エクソソームは生体内のさまざまな細胞および癌細胞から分泌される 40〜100 nm の膜小胞で,血液,尿,唾液,腹水などの体液に存在する9–12)。エクソソームが酸性環境でも安定していることに着目し,胃液からエクソソームを抽出し,エクソソームに含有される microRNA (miRNA) や DNA などの機能分子の胃癌に伴う変化を明らかにできれば,胃液採取による胃癌早期診断や胃発癌リスク診断マーカーの開発につながる可能性がある。
一方,世界の総人口の約半数は H. pyloriに感染しているといわれ,日本人でも年齢が上がるにつれて感染率が高い13)。中でも,病原タンパク質 CagA を持つ H. pylori (CagA 陽性 H. pylori) に感染すると,胃癌を始めとする胃粘膜病変を発症することが分かっている14)。IV 型分泌装置と呼ばれる H. pylori 表面のとげによって,CagA が胃の細胞へ注入されると,細胞内のシグナルが撹乱され,癌化が促進される。日本人が感染している H. pylori のほぼ 100%はCagA 陽性である。興味深いことに,H. pylori 感染胃癌患者の血液中に存在するエクソソームに H. py‐lori 由来 CagA が含まれることが報告されている15)。
本研究では,超遠心法を用いて胃癌患者の胃液からエクソソームを抽出し,その機能分子 (miRNA および DNA メチル化) の異常を明らかにすることを目的とした。
胃癌における miR-34 発現の低下の一因として,DNA メチル化が報告されている16)。エクソソーム機能分子として,DNA も注目されている17– 20)。従って,次に exoDNA をバイサルファイト処理し,パイロシーケンスによる DNA メチル化解析を行った。
miR-34b/c の DNA メチル化は,胃癌患者において,コントロールに比べ有意に高値であった。次に,核DNA のメチル化情報が exoDNA に反映されているか,両者の DNA メチル化解析結果を比較検討した。胃液から抽出した exoDNA を用いたメチル化解析結果と同一症例の胃癌組織パラフィン切片から抽出した DNA を用いたメチル化解析結果が相関していることを明らかにした。従って,メチル化 DNA が,エクソソーム内にパッケージングされていると考えられた。エクソソーム内にパッケージングされたメチル化 DNA の機能的意義は今後の検討課題であるが21),胃液由来 exoDNA を用いたメチル化解析は,胃癌の有望なマーカーとなりうると考えられた。マーカーとして使用する場合の利点として,コスト面があげられる。胃液の採取だけであれば,内視鏡検査を必要としないこと,超遠心法では,高額な市販のキットを使用せずにエクソソームの抽出が可能である。マーカーとしての有用性に関して,本研究での解析症例数は少なく,多数の症例を対象に,感度,特異度などを解析する必要がある。H. pylori 感染胃癌患者の血液中に存在するエクソ

Abstract
Molecular Analysis of Gastric Juice-Derived Exosomes in
Gastric cancer (GC) is one of the most common malignancies and is the leading cause of cancer-relateddeaths worldwide. CagA, encoded by cytotoxin-associated gene A (CagA), is a major virulence factor of Helico‐bacter pylori, a gastric pathogen involved in the development of GC. We previously developed a method forDNA methylation analysis, and early detection of GC using gastric washes rather than highly acidic gastricjuice. Extracellular vesicles known as exosomes present in the gastric juice may provide an alternative to gastricwashes for the molecular detection of GC. We determined tumor-related alterations in the functional molecularcontents within the exosomes, which were purified from the gastric juice of patients with GC, and whether theexosomes contained CagA. Using ultracentrifugation, we purified exosomes from the gastric juice of patientswith GC and analyzed their expression levels of microRNA-34 (miR-34) by real-time PCR. Methylation levels ofthe miR-34b/c gene in exosomal DNA were analyzed by bisulfite pyrosequencing. The microvesicles were veri‐fied as exosomes by transmission electron microscopy and western blot analysis with the CD9 exosomal marker.Expression levels of miR-34 were lower in tumor-derived exosomes than in non-tumor-derived exosomes. Meth‐ylation levels of miR-34b/c were higher in tumor-derived exosomes than in non-tumor-derived exosomes. Con‐cordant miR-34b/c methylation levels were observed between the exosomal and tissue nuclear DNA from pa‐tients with GC. CagA was detected in exosomes obtained from the gastric juice of CagA-positive H. pylori-infected patients with GC. These findings suggest the use of gastric juice-derived exosomes as a biomarker forGC in clinical settings in the future.
