Bulletin 2020-21 Edward Mallinckrodt Department of Pediatrics (01/15/21) Edward Mallinckrodt Department of Pediatrics The primary aim of the teaching program of the Department of Pediatrics (http://pediatrics.wustl.edu/) is to stimulate interest in developmental biology — in particular, human growth and development — to provide students with a foundation sufficiently comprehensive to have an appreciation of clinical pediatric problems, regardless of their future career choices in medicine. The major clinical and research facilities are in St. Louis Children's Hospital (http://www.stlouischildrens.org/), and the newborn services are at the Women and Infants Center (http://www.stlouischildrens.org/women-and-infants/). St. Louis Children's Hospital is a facility with 300 beds that accepts patients through 21 years of age with all types of medical and surgical problems. Hospital admissions average 11,200 annually. Pediatric medical ambulatory activity, including subspecialty and emergency visits, averages about 152,000 visits a year. Nearly 4,000 infants are born annually at the Washington University Medical Center. Using candidate gene sequencing, exome sequencing, wholegenome sequencing, and computational prediction and filteringstrategies for the discovery of deleterious variants in population-based cohorts, case-control cohorts, and trios of affectedinfant and parents, our laboratory focuses on discovering novelcandidate genes associated with neonatal respiratory distresssyndrome and understanding the contribution of genetic variationin candidate genes of the pulmonary surfactant metabolicpathway (including surfactant protein B, surfactant protein C,NKX2-1, and ABCA3) to the risk of neonatal respiratory distresssyndrome.
The focus of this lab is on clinical and translational researchin childhood kidney disease. Our group is involved in severaldifferent types of clinical and translational research, includingmulticenter clinical intervention trials to improve teen adherencewith transplant medications and to test new medicationsin children on dialysis; translational biomarker studies intransplant acute and chronic rejection and genomic studiesor post-transplant lymphoproliferative disease; and largetransplant database epidemiological analyses for associationsof immunosuppressive regimens with efficacy and morbiditybalance.

Our research team studies the epidemiology, microbial virulencemechanisms, and host defenses against community-associatedmethicillin-resistant Staphylococcus aureus (CA-MRSA)colonization, transmission and disease. We are investigatingthe transmission dynamics of CA-MRSA in households as wellas interventions to interrupt the transmission of CA-MRSA andto prevent subsequent infections. Our lab also explores themicrobial and host genomic determinants as well as the hostimmune response to staphylococcal toxins implicated in thepathogenesis of CA-MRSA in patients across the spectrum ofdisease states. Our goal is to develop novel approaches for theprevention of CA-MRSA infections.

Our focus is on the extracellular matrix in vascular developmentand disease. Specifically, we study the extracellular matrixproteins that make up the elastic fibers of blood vessels.Elastic fibers convey elasticity to blood vessels, allowing largearteries to store energy during systole and release it duringdiastole. Abnormalities in elastic fiber components lead tovarious complications, including hypertension, stiff vessels,and aneurysms. In the laboratory, we utilize mouse models tounderstand how abnormalities in these proteins lead to disease,which helps us not only to learn about the normal function ofthese proteins but also to identify potential novel therapeutictargets.

Work in our lab focuses on the interactions of pathogenicbacteria with their hosts. We aim to elucidate the modulationof host immune responses by pathogens and to determine themechanisms by which these bacteria present specific virulencefactors on their surfaces. Currently, we use cultured bladderepithelial cell models and murine models of cystitis to investigatethe ability of uropathogenic Escherichia coli to modulate hostinnate and adaptive immune responses. In addition, we arestudying the molecular mechanisms by which selected outermembrane proteins contribute to the virulence of uropathogenicE. coli. Our primary goal is to discover novel targets forinterventions that will prevent and better treat bacterial infectionsof the urinary tract. Along these lines, we are leveraging recentdiscoveries in UTI pathogenesis to design nanoparticle-basedtherapies for the prevention of acute and recurrent UTI. We havealso launched a new translational study of immune responsesto UTI in male and female infants, paired with an innovative newmouse model of male UTI that permits first-ever studies of sexdifferences in these infections.

Our focus is the study of infectious disease genomics. Ourlaboratory is interested in applying genomic analysis to avariety of problems in infectious diseases, mostly related to viralinfections. M65 Peds 808 Pediatric Asthma and AllergyIn a predominantly outpatient setting, students will evaluatepatients with a wide variety of allergic and immunologicdisorders including asthma, allergic rhinitis, anaphylaxis, foodallergy, atopic dermatitis, urticaria, angioedema and primaryimmunodeficiency. Rotation goals include: (1) the extension ofhistory-taking skills to include environmental exposures, (2) therecognition of physical findings suggestive of allergic disease,(3) understanding the indications and interpretation of diagnostictesting including skin testing and assessment of pulmonaryfunction, and (4) application of appropriate therapeutic strategiesto these disorders. Weekly didactic conferences and inpatientconsultations provide additional educational opportunities to thestudent.
M65 Peds 809 Pediatric Pulmonary SubinternshipOn the 7100 Respiratory Unit, the subintern is an active memberof a multidisciplinary care team, which consists of attendingpulmonologist, advanced practice nurses, second-year pediatricresidents, unit nurses, and other care providers. The subinterntakes responsibility for children with acute and chronic lungdiseases admitted to the unit. The student will be co-managedand directly supervised by senior pediatric residents andattending physician in the daily care of patients. The rotation isstructured to provide students with a clinical experience to allowthem to gain exposure to the breadth of repiratory diseases seenat St. Louis Children's Hospital. The volume of patients on the7100 Respiratory Unit and number covered by the subinternvaries. He or she will typically be responsible for the care of twoto six patients at any given time, and will be exposed to childrenwith wide-ranging lung diseases and breathing disorders,such as asthma, cystic fibrosis, bronchopulmonary dysplasia,bronchiolitis, pneumonia, chronic respiratory insufficiency, andcongenital lung anomalies during this rotation. The student willalso have the opportunity to participate in tests and proceduresessential to the practice of pulmonary medicine, includingpulmonary function studies, flexible bronchoscopy, and overnightpolysomnography. Subinterns do not have evening coverageresponsibilities, and weekend responsibilities are limited to twodays during the four-week block. They are strongly encouragedto attend departmental and divisional conferences.
M65 Peds 811 Pediatric Critical Care SubinternshipThis elective is designed to familiarize the student with thediagnosis and treatment of critical illness in infants and children.To this end, each student is made responsible for a smallnumber of assigned cases under the direct supervision ofpediatric residents, pediatric critical care fellows, and faculty.The teaching activities emphasize the understanding ofpathophysiological processes that lead to respiratory, circulatory,and central nervous system dysfunction and their therapy inthe developing subject. Students are expected to participate inall the daily activities of the Pediatric Intensive Care Unit at St.Louis Children's Hospital and be on occasional call after hours.
M65 Peds 813 Pediatric Cardiac CatheterizationThis elective will focus on the interpretation of hemodynamicand angiographic data acquired in the cardiac catheterizationlaboratory.
M65 Peds 819 Pediatric Cardiology - Outpatient ServiceStudents will be exposed to the wide spectrum of pediatriccardiology on an outpatient basis. In addition to generalcardiology clinics, several subspecialty clinics are also available,including heart failure/transplant, electrophysiology/inheritedarrhythmias, pulmonary hypertension, William's syndrome,Down syndrome, cardiac neurodevelopmental, and preventativecardiology clinic. Students will independently evaluate clinicpatients referred for a variety of cardiac complaints, suchas cardiac murmurs, chest pain, syncope, arrhythmia, aswell a wide variety of congenital cardiac lesions, and reporttheir findings to the attending. Cardiac auscultation skillswill be enhanced through auscultation of cardiac patientsin a clinic environment. Students will learn basics of ECGand echocardiogram interpretation by reviewing studiesperformed during clinic with the attending. Clinics are heldat St. Louis Children's Hospital and the Children's SpecialtyCare Center in West County. Students also have the optionto participate in outreach clinics that occur on a monthly basis(locations include Cape Girardeau, Poplar Bluff, Rolla, BonneTerre, and Columbia). Depending on interest, students mayspend additional time in the echocardiography laboratory formore in-depth exposure to echocardiography, including fetalechocardiography. Participation in weekly surgical conferenceand daily cardiology educational conferences is encouraged.
M65 Peds 826 Genetics and Genomic MedicineThe goal of this senior elective is to facilitate the acquisition ofclinical skills and knowledge in genetics and genomic medicine.The student will actively participate in the diagnosis andmanagement of pediatric and adult patients with genetic diseasein both the ambulatory and inpatient settings. Emphasis will beplaced on application of the science of genetics to the bedsideand will include a broad exposure to patients with biochemical,metabolic, structural and complex genetic diseases. Studentswill have an opportunity to visit clinical laboratories involvedwith diagnosis of genetic disorders, including the cytogenetics,molecular genetics, and biochemical genetics laboratories.Students will be expected to participate in the weekly clinicalcase conference.
M65 Peds 827 Pediatric Hematology/OncologySubinternshipStudents will assume the responsibilities of a pediatric residenton the inpatient Hematology/Oncology service at St. LouisChildren's Hospital. Serving in a classic subintern role, thestudent will perform evaluations and manage, under thesupervision of a senior resident, patients who span the scopeof diseases in our discipline. The student will also learnthe approach to patients with positive autoantibodies, jointpain, muscle pain, and other common complaints that ageneral pediatrician may encounter. M65 Peds 838 Pediatric Gastroenterology, Hepatology, andNutritionThe rotation in Pediatric Gastroenterology, Hepatology, andNutrition provides broad exposure to specialized and commonpediatric gastrointestinal and hepatobiliary problems. Divisionpatients are seen in the outpatient suites and in the hospital.Students evaluate outpatients with common pediatric complaintslike abdominal pain, constipation, and poor growth. Additionally,students experience the ongoing outpatient care of patients withliver disease, inflammatory bowel disease, short-gut syndrome,celiac disease, and other rare disorders. The inpatient serviceprovides experience in caring for patients with acute illnessessuch as gastrointestinal bleeding, malnutrition, liver failure,complications of inflammatory bowel disease, and pancreatitisas well as seeing patients on the pediatric gastrointestinalconsultation service. Students participate in diagnostic andtherapeutic endoscopic procedures. At weekly divisionalconferences, faculty, fellows, and students review pathologyslides from current cases and discuss difficult patient problemsand topics of interest.
M65 Peds 839 Antimicrobial Use, Resistance, andStewardshipIn 2013, the CDC estimated that 23,000 Americans die annuallyfrom antibiotic-resistant infections, and an additional two millionare infected with one of these difficult-to-treat pathogens.The primary driver of this resistance is the use -- and, moreimportantly, the misuse -- of antibiotics. In 2015, the WhiteHouse published the National Action Plan for CombatingAntibiotic-Resistant Bacteria. This plan calls for improvementin antimicrobial use in human and agricultural medicine,better diagnostics, increased collaboration domesticallyand internationally, and the accelerated development ofnew antibiotic agents. This fourth-year elective rotationwill be focused on educating the student on the currentstate of domestic and global antibiotic resistance and themechanisms by which health care systems are addressing thisproblem. The student will participate in the daily antimicrobialstewardship activities conducted at St. Louis Children's Hospital,attend weekly stewardship and clinical infectious diseasesmeetings both at the hospital and BJC system levels, reviewantimicrobial use data, and participate in hands-on activitiesin the microbiology laboratory. At the end of this rotation, thestudent will be able to do the following: (1) List the antimicrobialsand the pathogens they effectively treat; (2) Analyze bacteriafor genotypic and phenotypic resistance through standard andrapid microbiologic techniques; (3) Describe the antimicrobialstewardship interventions that can be implemented in differenthealth care settings; (4) List the social determinants that impactantimicrobial stewardship programs; and (5) Explain how themicrobiome and resistome are important in our efforts to improveantimicrobial use.
M65 Peds 840 Pediatric Infectious DiseasesThis elective is designed to introduce students to the clinicalaspects of routine and complex infectious diseases in childrenages birth to 18 years. Students will perform ID specialtyconsultations on both inpatients and outpatients. Regulardaily activities will include evaluation of new patients, workrounds on inpatient consults, and teaching rounds with the IDfellow and attending. Students will attend the general pediatricID clinic and the pediatric HIV clinic, one half-day each perweek. Formal teaching includes HIV and ID Core Curriculumsessions, a weekly pediatric ID case conference, a weekly jointclinical conference with the adult ID group, a weekly pediatric IDresearch conference, and weekly clinical microbiology teachingrounds in the bacteriology and molecular diagnostics labs.
M65 Peds 845 Pediatric Emergency Medicine SubinternshipThe goal of this elective is to provide the senior medicalstudent with a broad introductory clinical experience in pediatricemergency medicine. Functioning as a subintern in theEmergency Unit of St. Louis Children's Hospital, the studentwill have the opportunity to evaluate and manage patients witha wide variety of emergent and urgent medical and surgicalproblems. Examples include: respiratory distress, abdominalpain, lacerations, bone injuries, rashes, fever, etc. Students willwork either a day shift (7:30 am - 3:00 pm) or an evening shift(3:00 pm - 11:00 pm) in rotation. Daily teaching conferences areprovided by the attending staff. A weekly meeting of the studentsand senior faculty will occur to review interesting cases. Also,attending staff and senior pediatric residents provide 24-hour on-site supervision. Each medical student will be asked to prepare a20-minute presentation on a topic of his/her choosing.
M65 Peds 846 Child Abuse PediatricsThe medical student will work with the Child Protection (CPP)team at St. Louis Children's Hospital, which consists of twoChild Abuse Pediatrics attending physicians, one Child AbusePediatrics fellow, a Pediatric nurse practitioner, a clinic nurse,and 3 social workers. Pediatric residents also may be rotatingwith the team at the same time. The student will observeinpatient consults for physical and sexual abuse and outpatientclinic patients for physical and sexual abuse. The student mayattend court cases and watch expert witness testimony bythe CPP physicians/nurse practitioner. The student may seeacute sexual assault cases conducted by the Sexual Assaultnurse practitioners. The student will be asked to complete ashort project on a topic related to child maltreatment and willpresent it to the team at the end of the rotation. The studentwill be given a list of readings/didactic activities to do during therotation. The student will have daily (45 minute) lectures withone of the child abuse attending physicians on a variety of topicsrelated to child maltreatment and will attend the Child Protectionteam meeting (1 hour) every day. The student can also attendPediatric Residency noon conference during this rotation.
M65 Peds 849 Pediatric Endocrinology and DiabetesThis elective is designed to include broad clinical experiencein pediatric endocrinology and diabetes. The student will havean opportunity to evaluate both patients admitted to St. LouisChildren's Hospital and patients referred for consultation in ourthree outpatient clinics each week. In addition to a divisionalconference to review referred patients, several joint conferenceswith the adult Endocrinology and Diabetes Division (clinicalrounds, journal club/research seminar, case conference) areheld weekly.

Pediatric referralswill be seen in both an inpatient and outpatient setting.Rotation goals for students include: 1. gain greater insightsinto the genetics, epidemiology, pathophysiology, and clinicalpresentations of lung diseases in children, 2. learn theimportance of the physical examination using inspection,percussion, and auscultation, 3. understand indications andinterpretation of diagnostic tests, such as chest imaging, bloodgas measurements, polysomnography, pulmonary functiontesting, and bronchoscopy with biopsy and lavage, and 4. learnto apply therapeutic interventions to common lung diseases.Unique aspects of this rotation include, a broad exposure tochildren with asthma, cystic fibrosis, ciliopathies, interstitiallung diseases, chronic lung disease of infancy, congenitallung malformations, and end-stage cardiopulmonary diseasesreferred for lung transplantation. Weekly didactic sessions, aswell as divisional clinical conferences, provide opportunities forthe trainee to develop his or her presentation skills.
M65 Peds 861 Newborn MedicineThe goal of this course is to allow students the opportunityto assume primary responsibility for patients in the NeonatalIntensive Care Unit (NICU) under the direct supervision of firstor second year residents as well as fellows and attendings.Students will participate in formulation of diagnostic andtreatment plans, coordination of care and communication withfamilies. Throughout the rotation the students will broadentheir understanding of pathophysiology as it relates to thetransition from fetal to neonatal life and in common diseasestates affecting neonates. There will be emphasis on improvingclinical problem-solving skills, communication within the teamas well as with ancillary staff and families. Students duringeach rotation will have the option to rotate through the NeonatalIntensive Care Unit at St. Louis Children's Hospital and/or theNeonatal Assessment Center /Labor and Delivery services atParkview Tower -Barnes-Jewish Hospital. M65 Peds 870 Pediatric ImmunologyStudents in the two-week or four-week pediatric immunologyelective will learn to apply immunology knowledge to clinicalpractice by participation in caring for immunology patients invarious settings, including pediatric immunodeficiency, allergy,and rheumatology clinics. They will also participate in inpatientrounds with faculty and fellows in the immunology service.
M65 Peds 875 Pediatric Renal DiseaseThis course is designed to provide the student with a wideexposure to all aspects of pediatric renal disease and anopportunity to explore a desired aspect of the field in-depth.The student will be an integral part of the Renal Team andas such will see both inpatients and outpatients. Studentswill have an opportunity to follow the courses of patientswith acute renal disease as well as those with more chronicproblems and will help to plan the evaluation and therapeuticmanagement of these patients. Discussions and rounds with theattending staff and fellows emphasize the relationship betweenclinical problems and the pathophysiology of the underlyingdisease. These informal teaching sessions are supplementedby more formal sessions. These include renal attending rounds,pediatric nephrology educational conferences, renal researchrounds and grand rounds, which are conducted weekly inconjunction with the Renal Division of Barnes-Jewish Hospital.Renal biopsy material is reviewed with the renal pathologists.Attendance at the weekly pediatric grand rounds and pediatriccase conferences is encouraged. Opportunities in clinical andtranslational research projects will be discussed with interestedstudents.
M65 Peds 876 Pediatric Lung TransplantationSt. Louis Children's Hospital has the largest pediatric lungtransplantation experience in North America. This unique clinicalrotation will enable students to be exposed to the process oftransplantation from referral and listing to the actual surgery andpost-operative care. Both inpatient and twice weekly outpatientclinics will be available for participation and learning. The use ofdiagnostic tests, including flexible fiberoptic bronchoscopy withbiopsies and bronchoalveolar lavage, histopathology of infectionand graft rejection, and the complexities of immunosuppressionwill all be explored. Weekly multidisciplinary meetings with ourteam, as well as didactic, psychosocial, and ethical meetings willbe available. Our patient referral base is worldwide, and includesinfants and children with cystic fibrosis, pulmonary hypertension,complex congenital heart defects, surfactant protein defects andalveolar proteinosis.
M65 Peds 878 Clerkship In Rural Primary Care PediatricsThe clerkship in rural primary care pediatrics is designedto provide the student with first-hand experience in generalpediatric practice in a rural community setting. Studentswill have the opportunity to see patients in a private office,participate in delivery room resuscitation, evaluate patients inthe emergency department, and provide pediatric consultation tofamily practitioners, obstetricians, and surgeons. The objectiveof this elective is to provide the student with the experience ofserving as a general pediatrician providing comprehensive healthservices in a rural community. Students assume responsibilityfor ongoing care of patients and have opportunities to performprocedures.Credit 154 units.
