Management of Chronic Hepatitis B Beyond Antiviral Agents

The goals of chronic hepatitis B treatment are to decrease the
mortality rate and increase the survival rate by alleviating
hepatic inflammation and preventing the development of fibrosis,
which ultimately decrease the progression of hepatitis
to liver cirrhosis or HCC. Host, viral and environmental factors
contribute to an HBsAg-positive patients' risk of
cirrhosis and HCC. The main risk factors for HCC are not only
chronic hepatitis viruses (hepatitis B and C), but also
alcohol drinking, tobacco smoking, and aflatoxin exposure. Oral
contraceptive usage, iron overload, overweight, and
diabetes are also known or suspected as risk factors of HCC.
Currently, treatment of hepatitis B concentrates on antiviral
agents mainly. Although Peginterferon and nucleos(t)ide
analogues are effective, both therapies have limitations such
as
low response rate, side effects, long term use and drug
resistance. In contrast with viral factors, there are a few effort
and
study to improve the host and environmental factors. Considering
the synergistic effects of host and environmental
factors and HBV infection on the risk of HCC, efforts should be
focused not only on abstinence but also on promoting
and modifying generally healthy lifestyles. To maximize the
effects of treatment for chronic hepatitis B, personalized
approach should be conducted according to disease
characteristics, individual preferences, medical comorbidities
and
social circumstances. And proper education should be
considered.

서 론
만성 B형 간염의 치료 목표는 바이러스 증식을 억제하여, 염증 반응을 완화시키고 섬유화를 방지하여, 간기능의 상실
혹은 간경변 및 간세포암으로 진행을 예방함으로써, 간질환에 의한 사망률을 낮추고, 생존율을 향상시키는
것이다.1,2
B형 간염에서 질환의 진행에 관여하는 인자로는, 바이러스 관련 인자(viral factors), 인적 인자(Host
factors), 환경적 인자(Environmental factors)가 있다.(Table 1)3
간암 발생의 주 위험 요소는 B형 및 C형 간염 바이러스에 의한 만성 감염뿐만 아니라, 음주, 흡연 및 아플라톡신
노출 등이 있으며, 경구 피임약의 사용, 철 과잉, 과체중 그리고 당뇨 등도 간암의 추정 위험 인자로 알려져
있다.4

현재 만성 B형 간염에 대한 치료는 바이러스 관련 인자의 조절에 집중되고 있으며, 치료제로 페그인터페론을 포함한
주사제인 인터페론 알파와 경구용 뉴클레오시(티)드가 있다. 인터페론 제제는 바이러스 억제 효과 및 면역조절 작용이 있어
생화학 및 조직 소견의 호전을 기대할 수 있고, 엔테카비어와 테노포비어는 기존 약제에 비해 내성 변이를 현저히
감소시켰다. 그러나, 장기 투여에 따른 내성 발생 위험이 여전히 남아 있으며, 약제의 안전성도 완전히 해결된 것은
아니다.5,6 따라서 바이러스 침입 억제제, cccDNA 억제제, nucleocapsid 조립 억제제, HBsAg 분비
억제제 및 면역 조절제 등이 개발되어 항바이러스 효과에 관한 연구가 활발히 진행 중이다.
이에 비하여, 인적 인자(Host factors)나 환경적 인자들(Environmental factors)과 관련된
치료는 그 동안 상대적으로 많지 않았는데, 간암 발생에서 B형 간염과 이러한 인자들과의 상호 작용을 고려할 때, 치료
효과를 극대화하기 위해서는, 바이러스 치료제와 더불어, 질환의 악화나 호전에 영향을 미칠 수 있는 행동 양식이나 식이
습관 등 건강한 생활 습관의 증진 및 개인에 적합한 맞춤형 교육 및 진료가 이루어져야 한다.
본론
1. 환경적 인자(Environmental factors)
1) 알코올 섭취
만성 B형 간염의 예후는, 다양한 형태의 바이러스 및 인적 인자들(Host factor)에 의해 영향을 받는다.7
이중 알코올 섭취는, 그 자체가 간경변이나 간암과 같은 만성 간질환의 주요 원인이자, 다른 위험 요소가 동반되었을 경우
간 손상의 악화를 초래하는 것은 잘 알려져 있다.8
알코올 섭취는 B형 간염 보유자 및 만성 B형 간염 환자에서, 간경변으로의 진행을 증가시킬 뿐 만 아니라9, B형
간염에 의한 간경변 환자에서 비 바이러스성 간경변 환자에 비해 간암의 발생을 유의하게 증가시키는데, 이는 알코올과 만성
B형 간염이, 간암으로 진행되는 기전을 상호 공유하며, 서로 상승 작용을 나타내기 때문이다.10-13
간 손상을 유발할 수 있는 알코올 섭취량의 관점에서, 위험 한계량에 대해서 하루 30g에서 80g 사이로 보고하고
있으나 정해진 결론은 없다. 유럽의 연구에서는 하루 30g을 위험 양으로 보고하고 있으나,14 동양의 연구에서는 하루
20g의 음주로도 위험성이 관찰되었다.15
우리나라의 음주율 및 일인당 알코올 소비량은 세계에서 가장 높은 수준이며 점차 증가되는 추세이다.16 우리나라 B형
간염 보유자의 53.2%가 한 달에 한 번 이상 음주를 하고 있으며, 이중 11.8%가 위험 음주군에 해당하며, 교육
수준이 낮을수록, 흡연자일 경우, 비만일 경우에 위험 음주군이 많았다.17
만성 B형 간염의 치료 관점에서 보면, 백신 접종을 통한 일차적 예방뿐만 아니라, B형 간염 보유자나 환자에서
알코올 소비를 감소시켜, 질환의 진행을 줄이려는 노력을 기울여야 하며, 따라서 이들에 대한 알코올 소비에

대한 적절한 교육이 필요하다.
2) 흡연
흡연은 B형 간염과 독립적으로 간암의 위험 요소로 알려져 있으나, B형 간염과의 상호 작용은 잘 알려져 있지
않다.18,19 초기 연구에서는, HBV 감염과 흡연 사이의 상호 작용에 대한 결과도 다양하여, 일부에서는 흡연과 HBV
연관성을 보고하였으나,20,21 다른 연구에서는 관련이 없었다.22,23 이는 증례의 수가 작거나, 연구 설계의 차이에서
기인한 것으로 추정된다.24-27
간암 발생에서의 흡연과 바이러스 감염과의 상호 작용에 대해 여러 가설들이 제안되고 있다. 담배 연기 속에는 간에서
발암물질로 대사되고 활성화되는 여러 화학 물질을 포함하며,28 이러한 물질들이 간암 발생에 유도자로 작용하는데, 이는
HBV가 주로 만성 간염과 간경화를 통해 만성 염증과 세포 증식에 촉진제로 작용하는데 반하여, 서로 다른 간암 발생
단계에 작용하는 것으로 알려져 있다.29,30
유럽의 연구에서는 B형 간염 보유자 중 흡연자일 경우, 음주 횟수가 증가되고, 위험 음주군으로 진행될 가능성이
높아, 이것이 간암의 발생을 증가시키는 한 요인으로 보고하고 있다.31
전체적으로, HBV 음성 비흡연자와 비교하여 간암 발생의 위험은 HBV 음성 흡연자에서 1.87 (95% CI,
1.30-2.69), HBV 양성 비 흡연자에서 15.8 (95% CI, 9.69-25.7), HBV 양성 흡연자에서
21.6 (95% CI 15.2-30.5)로 높았다.32
2. 인적 인자(Host factors)
1) 당뇨 및 비만
당뇨는, 바이러스성 간염이나 알코올 섭취와는 독립적으로, 간세포암과의 odds ratio는 약 2.5로 알려져
있다.33,34
간경변은 당불내성 및 2형 당뇨병을 유발하며 또한 간세포암을 유발할 수 있는데, 당뇨와 간세포암의 연관성 분석에
일부 제한이 있으나, 당뇨 유병 기간이 길고, 당화 혈색소의 수치가 높으면 간세포암의 발생 위험이 증가되는 반면에
메트포르민 치료는 간세포암의 위험을 감소시킨다.35
만성 B형 간염 환자에서 당뇨와 간세포암의 odds ratio는, 젊은 연령을 제외 후 보정한 연구에서 9.22
(95% CI 4.52-18.80)였다.36
체질량 지수와 간세포암의 연관성은 지리적 위치, 알코올 섭취 혹은 당뇨의 병력과는 독립적이었다. 25337건의
원발성 간암의 증례를 포함한, 26개의 연구의 메타 분석에서는 체질량 지수 30kg/m2 뿐만 아니라 25kg/m2도
간세포암의 위험을 증가시키는 것으로 보고되었다. 5 단위의 체질량 지수 증가에 따른 상대 위험도는 1.39 (95% CI
1.25-1.55)이였고, 체질량 지수 >32 kg/m2의 환자들에서 4배 정도의 위험도 증가가 나타났다. 비만한
남성은 비만한 여성보다 간암 발생의 위험이 더 높았으며, 체질량 지수의 증가와 간세포암의 연관성은 C형 에 비해 B형
간염 환자에서는 미약하였다.37,38
2) 커피
커피는 전 세계적으로 가장 흔히 소비되는 기호 식품으로, 장기간의 커피 섭취는, 2형 당뇨병, 담석증, 파킨슨씨병,
심장 질환과 뇌졸중 등에 대한 위험을 감소시킨다. 미국 국립 보건원의 조사에 의하면, 커피 소비량과 총 사망률 사이에는
용량 의존적인 반비례 관계가 존재하며,39 매일 6잔 이상 마시는 경우 사망 위험을 남성과 여성에서 각각 10% 및
15% 를 감소시켰으며, 이는 이전에 커피 소비량이 질환에 의한 사망률을 감소시켰다는 보고들과도
일치한다.40,41

커피 섭취가 간질환 및 간암에 미치는 영향에 대해서도 다양한 연구들이 보고되었다.42-48 간에 대한 커피의 치료
기전은 정확히 알려지지 않았으나, 커피 섭취는 알라닌 아미노전달효소(alanine aminotransferase),
아스파르테이트 아미노전달효소(aspartate aminotransfer -ase) 및 감마 글루타밀
전달효소(r-glutamyl transferase)와 같은 간 효소를 감소시키고,49-51 간질환의 중증도를 낮추며,
간질환의 진행을 억제하는 것으로 알려져 있다.52-54
커피는 카페인(caffeine), 디터펜(diterpenes), 칼륨, 나이아신(niacin),
마그네슘(magnesium) 및 항산화제인 클로로겐산(chlorogenic acids) 및
토코페롤(tocopherols) 등을 포함하여 백 가지 이상의 화합물로 구성되어 있으며,55 이중 카페인과 디터펜(예,
cafestol and kahweol) 및 클로로겐산 등이 주로 연구되고 있다.
커피의 작용 기전은 여러 연구에서 알려졌으나,55,56 커피의 어떠한 성분이 간에 대한 효과를 나타내는지는 뚜렷하지
않으나, 여러 화합물에 의한 상호 보완적 상승 작용으로 인한 것으로 추정된다.57
최근 미국의 대규모 전향적 연구에 의하면, 커피를 마시지 않는 사람들과 비교하여 하루에 2-3잔의 커피를 마시는
경우 간암 위험률을 38%, 4잔을 마신 경우는 41% 감소시켰고, 만성 간 질환으로 인한 사망 위험은 각각 46%,
71% 감소시켰다. 그러나 이 연구에서는 아시아계를 포함한 소수 인구 집단이 충분히 포함되지 않아 커피가 간암과 만성 간
질환에 대한 효과를 단정하기에는 한계가 있다.58
아시아에서 발표된 연구를 살펴보면, B형 간염에서 커피의 효과는 뚜렷하지 않았다. 홍콩의 연구에서는, 1045명의
만성 B형 간염 환자를 대상으로, 매일 1잔 이상의 커피를 마신 군과 마시지 않은 군을 비교하였을 때 진행된 섬유화의
빈도는 차이가,59 234명의 B형 간염 보유자를 대상으로 한 연구에서 주당 4잔 이상의 커피 섭취는 간세포암의 발생을
60%까지 감소시켰다.60
싱가포르의 연구에서는 커피 섭취와 비 바이러스성 간염 관련 간경변 사망률의 위험 사이에는 강한 용량-의존적 반비례
관계가 있었으나, B형 간염 관련 간경변 사망률과는 관련이 없었고, 홍차, 녹차, 과일 주스, 또는 청량음료 소비 역시
관련이 없었다.61
우리나라의 연구에서, 커피의 섭취는 간암 발생에 대한 독립적인 방어 인자는 아니었다. HBV에 감염된 519
명(175명의 간암 환자와 344 명의 만성 간질환자)의 환자를 대상으로 한 연구에 따르면, 평생 커피 소비량이 많을수록
간암의 위험을 감소시켰지만, 연령, 성별, 비만, 많은 평생 음주/흡연, HBsAg 상태, 혈청 HBV DNA 수준과 항
바이러스 치료에 대하여 보정 후에는 통계적 유의성은 없었다.62
그러나, 커피의 효과에 관한 연구에는 많은 제한점이 있다. 모든 연구가 환자 스스로 작성한 설문지를 통하여 이루어
졌으며, 사회 경제적 지위의 차이와 다른 식사 요인을 충분히 반영하지 못했다. 커피를 마시는 사람들이 그렇지 않는
사람들에 비해 전반적인 건강 상태 및 생존 능력 지수가 더 열등한 경향이 있었으며, 흡연 및 알코올 소비량이 높은 경향을
보였다. 커피 잔의 크기와 커피의 종류도 표준화 하지 않았다. 또한 간암 발생이 장기적인 과정을 거치는 것에 비해,
대부분의 연구는 일생의 총 커피 섭취량 보다 최근 1-2년 동안 만을 분석했다. 따라서 이에 대한 추가적 연구가 필요할
것으로 판단된다.
3) 미량 원소 및 음식(Trace element & food)
셀레늄(Selenium)
셀레늄은 유리기들(free radicals)에 대한 보호 역할, 간의 해독 작용, 면역에 관련된 필수적인 미량
원소이다.63 C형 간염을 포함한 만성 간염 환자들에서 비감염자들에 비해서 셀레늄의 부족이 관찰되며, 일부 연구에서는
B형 간염 환자들의 혈중 셀레늄이 50%까지 감소되었으며, 셀레늄의 부족은 B형 간염의 중증도와 관련이 있었다.64
따라서 간 손상의 예방을 위하여, B형 간염과 C형 간염 환자에서는 셀레늄의 적정 여부를 평가하고 보충할 것을 권유하고
있다.64,65 4년간 진행된 동물 연구에서, 장기간의 셀레늄 보충은 B형 간염을 77%까지, 간의 병소를 75%까지
감소시켰다. 또 8년간의 셀레늄 공급을 통한 연구에서는 B형 간염 환자에서 간암의 발생을

35%까지 감소시켰다고 보고하였다.66
녹차(Green tea)
녹차와 그 주된 항산화 물질인 Epigallocatechin gallate (EGCG)는 실험실에서 isolated
liver cell의 HBV DNA의 replication을 억제함으로써 HBV DNA와 B형 간염 항원의 혈중 농도를
감소시키는 효과를 보였다.67,68 그러나, 환자를 대상으로 진행된 연구에서는 서로 상반된 결과가 관찰되었다. 중국에서
진행된 연구에 따르면 B형 간염 보유자들 중 간세포암을 진단 받은 204명의 환자에서 녹차의 섭취가 암의 진행을 절반까지
감소시킨다고 보고되었으나,69 110명의 간세포암 환자를 대상으로 한 일본의 연구에서는 의미 있는 영향을 주지
못했다.70
아연(Zinc)
신체의 많은 효소에서 발견되는 아연은 면역 조절에 있어서 중요한 역할을 하는 것으로 알려져 있으며,71 감염된
바이러스의 제거에 관여하는 T-cell 활성화에 영향을 끼친다.72 간질환과 관련하여, B형 간염 환아에서 몰리브덴,
망간, 셀레늄 뿐만 아니라 아연이 정상아들 보다 감소되어 있으며,71 성인 연구에서도 낮은 아연 수치는 간 손상을
반영하는 간 효소 수치의 상승과 관련이 있었다.65 아연 수치가 높은 소아에서 Interferon (IFN) 치료에 대한
반응이 더 좋은 것으로 보고되었고,73 B형 간염 환자에서 아연과 IFN-α을 함께 투여한 군과 IFN-α를 단독 투여한
군을 비교한 다른 연구에서는 치료 효과에 차이는 없었으나, 이는 투여된 아연의 농도가 낮았기 때문이라고 추측하고
있다.72
락토페린(Lactoferrin)
락토페린은 인체 세포들과 상호 작용 혹은 감염된 바이러스와 직접 결합하여, 바이러스 억제 효과를 갖는 항 미생물
단백질로 알려져 있다. 우유의 주 단백질인 락토페린의 항바이러스 작용은, 수유 시 B형 간염의 모자간의 수직 감염의 위험
감소로 설명될 수 있다.74 인간의 락토페린으로 전 처치를 한 간세포에서는 B형 간염 바이러스 감염에 대해 저항성을 갖는
것을 관찰되었으며75, 인간 간세포 배양에서 B형 간염 바이러스의 복제가 억제되었다.76
비타민 B군(Vitamins B)
만성 B형 간염 환자는 감소된 항산화 상태에서, 현저한 산화 자극의 증가와 지질의 과산화를 보인다.77 비타민 B1
(thiamine)은 B형 간염의 완화에 관련된 철분 대사 과정에서 중요한 항산화 및 조효소인
Dihydrolipoate의 합성에 필수적이다. 중국에서 시행된 소규모의 연구에서, 만성 B형 간염 소아에 대한 비타민
B1과 표준 IFN 치료의 병합 치료가 IFN 단독 치료에 비해 HBV DNA와 HBeAg의 감소가 관찰되었으나, 같은
환자들에게 비타민 B1을 2차로 투여한 연구에서는 효과가 확인되지 못했다.78 만성 B형 간염은 적혈구에서 비타민 B2와
B6를 감소시키는 것으로 알려져 있어, 비록 B형 간염의 질병 경과를 호전시키는지 여부는 뚜렷하지 않으나, 비타민의
보충이 도움이 될 수 있다.79
비타민 C와 E (Vitamins C and E)
만성 B형 간염 환자에서는 비타민 C와 E 또한 감소되어 있다.80 B형 간염에 감염된 소아와 성인에 대한 소규모
연구들에서 비타민 E 치료가 HBV DNA의 감소와 바이러스 항원에 대한 면역 반응의 조절 및 간수치의 정상화에 있어
항산화 작용으로 중요한 역할을 하고 있음을 보고하였다.78

레스베라트롤(Resveratrol)
폴리페놀의 일종으로 오디, 땅콩, 포도, 라스베리, 크렌베리 등의 베리류 등을 포함한 많은 식물에서 발견되며, 특히
적포도주에 다량 함유되어 있는데, 항암 및 강력한 항산화 작용을 하며 혈청 콜레스테롤을 낮춰 주는 역할을 한다.
B형 간염 동물 실험에서 Resveratrol은 간의 지방 축적 및 Mitochondria의 파괴와 같은 간세포의
구조적 변화를 감소시키고, 세포 내 Glutathione을 증가시키며, 활성 산소류를 감소시켰으며, 간세포암 발생을
5배까지 감소시키고, 간세포의 증식과 재생을 촉진시켰다.81
커큐민(Curcumin)
카레의 주성분으로, 강황 또는 울금(Curcuma longa)이라 불리는 식물의 뿌리에서 나오는 물질로서, 인체
간세포에 대한 실험에서 대사 조절 물질인 PGC-1α의 활성을 억제시켜서 B형 간염 바이러스 유전자들의 표현과 증식을
감소시킨다고 알려져 있다.82,83 PGC-1α는 공복시에 활성화 되어 당 생성을 증가시킬 뿐 아니라, B형 간염
바이러스의 복제를 증가시킨다고 보고되었다.83 과량 섭취 할 경우는 위궤양이 유발 될 수 있다.
N 아세틸 시스테인(N-acetyl-cysteine)
N 아세틸시스테인은 L-cysteine (시스테인)에서 유래되는 필수 아미노산으로, 강력한 항산화물로
유리기(free radicals)를 감소시키고 글루타치온(glutathione)을 증가시킨다.84 실험 모델에서 B형
간염 바이러스 입자의 결합을 방해하여 B형 바이러스를 감소시키는 것으로 보고하였으나,85 B형 간염 환자에 대한 소규모
연구들에서는 상반된 결과를 보여 아직까지 충분히 검증되지는 못하였다. N 아세틸시스테인을 200~1,800 mg/day
투여하였을 때 글루타치온을 증가시키고 빌리루빈은 감소시키는 것이 일부 실험에서 확인되었으나 간기능의 의미 있는 호
전은 나타내지 못했다.86,87 또한 N-아세틸시스테인의 투여 경로(경구 또는 정맥 투여)에 따른 바이러스의 감소
정도 또는 회복 소요 시간 단축에서도 뚜렷한 효과는 관찰되지 않았다.88,89
장내 세균 전이(Bacterial translocation)
간질환은 오랫동안 장내 세균총의 양적, 질적인 변화(dysbiosis)와 관련 있다. 동물 실험 모델에서 장내
세균총의 이상은 장내 염증 및 장관 방어 체계의 붕괴, 미생물에 의한 생성물의 전이(translocation)로
발생된다. 그러나, 단순히 미생물에 의해 생성된 물질의 전이로만 간손상과 염증이 유발되는 것이 아니라, 미생물의 대사
산물과 인체 인자의 상호 작용을 통하여 발생되나, 아직 명확한 기전은 밝혀지지 않았다. 알코올성 간질환에서의
프로바이오틱스(probiotics)의 효과는 입증되었으나,90 아직까지 B형 간질환에서는 연구가 미미하다.
B형 간염 환자와 알코올성 간경변 환자의 대변 미생물 군락의 분석해 보았을 때 병원성
Enterobacteriaceae와 Streptococcaceae가 증가되어 있으며, 유익한
Bifidobacteriae와 Lachnospiraceae가 감소되어 있었다.91,92,95 또한 carbon
tetrachloride을 통해 간경변을 유발한 쥐에서도 Enterobactericeae의 현저한 증가가 관찰되었다.93
간경화가 있는 동물에게 norfloxacin 같은 항생제나 Bifidobacterium 같은 프로바이오틱스는
Enterobacter를 감소시켰고 Bifidobacterium과 Lactobacillus를 증가시켰다.94 이것은
결과적으로 체내 내독소(endotoxin)를 감소시킴으로 간기능 개선을 유도한 것이다. 따라서 장내 세균총을 정상화시키는
프로바이오틱스가 B형 간질환 개선에 도움을 줄 가능성이 있음을 추측할 수 있다.95
운동
운동은 인슐린 저항성과 관련이 있다.96 유산소 운동이 만성 간질환 환자에서 인슐린 저항성을 개선시키며,97,98
특히 비만 환자에서 그 효과가 뚜렷하였다.97,99

운동 능력과 간질환의 중증도는 반비례 관계가 있으며,100-102 운동 능력의 감소는 간경변의 중증도와 관련이 있을
뿐만 아니라 간이식후 사망률 예측과 관련이 있다.103-104
아직까지 B 형 간염 환자에 대한 적합한 운동 처방은 밝혀진 바 없다. 그러나, 초기 대상성 간경변 환자들을
대상으로 한 연구에서, 체중 당 약 30 kcal 이상의 칼로리 섭취와 함께 하루에 5,000 걸음 혹은 그 이상의
보행이 부작용 없이 근육 감소증을 개선하고 예방할 수 있음을 보고하고 있다.105 실제로 노인 혹은 일부 만성 질환
환자에서 운동 관리는 근육 감소증의 호전 및 예방에 효과적이었다.106,107 최대 심박수의 60-70%의 강도로 주당
3회 1시간의 유산소 운동으로, 운동 능력이 향상되고, 근육량이 증가되어 삶의 질이 개선되었다.

B형 간염과 관련된 질환의 악화를 예방하고, 치료 효과를 극대화하기 위하여, 바이러스 치료제와 더불어, 개인에

만성 B형 간염 및 보유자에서, 음주, 흡연, 비만 등이 간암의 발생에 각각 독립적인 기여 인자임을 고려할 때, 그
치료는 단지 절제뿐 아니라 전반적으로 건강한 생활 습관을 증진시키는 교육을 포함하여야 하며, 이를 목표로 하는 건강
정책들 혹은 중재적 프로그램(intervention program)이 마련되어야 한다.
