社会医学研究.第 31 巻 1 号.Bulletin of Social Medicine, Vol.31 (1)2014 ― 53 ― 抄 録 近年、地域健康づくりの中では、平均寿命のみならず健康寿命の延伸が議論されるようになってきている。一般 に寿命は、生物学的な健康度のみならず社会経済的指標、格差等により規定されることが報告されているが、平均 寿命、健康寿命それぞれの指標がどのような背景により支えられているものか、詳細な研究は少ない。本研究では、 ヨーロッパの諸国を例にとり、欧州連合(European Union, EU)が行っている健康寿命延伸の施策をもとに、公 表されている寿命、社会経済指標など各種の統計資料を総合的に統計解析し、欧州連合各国の持つ主観的健康感、 社会経済状態が平均寿命、健康寿命にいかなる関与をもたらすかについて重回帰分析を用いて検討した。この研究 から得られるものは、方法論的にはマクロレベルの生態学的研究による結果であるが、平均寿命では人間開発指数 (HDI)、自殺死亡率と、健康寿命では主観的健康感と有意な関連が認められた。平均寿命と健康寿命それぞれの背 景について興味深い結果が得られた。 Abstract In regional health promotion, extending the healthy life as well as average life expectancy has been discussed in the 21st century. In the present study, multiple regression analysis was used to clarify the relationships between the lifespan and socio-economic conditions, such as human development index (HDI), GDP, GiNi index, suicide rate, and self-rated health in the 27 European countries (EU). 平均寿命、健康寿命を規定する要因について- EU27 カ国に関する研究
Factors Associated with Life and Health Expectancies in the 27 EU Countries
(2009,人口 10 万人対)、GiNi 係数(2010)、GDP (2007)に加えて人間開発指数(HDI,2010)の各項目であった。平均寿命は各国の国勢調査、健康寿命は、健康上の制約については European Statistics on Income and Living Conditions Survey (EU-SILC)のデータをもとに、Sullivan 法を用いて算出され、公表されている結果を用いた 22)。主観的健康感は、各国の 65 歳時の調査の結果、健康度(perceived health)が良い、普通、悪い、の三段階として公表された資料から、良いと回答した割合を入力した。
