatment,and she had visited friends who were patients atvarious times during her 31 years living on theMonterey Peninsula. The quality of care always impressed her.

“I have always had good feelings about Community Hospital,” Meredith says.

So after she retired from her job running the front desk of a Monterey hotel, she reported forduty with the Community Hospital Auxiliary in June 2007. Since then, she has donated bothmoney and time, logging 30 to 35 hours each month — with one exception. During theholiday season, she doubled her time to help decorate the holiday trees and displays set upthroughout the hospital.

Decorating duty comes on top of her volunteer work in the gift shop, delivering newspapersand magazines, and making presentations to kindergarten classes to allay any fears thechildren might have about hospitals. In January 2008, she was recognized as an AuxiliaryVolunteer of the Month.

“I like the satisfaction of knowing that I have made someone’s life a little more comfortable,”Meredith says. “I like people, and I like people to like me.”

Meredith, who is 70, says she hopes to maintain her busy schedule at Community Hospital foryears to come. She looks for inspiration to the dedicated volunteers who have been helpingout at Community Hospital for decades.

“I marvel at the people who have been doing it for 15 or 20 years and have put in thousandsof hours,” she says. “I expect to keep it up as long as I can.”

Twenty-six years ago, Ernesto Heres of Seasidelanded a job in dietary services at CommunityHospital of the Monterey Peninsula. Every twoweeks since then, Community Hospital hasgiven Heres a paycheck. And every twoweeks, he has given a little bit back.

Shortly after Heres reported for his new joba quarter-century ago, he heard about afundraising campaign for one ofCommunity Hospital’s building projectsand decided on the spot to have regularcontributions withheld from his biweeklypaycheck. He had never done anythinglike it, but he figured Community Hospital wasnot only his employer but also a worthy cause. Over the years,Heres moved through a series of jobs: from dietary services to housekeeping to themailroom to the business office, where he rose from file clerk to data entry clerk to a series of supervisory roles. Today, he oversees key patient billing functions as Patient BusinessServices coordinator.

To Heres, contributing to Community Hospital Foundation was the least he could do for aninstitution that not only provided him with a career but also was the place his children wereborn and his elderly parents passed away. He views his biweekly payroll deductions as hissmall way of saying thanks.

“Community Hospital really holds a special place in my heart,” Heres says.

Through it all, Heres has never missed a payroll deduction, upping the amount as his payincreased. He rarely discussed his dedicated giving, figuring his generosity was a personalmatter. But he watched with pride as hospital projects, upgrades, and remodels took shapeover the years, knowing that his quiet giving had helped make each one possible.

“I work here, so I know where the money goes,” Heres says. “I know it is money well spent. You can see how beautiful our campus is.”

TED AND SUE HOOKER

When Sue Hooker looks back on her life inMonterey, she’s reminded that it all could justas easily have taken place elsewhere. Andthat would have been a loss for CommunityHospital of the Monterey Peninsula.

Her husband, Dr. Theodore C. “Ted”Hooker, hoped to establish his practicein plastic and reconstructive surgery inSan Jose. There wasn’t an openingthere for his specialty at the time, butMonterey was very much in need ofhis skills. In 1957, the Hookersmoved to Monterey and never left,

becoming fixtures in local civic circles andunwavering backers of Community Hospital.

Their contributions helped support campaigns at the hospital for half a century. When Dr. Hooker died on Jan. 17, 2008, he left behind a respected legacy of service and giving that resulted in a wave of contributions. More than 40 donations were made to the hospital in his memory, a reflection of the care, compassion, and skill he demonstrated, and thepersonal relationships he developed.

Dr. Hooker’s service to the hospital and the community extended well beyond the operatingroom. At various times, he was president of the Community Hospital medical staff, chair of thehospital’s board, and chair of the Monterey County Medical Society. He also served on manyother community boards and commissions, including the Monterey City Council.

Through it all, the Hookers remained consistent donors to Community Hospital, keeping up thegiving after Dr. Hooker’s retirement in 1984.

The Hookers contributed to campaigns to create the Family Birth Center, the ComprehensiveCancer Center, and Rehabilitation Services. They have also been regular donors to thehospital’s general fund, so their money could go wherever it was most needed.

“He was proud of the hospital,” Mrs. Hooker says. “I think he felt that he owed themsomething because they had been good to him when he was practicing.”

HOSPICE FOUNDATION

When the Monterey-based Hospice Foundation conducts its annual evaluation of grant-worthyprograms that support those near the end of life and their families, Community Hospital of theMonterey Peninsula always earns high marks.

Over the past decade, Hospice Foundation has granted $14 million to 12 organizations servingthe region. Community Hospital’s share of that total: $8.5 million.

Part of the reason Community Hospital doesso well is that the foundation and the hospitalhave a special relationship that dates backto the foundation’s creation. Ten years ago,Community Hospital acquired the clinicaloperations and inpatient facility (nowcalled Westland House) of Hospice of theCentral Coast. As part of the deal,Hospice Foundation was created andendowed with $10 million fromCommunity Hospital. Income from thatendowment helps finance operationsof Hospice of the Central Coast.Investment revenue from otherHospice Foundation funds coversthe foundation’s operations, whichallows all of its donations to go directly to grants.

Alice Kinsler, president and CEO of Hospice Foundation, says one of the key reasonsCommunity Hospital consistently garners such a high level of foundation grant support isbecause it maintains the largest and most comprehensive hospice service in the region.

That resonates with donors, who give to the foundation to further the hospice mission.

“We want to transform the way dying is perceived and experienced,” says Larry Daniels, vicechair of Hospice Foundation and president of Daniels and House Construction Company ofMonterey. “I think we can change dying from a very dark and depressing experience intosomething with light around the grief.”

With the help of Hospice Foundation donations, Community Hospital is doing its part to makethat goal a reality.
