The histological features of osteonecrosis are death of osteocyte and the cellular elements in the adjacent marrow. Most investigators concede that the stage immediately preceding cellular demise is the interruption of the blood supply. Bone scintigraphy has been shown to play a useful role in early detecting osteonecrosis of femoral head. We present a case with alcoholism and right hip pain for months. Ischemic osteonecrosis of bilateral femoral heads was detected by bone scintigraphy with SPECT/CT, and was proven by operation and histologic examination 1/2 and 9 months later, respectively.

53 y/o male has alcoholism and right hip pain for months. X-flim of hips on 2007-10-18 showed no obvious bone lesion.

Imp: AVN in bilateral femoral heads, severer on the right side.
