Welcome

Dear Delegates,

It is with great excitement that we, the organizers, look forward to the upcoming hack-a-thon!

In the thick layers of complexity the world order acts within it is crucial to explore potential solutions to global threats through different perspectives and expertise! A hack-a-thon sets out a dynamic environment for various fields to integrate, explore, create and build bridges. Epihack will gather a vivid set of people ranging from developers to global health experts to address the important, borderless issue of disease surveillance.

Technology is an enabling toolkit that utilizes the sources of ‘big data’ for large-scale disease surveillance in order to prevent, minimize impact or retreat the expansion of diseases. Epihack will take place in Cambodia, which illustrates the importance of surveillance in the region which houses several global disease outbreaks as well as a place of interesting ideas and projects aiming to improve the health security. Epihack marks a milestone of fusing experts from various fields in the quest to ensure a healthier and safer world through the hack-a-thon model!

During the course of three days Epihack will join the forces of the leading stakeholders in South East Asia in the field of technology, health care and development to collaborate, create, adapt or improve five prototype projects for participatory surveillance. The Epihack organizing team has carefully selected five existing projects that will in partnership with the developers and global health experts combine the potential of technology and beyond! At the end of the hack-a-thon the intense input of effort and creativity will be shared with the health community in Phnom Penh, including the private and public sectors, NGOs and other stakeholders.

You will find useful information prior to the event in this file and we are warmly welcoming you once again to Epihack!

Transportation and meals

Roundtrip airport transportation has been arranged. Estimated transit time is 45 minutes from the airport to the hotel. Please look for a van marked with “<epihack />”

All delegates staying at the Raffles Hotel Le Royal will be provided with transportation to the venue during the course of the event. Please meet in the hotel lobby 30 minutes before the start time each day for group transportation.

The Raffles hotel offers a very nice complimentary breakfast. Please be sure to eat breakfast prior to departing for EpiHack each morning. Refreshments, lunch and dinner will be provided for the duration of EpiHack. Note: dinner will not be provided on the evening of Friday, August 8.

Phnom Penh has a!tropical wet and dry climate. The climate is hot year-round with only minor variations. Temperatures typically range from 22° to 35 °C (71.6° to 95 °F) and weather is subject to the tropical monsoons. The southwest monsoon blows inland bringing moisture-laden winds from the!Gulf of Thailand!and! Indian Ocean! from May to October. The city experiences the heaviest precipitation from September to October.

Deciphering the epidemiological characteristics and impact of the 2009 influenza pandemic is an important public health objective—it will lead to a better understanding of the public health implications of influenza outbreaks, improvements in epidemiological modeling, and more efficient management of future influenza epidemics. The Cohort for Pandemic Influenza in Laos (CoPanFlu Laos) is a laboratory-based system first implemented in Vientiane, Laos in 2009 to investigate 2009 A(H1N1) influenza seroconversion rates and their determinants within an unvaccinated population. CoPanFlu Laos follows a general population cohort of 807 households and 4,072 participants that was established in March 2010. Sociodemographic data, epidemiological data, and capillary blood samples were collected from all the household members on two occasions in the past: during the inclusion visit (March – April 2010) and follow-up phase (October – November 2010), a team of interviewers visited the household at an appointed date in order to present the project, collect the written consent forms from each household member, identify a ‘‘referent’’ household member, collect inclusion information and draw the blood samples. If any household member refused to take part, the household was not included in the study. The questionnaires collected information on the participants’ demographics, work environment, general health status, history of influenza-like illnesses (ILIs) and other chronic and acute diseases, their history of vaccination, living conditions and household environment. A capillary blood sample was systematically collected from each member of the household with a single-use automatic lancing device, and taken to the Centre d’Infectiologie Christophe Me´rieux of Lao PDR (CICML) for centrifugation, aliquoting and storage at -80 degrees C.

We would like to explore adding participatory surveillance, or self-reporting to this effort moving forward with the ability to verify actual infection with influenza through laboratory analysis linked to the self-reported data.

The follow-up phase (October–November 2010) consisted of a systematic visit to each household in order to take a second capillary blood sample from each member, using the same methodology as at the inclusion visit, and to update the demographic, environmental and clinical data.

Results for households and individuals are shown in Tables 1 and 2, respectively, below.

In 2011, ChangeFusion, Opendream, Folk Doctor Foundation, and Thai Health Promotion Foundation collaborated to develop and launch “DoctorMe”, the first mobile application for health in Thailand on iOS and Android systems. DoctorMe, a free application, was created to promote good health care and first aid available to all in Thailand.

The application was first designed to be an electronic health manual with a database of more than 1,000 hospitals in Thailand for users to locate a nearby hospital when needed. The updated version of the DoctorMe connects the user with the National Institute for Emergency Medicine (NIEM), offering free services by sending an ambulance instantly when you call 1669. Additionally, push notification is also being used in this version to keep people alert about health care issues.

Since the launch in August 2011, DoctorMe has been consistently ranked among the most popular applications in the health and fitness section, both in the Apple App Store and Android Market.

There are currently over 250,000 installed users and over 30,000 active users monthly. Some areas for improvement in the future include: Registered Users’ Weekly survey, geolocated data collection, sophisticated data analysis, ONE HEALTH open data collection.

The Bureau of Epidemiology in Thailand set up a web-based reporting systes for Influenza like Illness (ILI) with 987 government hospitals around the country. The project was established in 2009 during A(H1N1) pandemic and, therefore, needed to have a timely reporting system. BOE assigned hospitals to submit the number of ILI cases weekly. The system then detects trends of flu outbreaks and notifies the BOE as soon as possible. After the hospitals submit reports, the system analyzes and sends back the results of the ILI situation to the hospitals and other responsible stakeholders in the area through the Internet, email, and SMS for up-to-date information on the flu situation weekly

Maetao Clinic (Myanmar/Thailand)maetaoclinic.org

The Maetao Clinic is a health provider and training center based in Mae Sot, near the Myanmar-Thailand border. Maetao was established to contribute and promote accessible quality health care among displaced Burmese and other ethnic peoples in this border location. The Maetao Clinic provides health care to 100,000 patients per year. The group has developed an electronic medical record that could be a good data source for disease surveillance.

Maetao Clinic has transitioned from paper-based information to using Microsoft Access in order to become a more digitized and advanced system, however, the past data is still filed in paper format. There are approximately 280,000 cases for past year records. Additionally, the current staff are being trained to use Microsoft Access by volunteers, yet this holds a short term commitment which is not sustainable in the long run. Also, Tak province health care officials visited the clinic to give further training on medical recording. Furthermore, the data of diagnosis reports are added to individual patient records but are not interlinked between departments.

Maetao also faces issues regarding Health Information System (HIS) since the Clinic sends the HIS surveillance records through MOPG through email, which requires time to train the staff on coordination between the systems. Thus, the IT problem from an organizational perspective involves; 1) staff turn-over; 2) long term maintenance strategy; 3) security and back up and 4) working servers.

Voice to voice interaction remains the most effective and primary form of human communication for both health care service providers and vulnerable groups who live in rural Cambodia. Due to poor network connectivity and literacy barriers, rural Cambodians are inhibited from using many technologies. While many people in rural Cambodia have access to mobile phones, literacy and Khmer language support on mobile devices is still limited. This poses a significant barrier to using SMS based mobile applications and makes voice based systems a more desirable alternative.In September 2012, with funding from!Spider (The Swedish Program for ICT in Developing Regions), the! InSTEDD iLab Southeast Asia! introduced and extended! Verboice, an open-source tool developed by InSTEDD to support health, safety and sustainable development programs in Cambodia. Verboice makes it easy for anyone to create and run applications that interact via voice, allowing people to listen and record messages in their own language and dialect, or answer questions with a phone keypad. Verboice makes it possible to improve lives even in communities previously closed off by literacy and technological barriers.The Verboice system for participatory health surveillance in Cambodia is built on the ideas that 1) participation from ordinary people in reporting about abnormal or suspected events can be a great source of data for any health surveillance system and 2) automatic voice based systems designed to support health expertise can easily capture reports from ordinary people in the local language via phone call. Data captured can be analyzed algorithmically or via crowdsourcing and monitored or shared across health domains (e.g. Animal and human health). It leverages pre-existing relationships with operators; and local staff can help the design, support and ongoing sustainability of the project. In addition to the Verboice participatory surveillance system in Cambodia, the technology is also currently deployed in Cambodia for National Election Hotline, post abortion follow up, and youth education programs, in Kenya for a Baby Monitor Program, and in Ghana, Ethiopia, India, Chile and the U.S. to support health and social projects.

The Royal University of Phnom Penh (RUPP) is Cambodia’s oldest and one of the largest public universities. It hosts more than 12,000 scholarship and full-fee paying students, across a diverse range of undergraduate and postgraduate programs. It is unique in Cambodia for offering specialist degrees in fields including the sciences, humanities and social sciences, as well as professional degrees in fields such as information technology, electronics, psychology, social work, and tourism. RUPP also provides Cambodia’s foremost degree-level language programs through the Institute of Foreign Languages. Due to its many achievements, RUPP has full membership of the ASEAN University Network (AUN).

RUPP has over 450 full-time staff. All of its 335 academic staff members hold tertiary qualifications, including 15 PhDs and 280 Mastersdegrees. They are supported by over 120 administrative and maintenance staff. In addition, the university maintains links with Cambodian and international NGOs, local and international universities and government ministries. As a result, various international and non-government organizations and government offices regularly contribute adjunct faculty members to help expand RUPP’s capacity.

RUPP is located on three spacious campuses. The main campus lies roughly five kilometers from the center of Phnom Penh. Adjoining the main campus is the Institute of Foreign Languages, which boasts peaceful landscaping features alongside its modern architecture. Roughly one kilometer further from town lies RUPP Campus II, in which the departments of History, Sociology and Philosophy reside.
