1 Educational Service Provider Evaluation Form The charter contract requires an annual evaluation of the Educational Service Provider (ESP) as per the ESP Policy: Section A3: Note to Academy Boards: This instrument has been designed to assist Boards in the evaluation of their Educational Service Provider (ESP) as per the contractual policy of the Ferris State University Charter Schools Office (CSO). The completed form must: 1. Be signed by each member of the Board and a representative of the ESP 2. Reviewed with the ESP in a regular public meeting of the Board 3. Submitted to Epicenter no later than October 31 of each year The instrument has been broken into key sections for the Board’s consideration: Evidence: In addition to the rating points, the Board must list evidence as to how the score given was reached. The list need not be all inclusive, but should be fairly representative. Measure 3: Student Achievement‐Relative Performance and State/Federal Accountability

a) New Academies After the first and second year of operation, new academies will be expected to improve academic achievement for all groups of students using the following measures: Measure 1: Student Growth Measure 2: Student Achievement‐ Exempt Measure 3: Relative Performance and State/Federal Accountability
At the completion of the third year, cohorts (students enrolled three years) will be established allowing the FSU‐CSO to measure student achievement using Performance Series testing data.
b) Strict Discipline Academies Academies designated as Strict Discipline Academies as per the Michigan School Code may be exempted from certain parts of these requirements due to their unique nature. In all cases, specific educational goals will be mutually developed and agreed upon by the Academy and FSU‐CSO and shall be attached to this contract.
For the remaining questions, a 5‐point rating scale is used. Boards should select the number that closest aligns with their rating for the question. AREA – 2: INSTRUCTIONAL LEADERSHIP (25 POINTS TOTAL) 1. The ESP provides the necessary leadership to ensure that financial and
human resources are appropriately aligned to achieve the academy’s contractual goals.
On average, the Academy will score equal to or greater than the performance of its Composite Resident District in reading and math. Grade 11 Michigan Merit Examination
