FACTS OF ELECTRICAL INCIDENT

• Date : Mon 15 Oct 2007

• Location : Overground Box (OGB)

• Deceased: A 35-year old male

FINDING

The deceased was electrocuted while attempting

to steal cables.

CASE STUDY NO. 2 – SOMERSET 66KV SUBSTATION

FACTS OF ELECTRICAL INCIDENT

• Date : Wed 02 Jul 2008

• Location : Somerset 66kV substation switchgear bay

• Injured: A 34-year old male who suffered second

degree burns on the right side of the body, right arm

and right leg.

FINDING

The lack of clear instructions from the maintenance

team leader and incorrect marking on site misled a

worker into working on Isolator 6-289 instead of

Isolator 6-189 resulting in a flashover.

LESSONS LEARNT

• The Authorised Person from SPPG failed to follow

established practice as specified in SPPG’s document

“Rules for the Control and Safe Operation of High and Low

Voltage Apparatus” – 4th Edition June 2006

• A clear message must be sent to all SPPG Operational

Personnel that safety is of paramount importance and that

deviation from best practices will not be tolerated.

• SPPG was fined S$850K under our Financial Penalty

Scheme for the outage caused.

• Authorised Officer was also penalised.

• When carrying out cable repair work, there is a

need for proper supervision by the Authorised

Person over the entire cable cutting process.

• Need to develop and prescribe the types of

support for different types of cables and different

types of cutting tools.
