First Stage

This stage shows us a boxes that we need to tick to get onto another stage and fill out contact details for Wayne. We need to select a Service (what he wants to be done), Therapist (who he want it done by) and we also need to select a date of a day in which he is available to get the therapy done.

Second Stage

Second stage shows us selected and approved information we ticked in the first stage. The diary clearly shows us that the appointment we arrange is going to be Hot Stone Massage on Monday April 12th at 1:00pm done by Manny.

Third Stage

In the third stage we need to put our contact details as a first time users and below it is necessary to fill the box with Special Instructions that will tell more about our needs such as The manager of Wayne Rooney wants the products and items to be dermatologic ally tasted before using on his body. We also put name, age, height and weight of the player.

Fourth Stage

On the end of the filling the application form and booking an appointment through a electronic diary, now we are ready to confirm, delete or change the date of the appointment. Once the contact details has been written on their website, the management system automatically saves the date and it keeps our contact details on it in case of another appointments.
