Conclusion: Not only will this result be helpful to complete nurse's lack of knowledge and experience, it will also help to determine nursing diagnosis logically by using standardized nursing process. It will be utilized as the method to construct ontology including interoperability in other nursing units. It will be presented nursing interventions according to nursing diagnosis and thus will be easier to establish nursing planning. This can provide immediate feedback of the nursing process application. Construction of the Nursing Diagnosis Ontology in Obstetric and Gynecologic Nursing Unit using Nursing Process and SNOMED CT

서 론
1. 연구의 필요성
간호사는 병원, 요양원, 지역사회건강센터 등 다양한 건강
영역의 실무를 담당하며, 전문화된 지식을 기반으로 체계적이
고 일관된 방법으로 실무를 수행한다. 다양한 간호사 업무는
공통적인 핵심 가이드라인으로 간호과정을 사용하고 있으며,
간호과정은 사정, 진단, 계획, 중재, 평가로 이루어져 있다
(von Krogh, Dale, & Naden, 2005). 예를 들어, 골반 부위
통증과 월경과다를 호소하는 32세의 미혼여성 환자에게서 간
호사는 간호문제의 예로 ‘불안’이나 ‘통증’의 증상을 발견할
수 있을 것이다. 간호사는 ‘불안’, ‘급성 통증’의 진단을 내릴
수 있으며, ‘불안 수준’, ‘통증 수준’을 평가한 후, 우선순위에
따른 계획을 세워 중재를 수행하며, 환자의 불안이나 통증의
정도가 완화되거나 변화가 없거나 혹은 더 악화된 상태인지를
평가하게 된다. 문제가 해결되었다면 그 과정을 어떻게 끝내
는지, 또 문제가 해결되지 못하였다면 이를 해결하기 위해 간
호과정을 어떻게 다시 적용하는지에 대한 순환적인 문제 해결
방법이 간호과정이다.
현재 의료 환경은 병원 내에 전자의무기록(Electronic Me-
dical Record, EMR) 시스템이 급속히 도입되면서 진료뿐 아
니라 간호 분야에도 정보기술을 통합 관리하는 방법이 필요하
게 되었다. 보건의료분야에서 확산되고 있는 전산화된 정보
체계의 사용과 표준화에 대한 요구에 부응하기 위해 미국은
1996년 Nursing Information and Data Set Evaluation
Center (NIDSEC)를 설립하였다. NIDSEC (1997)는 간호기
록을 위해 사용하는 전산화된 정보체계에 대한 표준을 개발하
여, 13개의 표준화된 간호용어를 승인하였으며, EMR의 간호정
보시스템에 표준화된 간호용어의 사용을 적극 강조하고 있으
며, 간호학에서 표준화된 분류체계를 이용한 연구를 촉구하고
있다. 이 중 대표적으로 활용되는 간호용어는 간호과정을 표현
하는 표준화된 분류체계로서 국제적으로 승인된 북미간호진단
협의회(North American Nursing Diagnosis Association,
NANDA)의 간호진단분류체계(이하 간호진단)와 Iowa 대학
을 중심으로 개발된 간호중재분류체계(Nursing Interven-
tions Classification, NIC), 간호결과분류체계(Nursing Out-
comes Classification, NOC)가 적극적으로 활용되고 있다.
이 3개의 분류체계를 합쳐 NNN으로 칭하며(Johnson et al.,
2012), NNN의 적극적인 사용은 의사소통의 증진, 중재의 가
시화, 정보수집의 향상, 표준화된 간호의 준수, 간호의 역량평
가를 촉진하고 있다(Scroggins, 2008). 실무의 근간인 간호과
정의 중요성이 강조되지만, 간호사들은 시간이나 지식의 부
족, 분류체계의 포괄성, 의사결정의 어려움 등의 다양한 이유
로 실제 간호과정을 적용하는데 어려움을 겪고 있는 실정이다
(Lee & Park, 2008).
NNN 기반의 간호과정을 EMR에 포함하기 위한 선행연구
들에서 제시한 간호진단의 수를 보면, 내외과 간호단위를 대
상으로 한 Kim (2000)의 연구에서는 98개, 내과, 정형외과,
신경외과 및 일반외과 간호단위를 대상으로 한 Park (2003)
의 연구에서는 155개, 정형외과 간호단위를 대상으로 한 Kim
(2005)의 연구에서는 35개, 심장혈관계 중환자실 간호단위를
대상으로 한 Kim (2006)의 연구에서는 10개, 내과 중환자실
간호단위를 대상으로 한 Park (2010)의 연구에서는 23개의
간호진단이 보고되고 있으나, 대부분 특정 간호단위에 한정되
어 있어서 산부인과 간호단위에 적용하는데 한계가 있으며,
타 영역과는 분리되고 단절된 상황이다.
NANDA의 간호진단은 간호의 주요 현상을 나타내지만 추
상적인 수준에서 표현하는데 그친다. 간호현상을 구체적으로
표현하기 위해, 간호진단의 의미를 더 풍성하고 구체적으로 만
드는 관련요인을 텍스트로만 제시하고 있다. 그러므로 EMR에
표준화된 간호용어인 간호진단과의 의미적인 연결을 통한 정
보와 지식을 생산하는 의미 있는 데이터로서 저장되지 못하고
있다. 즉, 간호진단 00126 ‘deficient knowledge (지식 부
족)’은 지식부족이라는 현상의 추상적인 수준의 요약일 뿐, 지
식부족을 구체적으로 표현하지 못한다(예: 질병과정에 대한
이해부족과 관련된 지식부족). 또한, 텍스트 기반의 관련요인
은 상황에 따라 다르게 표현이 가능하여, 전산화 될 경우 대부
분 누락되거나 생략되어 정보로서의 의미를 잃게 된다. 관련
요인의 누락은 NNN 분류체계의 불완전함과 EMR에 들어갈
중요한 데이터의 손실을 의미한다. 이는 지식의 단절을 의미
하며, 의사결정의 근거인 지식이 쌓이지 않는다면 실제 간호
과정 적용의 의미를 놓칠 수밖에 없다. 결국 간호사는 제한된
용어들에서 임상경험이나 직관, 관습에 따라 진단, 결과, 중재
를 선택해야 하며, 이러한 불확실한 의사결정과정에서의 추상
적이고 부정확한 간호진단의 선택은 불확실한 간호과정을 적
용하는 출발점이 된다.
따라서 데이터의 누락 방지를 위해 텍스트 기반의 관련요인
들을 실제 간호과정 내로 들어갈 수 있도록 해야 하며, 간호진
단의 구체적인 표현을 위해 간호진단과 관련요인을 연결하는
지식체계를 구축할 필요가 있다. 의학적 진단은 이미 다양한
증상/징후가 어떠한 질환으로 발현되는지에 대한 지식체계

구축을 위해 2003년부터 주 증상(chief complaint) 표준화에
대한 연구와 표준화된 임상 용어 세트의 개발을 위해 System-
atized Nomenclature of Medicine Clinical Terms (SNOMED
CT)를 EMR에 적극적으로 적용하고 있다(Chin & Kim, 2003;
Kim, Han, & Choi, 2005).
간호진단과 관련요인을 연결하는 지식체계를 구축하기 위
한 기술로는 온톨로지가 있다. 온톨로지는 특정 분야의 지식
체계를 모델링할 수 있는 기술로서, 온톨로지를 간호용어시스
템에 도입하기 위한 선행연구들을 보면, 간호중재 영역의 온
톨로지(Hardiker, 2001, 2003), 통증간호 영역의 온톨로지
(Kim, Lee, Park, Choi, & Kim, 2005), 성인 중환자실 간호
영역의 온톨로지(Kim, Harris, Savova, & Chute, 2008), 간
호계획 영역의 온톨로지(Din, Abidi, & Jafarpour, 2010)가
보고되고 있으나, 산부인과 간호단위에서 사용되는 간호진단
중심의 온톨로지의 개발과 평가는 국내에서는 보고된 바가 없
으며 특히, EMR과 연계하기 위한 표준화된 간호용어 간의 통
합적인 접근 방법이 아니므로 간호정보의 일관성을 보장하기
는 힘들다. SNOMED CT는 ANA에서 승인한 13개의 표준화
된 간호용어 중 가장 최근에 승인된 표준화된 간호용어이자
유일한 참조용어이다. 1965년 미국병리학회에서 병리학적 초
점에서 개발을 시작하였으나, 현재는 총 19개의 다축구조로
서 보건학, 간호학, 수의학, 생물학, 화학 등을 포괄하는 EMR
의 참조용어로 활용되고 있다.
따라서 본 연구는 산부인과 간호단위의 EMR에 포함된 간
호기록 자료에 포함된 간호진단진술문을 추출하여, 간호진단
진술문의 진단은 NANDA의 간호진단과 연계하고 관련요인
들은 표준화된 간호용어인 SNOMED CT와 연계하여, 간호진
단과 관련요인을 연결한 간호진단 온톨로지를 구축함으로써,
EMR 기반에서 산부인과 간호단위의 간호과정을 확장 및 개
선시켜 지식을 생산할 수 있는 온톨로지 방법론을 제시하고자
한다.
2. 연구목적
본 연구의 목적은 산부인과 간호단위의 표준화된 간호과정
을 기반으로 한 간호진단 온톨로지를 구축함으로써, EMR 기
반에서 산부인과 간호단위의 간호과정을 확장 및 개선시킬 수
있는 방법론을 제시하기 위함이다. 구체적인 목적은 다음과
같다.
산부인과 간호단위의 EMR의 간호기록에서 간호진단진
술문을 추출한다.
간호진단진술문의 진단과 NANDA의 간호진단을 연계
하여 간호진단을 표준화한다.
간호진단진술문의 관련요인과 SNOMED CT를 연계하
여 관련요인을 표준화한다.
간호진단과 관련요인을 연결하는 간호진단 온톨로지를
구축한다.
3. 용어정의
1) 간호기록
간호기록은 환자 상태와 경과에 대한 간호사의 판단, 환자
의 반응과 상태, 환자에게 행해지는 간호의 결과를 기록한 자
료이다(Chi, Choi, Park, & Jung, 1999). 본 연구에서는 D시
에 위치한 일개 대학병원의 산부인과 간호단위에서 입원 치료
후 퇴원한 환자 244명의 International Classification for
Nursing Practice (ICNP)와 NNN을 기반으로 한 EMR의 간
호기록 자료를 의미한다.
2) 간호진단진술문
본 연구에서는 D시에 위치한 일개 대학병원의 산부인과 간
호단위에서 입원 치료 후 퇴원한 환자 244명의 EMR의 간호기
록 자료에서 추출한 141개의 간호진단진술문을 의미한다.
3) 간호진단
본 연구에서는 NANDA 2009-2011에서 제시한 간호진단
중 간호진단진술문의 진단과 연계한 27개의 간호진단을 의미
한다.
4) 관련요인
본 연구에서는 간호진단진술문에서 간호진단과 연계한 관
련요인 중 중복을 제거한 총 113개의 관련요인을 의미한다.
5) 간호진단 온톨로지
온톨로지(ontology)란, 특정 분야의 지식체계를 모델링할
때 추출한 용어들의 뜻을 분명하게 정의하며 컴퓨터가 이해
하고 처리할 수 있는 형태로 표현하여 나타낸 용어들의 논리
적 집합이다(Hardiker, 2003). 즉, 특정 분야에 관련된 용어
들을 정의하고, 용어 간의 관계들로 구성된 일종의 사전이다.
의학, 생물학 분야의 온톨로지는 Unified Medical Language
System (UMLS), Medical Subject Headings (MeSH),
SNOMED CT 등이 대표적이다. 본 연구에서는 SNOMED

CT로 표현된 113개의 관련요인과 27개의 간호진단으로 구성
된 지식체계를 간호진단 온톨로지(nursing diagnosis ontol-
ogy)로 정의한다.
연구방법
1. 연구설계
본 연구는 문헌고찰을 토대로 연계한 간호과정의 간호진단
-간호결과-간호중재를 기본적으로 구축하고, 온톨로지 구축
에 앞서, 온톨로지에 들어갈 용어의 표준화 작업을 위해, EMR
의 간호기록 자료에서 간호진단진술문을 추출하여 진단은
NANDA의 간호진단과 연계하고, 관련요인은 SNOMED CT
와 연계하여 표준화하였다. 간호진단과 관련요인간의 연결 및
확장을 위해 온톨로지를 도입하여, 간호진단 온톨로지를 구축
하였다(Figure 1).
2. 연구대상 및 자료수집
본 연구의 대상은 2010년 8월부터 2010년 10월까지 D시
에 위치한 일개 대학병원의 산부인과 간호단위에서 입원 치료
후 퇴원한 환자 244명의 EMR의 간호기록 자료이며, 산부인
과 환자가 입원 시 EMR 활용(교육, 연구)에 서면으로 동의한
자료를 대상으로 하였다. 자료수집은 EMR의 간호기록 자료
를 엑셀파일로 송부 받은 후, 사정, 진단, 계획, 중재, 평가로
분리하여 진단에 해당하는 간호진단진술문만을 분석하였다.
3. 연구도구
1) 간호진단-간호결과-간호중재 연계
간호진단은 실제적이고 잠재적인 건강문제와 삶의 과정에
대한 개인, 가족 혹은 지역사회의 반응을 간호사가 임상적으
로 판단한 것이다. NANDA 간호진단분류체계에서 제시한 간
호진단은 202개이며, 이 중 사고과정장애(00130)는 2009년부
터 사용하지 않는 진단으로 실제로는 201개의 간호진단이 사용
된다(NANDA International., 2009). 간호결과는 간호중재에
대한 반응으로 측정되는 환자, 가족, 지역사회의 상태, 행위 및
인지정도이다. 간호결과분류체계(NOC)에서 제시한 간호결
과는 385개이다(Moorhead, Johnson, Mass, & Swanson,
2008). 간호중재는 대상자의 결과를 향상시키기 위해 임상적
판단과 지식을 기반으로 간호사가 수행하는 처치이다. 간호중재
분류체계(NIC)에서 제시한 간호중재는 542개이다(Bluechek,
Butcher, & Dochterman, 2008). 간호진단-간호결과-간호
중재 연계는 NANDA의 간호진단과 간호결과분류체계(NOC),
간호중재분류체계(NIC)간의 연계를 Iowa 대학의 NOC과
NIC 연구팀이 제안한 것이다(Johnson et al., 2012). 본 연구
에서는 NANDA의 간호진단 27개와 간호결과 152개, 간호중
재 861개를 이용하여 산부인과 간호단위의 간호과정 용어를
구축하였다.

essential core of nursing practice. Nonetheless, nurses' do not use nursing process for various reasons.
