Although several studies, includinggenome-wide association scans, arerevealing more about the role of genetics inPsA, the clinical usefulness of currentknowledge in predidcting individualrisk or diseases prognosis is stillundetermined.
Early psoriatic arthritis
• I pazienti con psoriasi o familiarità per psoriasi e alterazioni unguealidovrebbero essere sottoposti a valutazione reumatologica, anche inassenza di chiari segni di artrite.
• La presenza di dolore infiammatorio lombare, l’entesite e la dattilite, in unsoggetto affetto da psoriasi, sono fortemente indicativi di una malattiapsoriasica.
• L’entesite, dimostrabile con la positività al PW, è fortemente indicativa perAPs se interessa un’area non superiore ai 2 mm dall’inserzione sull’osso.
• I criteri CASPAR hanno dimostrato di rappresentare ottimi criteri anchediagnostici in una percentuale molto alta di pazienti.
• Allo stato attuale, nessun marker bioumorale o studio genetico è in gradodi predire la comparsa di ApS in soggetti affetti da psoriasi o diconfermarne la diagnosi.
