The following instructions provide information on how to run reports of all patients with an A1C >9% and with established cardiovascular disease. For additional questions, please reach out to your EHR vendor for technical support.
Click Reporting from the top menu
Select My Reports
Epic: Patients with an A1C >9%
Select Chronic Conditions: My Patients with Diabetes
Epic: Patients with an A1C >9%
Select Filters
Select A1c
Enter 9
Enter 99
Click Apply
Epic: Patients with an A1C >9%
Results will display
Epic: Patients with an A1C >9% and Established Cardiovascular Disease

Select the Library tab
Select Clinical from the Type menu
Select Find Patients – Generic Criteria from the Template menu
Select My Diabetic Patients from the Report menu
Click Edit
Select Hemoglobin A1c
Enter ≥9
Epic: Patients with an A1C >9% and Established Cardiovascular Disease
Select Equal to
Enter all desired Diagnosis Grouper
Click Run
Results will display
