I. STATEMENT OF PURPOSE
This Emergency Action Plan (EAP) has been prepared on behalf of
the General Electric Company
(GE) for Woods Pond Dam, which is owned by GE. The purpose of
this EAP is to:
1. Define the responsibilities of various officials involved in
emergency management;
2. Provide procedures designed to identify, in a timely manner,
unusual and unlikely
conditions which may endanger the structural integrity of Woods
Pond Dam;
3. Identify actions to take to mitigate the situation or
developing situation; and
4. Provide procedures to notify the appropriate emergency
management officials, if
possible, of impending or actual failure of the dam.
In accordance with the regulations of the Massachusetts
Department of Conservation and
Recreation, Division of State Parks and Recreation (DCR)
(formerly known as the Environmental
Management (DEM)) Regulations (Chapter 302 CMR 10.00, December
27, 1996), Woods Pond
Dam is designated as a "large" size dam with a "significant"
hazard (Class II) potential. This size
classification is based on the storage capacity that occurs
during the spillway design flood, while the hazard potential
classification is based on the presumption that failure of the dam
may result in the
loss of life and significant property damage immediately
downstream of the dam.
According to DCR Regulations (Chapter 302 CMR 10.11, December
27, 1996), all dams classified
as "high" hazard (Class I) shall have an EAP. If the
Commissioner requires it, a facility with a non-
high hazard dam shall also provide an EAP. DEM requested that GE
prepare an EAP for Woods Pond Dam; GE prepared this EAP in response
to that request and as an exercise of good engineering judgment.
The EAP conforms to the requirements for EAPs outlined in Section
10.11 of the DCR
Regulations. All EAPs are subject to approval by the DCR. The
dam owner shall submit a copy of
the proposed EAP to the local and state emergency agencies and
all local emergency coordinators
involved in the plan for review. The owner shall incorporate
applicable recommendations received
and provide the documented recommendations in appendices to the
final version of the EAP.
Annually, the owner shall review the EAP and update it as
necessary

The EAP shall, at a minimum, contain the following:
1. The identification of equipment, manpower and material
available for implementation of
the plan;
2. A notification procedure for informing local emergency
agencies;
3. A topographic map showing the stream which will be flooded;
and
4. A procedure for warning nearby local residents if failure of
the dam is imminent and a
listing of addresses and telephone numbers of downstream
residents that may be affected
by failure of the dam.
As described in Section IV, "Emergency Detection, Evaluation and
Classification," two alert
conditions have been adopted for Woods Pond Dam:
1. Condition A - Failure Is Imminent or Has Occurred, and
2. Condition B - Potential Failure Situation Is Developing
A notification flowchart for each of these alert conditions is
presented in this EAP.

II. PROJECT DESCRIPTION
A. Background
The original Woods Pond Dam was reportedly constructed around
1864 on the Housatonic River
near the towns of Lee and Lenox, Massachusetts, as shown on
Exhibit 1. The original structure
consisted of a 9-foot high rock-filled timber crib overflow dam
with a 14-foot-high earthfill
embankment parallel to the river channel as its east (left)
abutment (left and right refer to the side of the river channel
when viewed looking in the downstream direction). There are no
design drawings
or information available for any of the original project
structures. The original dam structure was replaced in 1989 with a
new concrete overflow spillway and new abutments of concrete and
steel
sheet pile. The earthfill (raceway) embankment separates the
main river channel from the 30-to-50 foot wide raceway channel
along the left bank of the river. The raceway channel conveys flow
to a
small mill pond, which at one time was a forebay for a
water-powered mill that has since been retired. The dam impounds
the Woods Pond Reservoir, part of the Housatonic River Valley
Wildlife Management Area.
Prior to the 1989 rehabilitation, Woods Pond Dam was in a
deteriorated condition. The original project could not pass a flood
greater than about the 10-year flood event without overtopping
the
earthen raceway embankment. Overtopping of the embankment could
have caused a breach of the embankment, resulting in the
uncontrolled release of the reservoir water and sediments that had
settled to the bottom of the reservoir.
Beginning in 1979, studies and investigations were performed to
assess the condition of the dam and develop recommendations for
repair and rehabilitation of the dam. In 1983, the timber planking
and heavy gage sheet metal on the main overflow dam were replaced
with an 18-inch-thick reinforced
concrete cap. After placement of the cap, a leak developed at
the east abutment masonry wall, presumably due to the cut-off of
natural seepage through the timber crib dam. The joints in the
masonry wall were filled with mortar shortly after the 1983
construction, temporarily stopping the leakage, but contributing to
pressure buildup behind the wall, causing lateral movement of the
wall.
In 1988, a plan was developed to rehabilitate the dam to provide
a safer, more reliable project
structure. The rehabilitation was carried out in two stages. The
first stage consisted of the
construction of a raceway closure structure with concrete
stoplogs to divert flow away from the
raceway and mill pond. The purpose of the closure structure is
to protect the raceway embankment
from overtopping and potential failure during high flows. The
second stage consisted of the
construction of a replacement spillway and non-overflow gravity
dam a short distance downstream
of the original spillway. A rockfill berm was also constructed
on the west (right) bank of the river

between the original spillway and new spillway, and the top 2.5
feet the original timber crib
spillway was demolished. The original sluice gate structure at
the downstream end of the raceway
embankment was rehabilitated in 1991. This structure controls
the water level in the Mill Pond.
Additional modifications to the project were performed in 2001
to address recommendations made
in the January 2001 Structural Integrity Assessment Report, and
in 2003 to address recommendations made in the May 2003 Structural
Integrity Assessment Report. The 2001
modifications included repair to deteriorated masonry at the
raceway closure structure; removal of
trees and other vegetation along the raceway embankment;
regrading of the raceway channel slopes;
and removal of the masonry wall and reworking of the riprap at
the upstream raceway embankment. The 2003 modifications included
removal of trees and other vegetation and the placement of riprap
along the river side of the raceway embankment. Based on the
structural integrity assessment
inspection conducted in October 2002, the dam, raceway closure
structure, and riprap constructed in 1989 are in good condition,
and there was no indication of deterioration or distress to the
spillway
and tailrace.
A general plan of the rehabilitated project is shown on Exhibit
2. Drawings for the 1989 rehabilitation are included in Appendix A
for reference. The "General Design Report for Woods Pond Dam
Rehabilitation" for the 1989 rehabilitation was filed with the DEM
and is available from GE. It also appears in Appendix B of the
March 1999 Woods Pond Dam Inspection Report.
B. Project Features
The present dam consists of a 140-foot-long concrete overflow
spillway, a concrete non-overflow gravity section at the west
(right) abutment, and a concrete and steel sheetpile raceway
closure
structure at the east (left) abutment, all constructed since
1989. All the dam structures are founded on bedrock. The dam has a
maximum height of approximately 14 feet, with an ogee-shaped
spillway with a crest elevation of 948.3 feet National Geodetic
Vertical Datum (NGVD). The west
(right) abutment is a non-overflow gravity structure with a
sloped downstream face and a top elevation of 954.0 feet NGVD. The
raceway closure structure forms the east (left) abutment of the
dam and has a top elevation of 954.0 feet NGVD. The drawings for
the dam and raceway closure
structure are provided in Appendix A. The current structure is
completely independent of the original structure and does not rely
on any of the original structures for support.
The current structures are founded on shallow bedrock, which is
vertically bedded and is generally
fine grained, hard with variable medium to close joint spacing.
This was determined during the
1988 geotechnical investigation program which included soil
borings and water pressure testing of
the rock. Details of the subsurface field investigation can be
found in the "General Design Report

for Woods Pond Dam Rehabilitation" filed with the DEM in 1989
which was also included in the
March 1999 Woods Pond Dam Inspection Report.
The raceway embankment upstream of the dam is earthen fill with
riprap over the surface. The
raceway embankment upstream of the dam is no longer a
water-retaining structure and serves no
dam safety function. Failure of this portion of the raceway
embankment would have no impact on
the safety or integrity of the dam.
Downstream of the dam and raceway closure structure is an
earthen structure protected with riprap
along the length of the channel. This embankment is located
between the tailrace of the current
overflow spillway and the raceway channel. The embankment has a
crest width of about 8 to 10
feet. The water elevation in the raceway is a few feet higher
than the water elevation in the tailrace
and river channel downstream of the dam.

III. POTENTIAL FLOOD LIMITS
Exhibit 3 shows a map of potential flood limits due to the
failure of Woods Pond Dam under
"Sunny-day" and "Flood-day" conditions. Flood limits are for
failure when the abutments would be
overtopped by two feet. "Sunny-day" means a fair-weather,
non-rainy day. "Flood-day" means a
rainy, non-fair-weather day. The term "day" includes nighttime.
The potential flood limits are
plotted on Exhibit 3 to show the maximum inundation area due to
dam failure to the point
downstream where the water would be contained within the banks
of the river. These limits were
estimated based on the information and data provided in two
reports entitled:
1. "Woods Pond Dam, Phase 1 Inspection Report, Lee and Lenox,
Massachusetts, prepared
for EPA Region 1, prepared by U.S. Army Corps of Engineers,
Omaha District,
September 1987,"
2. "Flood Insurance Study, Town of Lee, Massachusetts, Berkshire
County by Federal Emergency Management Agency, December 1,
1981."

IV. EMERGENCY DETECTION, EVALUATION, AND CLASSIFICATION
This EAP has been prepared to aid GE personnel in the timely
detection, evaluation, and
classification of an existing or potential emergency situation
at the dam. The EAP provides two
classifications of dam failure emergency according to their
severity and urgency: Condition A
Failure is Imminent or Has Occurred, and Condition B - Potential
Failure Situation Is Developing.
The conditions or measures for detection and evaluation of an
existing or potential emergency are
described below.
For purposes of this EAP, GE has designated a Facility Manager
and an Alternate Facility Manager
(as specified in Section V). The Facility Manager shall have
primary responsibility for carrying out
this Plan on behalf of GE. In the event that the Facility
Manager is unavailable, the Alternate
Facility Manager shall be the point of contact and shall have
all the duties and powers of the Facility
Manager as set forth in this Plan. In addition, GE has
pre-selected contractors, also specified in
Section V, to assist in emergency response under this EAP.
A. Condition A - Failure is Imminent or Has Occurred
The following conditions may lead to the "Failure is Imminent or
Has Occurred" emergency
situation. These conditions should not be considered inclusive
of all conditions that might occur:
Rapidly developing structural problems involving the spillway,
raceway embankment,
abutments, and appurtenant structures. Some of the potential
causes of developing
structural problems are listed on Exhibit 4.
The notification procedures of Section V, "Notification
Flowcharts and Contact Numbers" and
given on the flowchart for Condition A (Exhibit 5), should be
invoked if any of the situations listed
on Exhibit 4 are observed. An imminent situation at the dam site
will probably manifest itself as an
excessive increase in flow in the Housatonic River and/or an
unexpected drop or rise in water level
in the reservoir, river channel, raceway channel or mill
pond.
When failure is observed, the Observer (as defined in Section V)
shall inform the Facility Manager.
The Facility Manager shall decide the severity of the problem
and invoke the notification procedure
if appropriate.
The message from the Facility Manager to the persons identified
on the flowchart for this situation
(Exhibit 5) will contain the following information:
Name and title (GE position) of the person making the call.

A statement that this is an emergency call, due to a reported
failure or imminent failure of Woods Pond Dam.
A statement that the person receiving the message must initiate
contacts as per the flowchart for "Condition A - Failure is
Imminent or Has Occurred."
A statement that warning and evacuation should begin
immediately.
Signs will be posted around the dam site showing the phone
number to call in case an emergency situation is observed.
B. Condition B - Potential Failure Situation is Developing
The conditions that may lead to the "Potential Failure Situation
is Developing" emergency situation are also listed on Exhibit 4.
Upon confirming any of the situations listed on Exhibit 4, the
notification procedures will be implemented by the Facility
Manager. There will be two messages from the Facility Manager for
this situation. The first message will contain the following
information:
Name and title (GE position) of the person making the call. A
statement that a potential failure situation is developing at Woods
Pond Dam that may
lead to failure of the dam.
A statement that the person receiving the message should put his
or her contacts on alert for a second notification message.
A statement that emergency personnel should be ready for an
evacuation process.
If failure of the dam becomes imminent, the second message from
the Facility Manager shall be as given in this section under
"Condition A - Failure Is Imminent or Has Occurred."
If the dam failure is averted and the developing situation is
mitigated, the emergency will be called off via the same
communication channels. In such an event, the message will contain
the following information:
Name and title (GE position) of the person making the call.
A statement that the potential failure situation at Woods Pond
Dam has been averted, and that the recipients' contacts should
stand down from emergency alert.

V. NOTIFICATION FLOWCHARTS AND CONTACT NUMBERS
The flowcharts shown in Exhibits 5 and 6 summarize who is to be
notified and who is responsible
for notifying individuals and organizations in the order
designated for the following emergency
situations:
A. Failure Is Imminent or Has Occurred (Condition A):
Corresponds to situations where a
failure either has occurred, is occurring, or obviously is just
about to occur.
A. Potential Failure Situation is Developing (Condition B):
Corresponds to situations
where a dam failure may develop, but preplanned actions may
prevent or mitigate
failure. Generally, there is more time available under this
condition than under
Condition A to take preventative action and bring the situation
under control.
A copy of the flowcharts shall be prominently posted near the
telephone or radio transmitter of each
person identified on the flowcharts as a caller. The key
personnel of GE and police departments shall have copies in their
offices and at homes (see Appendix C for EAP distribution).
Detailed information supplementing the flowcharts is provided in
Section IV - Emergency
Detection, Evaluation, and Classification. The location of the
dam, reservoir, and streams are
described in the Section II - Project Description. Potential
inundation limits are discussed in
Section III - Potential Flood Limits. In addition to the
individuals and agencies listed as contacts in Actions 1 through 8
for Condition A and Condition B, GE has additional notification
requirements
under the Consent Decree for the GE-Pittsfield/Housatonic River
Site (the Consent Decree). The contacts for those notifications are
listed in Action 9. Paragraph 90 of the Consent Decree requires
that notice be given "[i]n the event of any action or occurrence
during the performance of the Work
which causes or threatens a release of Waste Material from the
Site that constitutes an emergency
situation or may present an immediate threat to public health or
welfare or the environment." The
phrase "constitutes an emergency situation or may present an
immediate threat to public health or
welfare or the environment" is further defined to mean "an
unforeseen combination of
circumstances, or the conditions resulting from such
circumstances, not normally anticipated to
occur as part of the Work, that require immediate action to
avoid harm or an immediate threat of
harm to human health, welfare or the environment."
For Condition A, notification to the contacts listed in Action
10 is required. For Condition B, the
Facility Manager shall provide notification to the contacts
listed in Action 10 as appropriate, based
on the requirements of Paragraph 90 of the Consent Decree quoted
above.

A. Condition A - Failure Is Imminent or Has Occurred
In the event that failure is imminent or has occurred at Woods
Pond Dam, GE personnel shall
initiate the notification actions in accordance with the
flowchart shown on Exhibit 5. The positions
and the sequence of notification for the flowcharts are
explained below.
Observer
The Observer will be the GE employee or contractor who monitors
the condition of the dam and its appurtenant structures (as
described in Section VI.A). In the event of a threatening situation
at the
dam site, the Observer shall immediately notify the following GE

VI. GENERAL RESPONSIBILITIES UNDER THE EAP
Failure of Woods Pond Dam would present a potential hazard to
downstream areas. Areas of
concern at the project include a few houses located near the
gravel pit and sewage disposal plant.
Numerous downstream roads and houses, primarily in the flood
plains and close to the banks of the
river, would be flooded. Refer to the inundation map, Exhibit
3.
Once an emergency condition is identified, it is important to
the success of the EAP that responsible
personnel respond immediately. Specific responsibilities of the
persons or organizations for the maintenance and operation of the
dam and implementing various phases of the plan are discussed
below.
A. General Electric Responsibilities
GE has a plant in Pittsfield, Massachusetts, located
approximately 12 miles upstream from the dam. A member of GE's
Corporate Environmental Programs (CEP) staff from that plant, or a
contractor designated by GE, will perform a routine visit to the
dam site and its appurtenant structures at least monthly (or more
frequently as warranted by weather, river levels, or other
conditions). This person will be the "Observer," as defined in
Section V ("Notification Flowcharts and Contact Numbers") in case
of an occurrence at the dam. The person will carry a two-way radio
and/or telephone and/or pager, and will be properly equipped to
perform a watch during night and severe weather.
In addition, the Facility Manager or the designated contractor
will conduct an inspection of the dam and its appurtenant
structures at least quarterly to identify any changes in site
conditions that might indicate a problem for the dam. (This
inspection will be combined with one of the monthly site visits
described above.). GE will provide a report on each such quarterly
inspection to the Lead
Administrative Trustee (LAT) designated under Paragraph 113 of
the Consent Decree.
GE will also arrange for a licensed professional engineer to
conduct an assessment, at least once every two years, of the
structural integrity of Woods Pond Dam, and to provide to GE a
report on
that assessment and any recommendations of measures necessary to
maintain dam integrity. GE
will provide a copy of each such report to the LAT.
The GE Facility Manager will have the authority to declare an
emergency at Woods Pond Dam or to
change the degree of severity of an existing occurrence.
Upon receiving information of a threatening situation at Woods
Pond Dam from the Observer (or
from other sources), it will be the primary responsibility of
the Facility Manager to direct the

APPENDIX B
INVESTIGATION OF POTENTIAL FLOOD LIMITS
Areas and streams downstream from Woods Pond Dam (Valley Mill
Dam) that would most likely
be inundated in case of a failure of the dam have been
identified.
A dam break study was not performed for Woods Pond Dam because
it was not needed as per
requirements of the DCR. The stream affected due to a failure
would be the Housatonic River.
There would be some backwater effect on small tributaries
joining the Housatonic River (from
upstream to downstream: a small unnamed stream, Washington
Mountain Brook, Codding Brook,
small unnamed stream, Goose Pond Brook and Hop Brook). Further
downstream, the backwater
effect on the tributaries might not be significant. Exhibit 3
shows a portion of the Housatonic River
and tributaries downstream from the dam.
Although the DCR does not require a dam failure inundation map
for "significant" hazard (Class II)
dams, the identification of roads and residences likely to be
affected due to a failure of Woods Pond Dam was considered
necessary for warning purposes. This required some approximation
of
inundation boundaries along the Housatonic River. To draw these
inundation boundaries, guidance
was obtained from two previous reports. First was the dam break
analysis made by U.S. Army
Corps of Engineers, Omaha District (COE), presented in the Woods
Pond Dam Phase 1 Inspection
Report, Lee and Lenox, Massachusetts, September 1987. The second
report was the Flood
Insurance Study for the town of Lee, Massachusetts, Berkshire
County, Community No 250028, December 1981 by the Federal Emergency
Management Agency (FEMA).
A hypothetical failure of Woods Pond Dam could occur either on a
sunny day or during a flood. For
failure under the sunny day condition, COE assumed the failure
to occur at a reservoir elevation of
952.7 ft, 4.4 ft above the spillway crest. Failure under this
condition resulted in a maximum outflow
of about 11,200 cfs and a water surface elevation of about 948.2
feet at a distance of 0.5 mile
downstream from the dam. The FEMA study indicated a 100-year
flood of about 11,700 cfs and a
500-year flood of about 16,300 cfs.

APPENDIX C
PLANS FOR TRAINING, UPDATING, AND POSTING THE EAP
Training
The Facility Manager will conduct a biennial in-house EAP
training session. In that session, the
Facility Manager will present detailed instructions, as noted
earlier in the plan, to GE personnel to follow during a dam failure
emergency. The instruction will include description of the chain
of
command. Technical personnel will be trained in problem
detection and evaluation and appropriate remedial measures.
Training may include testing the training and readiness of key
personnel
responsible for action during an emergency to determine that
they know the required procedures and actions. A statement on the
training session will be included in the annual review of the EAP
review if a training session occurred during the previous year. The
statement will include date of session, attendance list, and
summary of topics discussed. The annual review of the EAP will
include any plan revisions indicated by the outcome of the
training.
Updating
GE will annually review and update the EAP in accordance with
DCR Regulations. Updates will reflect changes in names, titles, and
phone numbers of people with responsibility to take action during a
dam emergency.
Review will identify any new developments or other changes
upstream or downstream which might necessitate revising the EAP. If
there is such a change, GE will promptly inform DCR, determine in
consultation with agencies and others whether revision is
necessary, and distribute any resulting
revisions.
All updates and revisions will be immediately furnished to DCR
(three copies) and all other EAP
holders (two copies) to keep the plan workable. Revised pages
will be marked "Revised
mo/day/yr," and revised material will be highlighted.
