• Applicant details – General information about your organisation and contact details –to be filled by your organisation

• The Journey – information about your organisation’s experience with the EFQMModel implementation and assessments – to be filled by your organisation

• Declaration – to be signed by the highest ranking official• Terms and Conditions – to be read by the people involved within your organisation

Section 3: Declaration

I agree, on behalf of my organisation, to abide by the rules of the EFQM Global Award and accept that the decisions of the EFQM are final. I accept that the EFQM has the right to reject applications that do not meet the rules of the programme. In addition:

Section 4: Key Dates

• travel and accommodation for the whole assessor team visiting you for the site-visit(one week)

• travel and accommodation for the team leader for the face-to-face feedback meetingafter the receipt of the feedback report (online meeting is an alternative)

• If you have to cancel your application at any point during the EFQM Global Awardprocess after successfully accepted your signed application by EFQM, pleaseadvise us immediately in writing.

• After sending the signed Application form, you have 15 days to cancel yourparticipation. After 15 days, the Acceptance fee of 30% will be charged, which isnon-refundable. Furthermore, in case the assessors or EFQM have alreadyincurred in any additional costs, you will need to cover these costs upon showingwritten evidence (e.g. receipts, invoices) to the assessors directly and to EFQM.

15 days after receipt of your signed Application form, EFQM will issue the invoice for the Acceptance fee, which is 30% of your total application fee. This is a non-refundable fee due to the costs and planning involved to secure resources in advance for the site-visit.

The main Application fee invoice (the 70% of the total application fee remaining) will be issued one month before the site-visit and it needs to be paid before the start of it.

Participation Fee

The participation fee is based on size, scope and complexity of your organisation. Please contact EFQM to confirm your application fee.

The participation fee covers all fixed costs to organise and run the award process but excludes all variable costs as:

Only when there is no other alternative, the site-visit will be conducted remotely.

Documentation

Each applicant needs to complete the Application Form and send it to EFQM. As a submission document, each applicant needs to produce an online “Management Report” in the EFQM AssessBase that consists of:

1. Key Information: it summarises the organisation’s operating environment, structure,stakeholders and strategic objectives.2. Good Practice description: summary of Good Practices the organisation identifies.3. Approaches section: it describes the key approaches they have adopted to achieve thestrategic objectives.4. Results Section: This gives an overview of the key results the organisation has achieved,showing how effectively they are progressing towards their strategic goals.

Eligibility to apply/Qualification

Eligibility rules apply for participation as EFQM tries to drive for a diverse and high level set of role model applicants. These rules strike a balance between creating as diverse set of role models as possible and encouraging the best to be successful.

Once you send this application form signed, EFQM will review it and contact you to define the next steps. All applicants must pass the EFQM Global Award Qualification phase in order to be officially accepted to participate in the process. The Qualification process will depend on the maturity your organisation can demonstrate.

Site-visit

In regards to the COVID crisis and its consequent travel restrictions, please note that for the EGA 2022: • Whenever it is possible, taking into consideration the travel restrictions and the well-

being of all the parties involved, the site-visits will take place physically, with thewhole assessment team on-site

• When the above is not possible for the regulations in place at the time of the site-visit,a blended approach will be adopted: local assessors and assessors coming fromcountries with no restrictions will be on-site and foreign assessors who are notallowed to travel will connect remotely.
