    Introducing CVS Caremark!Beginning January 1, 2017, Aetna/Innovation Health and CareFirst members will transition to CVS Caremark for management of prescription benefits. This change will provide members additional convenience and savings options, while providing access to a broad network that includes thousands of retail pharmacies nationwide.

    In addition to CVS Pharmacy, members will continue to have access to major retail pharmacy chains, such as Walgreens, Giant, Safeway, Wegmans, and many others. Copayment levels will remain the same, and new savings options are available for members who take maintenance medications.

    New cost-saving options for maintenance medicationso Beginning January 1, you have the option of filling maintenance medications at a

    CVS Pharmacy and enjoying the savings previously available only through mail order. Members will be able to fill up to a 90-day supply of maintenance medications at a CVS Pharmacy and pay only 2 copayments. You may also continue to receive up to a 90-day supply of maintenance medications through mail order and pay 2 copayments, or use a non-CVS retail Pharmacy and pay 3 copayments.

    Additional convenienceo Employees who take specialty medications will transition to CVS Specialty Pharmacy.

    While speciality medications will continue to be filled via mail order, you will also have the option of confidential and convenient delivery of your specialty medications to CVS pharmacies at no additional charge.

    o Diabetes supplies (including glucose meters, lancets, and test strips) will be available through your prescription benefit instead of your medical benefits. Some diabetes equipment, such as insulin pumps, will continue to be covered under your medical plan.

    Except where prohibited by law, your medication history, remaining refills and existing mail-order prescriptions will be transferred from Express Scripts to CVS Caremark in early December. You will receive a new pharmacy ID card from CVS Caremark in mid-December. Please make sure you provide this card to your pharmacy in the new year.

    Look for more communications later this fall on how to begin accessing your benefits under CVS Caremark. You may also visit http://info.caremark.com/fcps for more information.

    Please note: Express Scripts will continue to provide pharmacy benefits for Aetna/Innovation Health and CareFirst members through December 31, 2016. No change in pharmacy benefits manager will occur for Kaiser Perman-ente participants.

    Preventive medications covered by the Affordable Care Act remain available for $0 copay. This includes aspirin for pre-eclampsia, oral fluoride, folic acid, smoking cessation, vitamin D, women's contraceptives, and colonoscopy preparatory agents. Age limits may apply to some medications.

    2017 Means Focus on WellnessNew for 2017 Earn a $100 Wellness Incentive!As part of the strategic plan, FCPS has made an ongoing commitment to support employee health and well-being. In 2017, employees participating in an FCPS health plan will have an opportunity to earn a $100 wellness in-centive for completing their health vendors online health assessment (HA).

    The HA is a questionnaire related to different aspects of general health and well-being that is easy to complete in about 15-20 minutes. It provides a snapshot of your current health status and includes a wellness score so you can become familiar with any potential health risks and strategies to help reduce them. Your individualized health report may also include information on how to obtain health coaching and/or receive other health or well-ness programs that are offered through your health plan.

    The HA is confidential! Only you and your health plan receive the individual information. Your health-related information is not shared with FCPS in compliance with state and fed-eral privacy laws.

    Continued Focus on Disease ManagementIn 2017, there will be an increased focus on disease management programs (DM) through all of the health plans. DM programs are designed to help people with certain chronic disease conditions better manage their health and decrease complications by coordinating the delivery of high quality care. DM pro-grams do not replace your doctors care or advice. Since chronic disease conditions are one of the highest drivers of health care costs for FCPS, these programs can also help to save you and the plan money.

    Your insurance plan may identify you to par-ticipate in a DM program through a physician referral, medical claims or when you take your annual health assessment survey. If identified, a registered nurse or other health professional would contact you by phone or mail to engage in one-on-one support tele-phonically or to participate in online health coaching programs. They can help you moni-tor your condition, answer your questions and provide personalized care. You will choose whether you want to access the program and/or how often you would like to be contacted. There is no cost to you for accessing resourc-es through the DM program.

    Participation in DM programs is confidential and is done strictly through your insurance plan. FCPS does not receive any identifiable health information in accordance with state and federal privacy laws.

    2017 Premiums

    1 All benefits-eligible employees in active status pay the same rates, regardless if part-time or full-time. Monthly paid employees have deductions taken September through June. Biweekly paid employees have deductions taken October through June.2 Employees and their spouses who both work for FCPS in benefits-eligible positions are eligible for a spousal discount on their health and dental insurance. The FCPS Spousal Rates reflect an employee contribution of 20% of total premium for medical and dental coverage. If you are eligible but not currently receiving this discount, complete the FCPS Spouse Health Plan Deduction form (HR Form 134) and submit during Open Enrollment. As a reminder, employees are required to notify the Office of Benefit Services within 30 calendar days of any event that would cause an employee to qualify for (or cease to be eligible for) the discounted rate; including marriage, divorce, termination of employment, or commencement/termination of a leave of absence.3 COBRA premiums apply to employees/dependents who are eligible and have elected COBRA continuation coverage. Premiums are paid monthly (over 12 months) on a direct bill basis.

    Additional Updates for 2017 VRS Hybrid Plan MembersVoluntary contributions increase in January, unless you opt out

    Most full time, instructional/school-based employees hired after January 1, 2014, are members of the VRS Hybrid Retirement Plan. Hybrid members have a unique opportunity to save for retirement by enrolling in the VRS voluntary 457 retirement savings plan, which provides pro-rated matching contributions to the defined contribution component of their retirement account. To encourage members to maximize their retirement savings, the VRS Hybrid Retirement Plan is designed so that voluntary contributions automatically increase by 0.50% every three years. Beginning January 1, 2017, the first automatic increase, or auto-escalation, will occur.

    Increasing your retirement savings not only provides you with additional financial security at retirement, it also allows you to receive additional retirement contributions from FCPS through matching contributions. Below is the schedule of matching contributions.

    VRS will notify members about the automatic increase before the December 15th opt-out deadline. While you may elect to opt out, you will lose any matching contributions, which may lower the dollars available at retirement.     Health Plan Task ForceEarlier this year, Superintendent Dr. Garza convened a Health Plan Task Force to review and evaluate current FCPS health programming options and develop recom-mendations for plan structures to be implemented in calendar year 2017 and beyond. Task Force Members included representatives from a variety of employee associations, councils, and groups who serve in numer-ous functional roles throughout the system, a citizen representative, and representatives from the two retiree associations. The wellness incentive being implemented in 2017 and the plan changes being evaluated for 2018 were among the Task Force key recommendations. Ad-ditional recommendations of the Task Force are being evaluated for future years. A well-deserved thank you to the members of the Task Force for the many hours they devoted to these meetings.

    Dental EnhancementsEach year, FCPS evaluates coverage under the medical and dental programs to ensure the benefits remain market relevant. As part of this review, some enhancements will occur to the Aetna DPPO plan in 2017. Starting next year, coverage levels will be enhanced for periapical x-rays and re-cementation of crowns and dentures. Additionally, orthodontia for dependent children remains a covered benefit if work began prior to age 20 (currently age 19).     Enhanced Vision Benefits for Kaiser Permanente ParticipantsStarting January 1, Kaiser Permanente participants will be eligible for a 25% discount on frames and lenses, and a 15% discount on contact lenses, when obtained at a Kaiser Permanente optical facility. This discount provides greater savings for vision hardware, and is in addition to the $150 vision benefit allowance currently provided to all participants.     Expecting Parents Mini-WorkshopsThe Office of Benefit Services will offer mini-workshops during this years Open Houses, providing information on how to prepare and plan for maternity/paternity-related leaves. Participants will learn about the maternity programs available through their health plan, review the FCPS leave benefits available, and receive information about other resources available.

    The workshop is open to all employees who would like more information on these benefits provided by FCPS. Spouses and significant others are welcome to attend! There is no charge for the workshop. You must register through MyPLT.

    Health Plan Changes Being Evaluated for 2018Looking forward to calendar year 2018, FCPS is evaluating common practices of other large employers, along with recommendations of the FCPS Health Plan Task Force that was formed last spring. Currently, the following changes are being evaluated and considered for implementation in 2018:

    Tobacco-free DiscountIn addition to the negative impact to individual health, use of tobacco products is widely documented to create thousands of dollars additional in health care costs. Many employ-ers offer discounts to employees who do not use tobacco products in any form, and FCPS is evaluating adding this incentive to health plan premiums for 2018. The amount of the discount is yet to be determined. Next fall, employees may be asked to attest that they have not used tobacco products for at least the previous 6 months in order to receive the discount. If you currently use tobacco products, there are many resources available to help you quit.See page 9 for resources on tobacco cessa-tion programs.     Spousal SurchargeIn order to keep coverage affordable, FCPS will be evaluating a "surcharge" for spouses who have access to other health coverage, but are enrolled under the FCPS health plan. Spousal surcharges are common com-ponents of large employer plans as a way to minimize costs and keep participant premi-ums low. Surcharge amounts, and a definition of what constitutes other coverage, are still being evaluated.

    Planning for Retirement in 2017?If you are considering retiring from FCPS during 2017, its important to consider the following during Open Enrollment. A little preparation and research now means a smooth transition into retirement.

    Will I be eligible for Medicare at retirement?FCPS health plans require that retirees (and dependents of retirees) enroll with Medicare Parts A&B when first eligible. Failure to enroll in Medicare could result in significant ex-pense, as claims will pay as if Medicare were in place. Aetna/Innovation Health and Kaiser Permanente are the two FCPS plans avail-able to Medicare retirees.If you will be eligible for Medicare at time of retirement, we encourage you to coordinate enrollment with Social Security (typically 3 months prior to date of retirement).Please note: active employees who are Medicare-eligible do not have to elect Medicare coverage until retirement. This is also true for dependents of active employees.

    Does my health plan have a deductible? If you anticipate changing plans during the calendar year, consider any potential impact of meeting your deductible or out-of-pocket maximums. Reasons that may cause you to change plans mid-year, include moving out of the plans service area or eligibility for Medi-care.     Free Seasonal Flu ImmunizationsFree flu shots continue to be offered for all active employees on assignment at a FCPS site during Fall 2016. This includes hourly, temporary, and substitute employees currently on assignment. In addition to most schools holding flu shot clinics of their own, Employee Wellness also sponsors several large clinics throughout the season. Registration is required on MyPLT.Employees must present their employee ID cards. FCPS retirees and family members of employees and retirees (18 and older) may also receive a seasonal flu immunization at any FCPS clinic for $28, payable to INOVA by cash or check at the time of the immunization.     Review your health care claims to help estimate your out-of-pocket expenses for 2017. Think about enrolling in a Flexible Spending Account (Health Care and/or Dependent Day Care) to help with those out-of-pocket costs and help you save money on taxes in 2017.

    Become familiar with each vendors tools and resources, which will help you determine which plan might best fit you and your familys needs.

    If you will be requesting coverage for your spouse and/or dependent children who are not currently covered under an FCPS plan, begin gathering legal documentation needed to add them to your coverage.

    During Open Enrollment: Review this Benefits Briefing carefully as you consider your plan choices. Take

    advantage of the many open enrollment resources available to you, including information on the FCPS and benefits vendor websites, attend a Benefits Open House, or visit the Gatehouse Administration Center.

    After Open Enrollment: Review carefully the benefits confirmation statement that will be sent to your

    FCPS email account by the end of November.

    Aetna Innovation Health and CareFirst participants:- Look for new pharmacy identification cards in the mail by mid-December. - Ensure you provide copies of your cards to your local pharmacy in the new year. - Begin planning for transition to your pharmacy benefits. Make sure you have an appropriate supply of your medication to ensure a smooth transition.

    Employees covering spouse/dependent children, please note FCPS must collect your spouse/dependent child(ren)'s SSN for reporting health plan enrollment to the IRS. Please review these SSNs to ensure they are accurate. If you are requesting coverage for your spouse and/or dependent children, you must supply the
