Correlates of Violence in Patients with Schizophrenia

Therapeutic social environments contribute to the successful treatment and rehabilitation for patients with schizophrenia. How-ever, social stigmata of psychiatric disorder in our society have kept away schizophrenic patients from their recovery and adapta-tion. Many persons have thought that psychiatric patients are more violent and criminal because of overwhelming prejudice on untreated or mistreated patients with schizophrenia. The purpose of this review is to propose the new and updated fundamentals of our knowledge on the relationship between schizophrenia and violence. In 1980s, many researchers have a belief that rates of crimi-nal behavior vary independently on the rates of mental disorders. The following scientific studies on various population and meta-analyses of relevant data, overall violence rate have been higher in patients with schizophrenia and other psychotic disorders than in healthy controls. Even though schizophrenia and other psychotic disorders are associated with increased risk of homicide, the association has been not attributed to the psychopathology limited to schizophrenia. Most of all, co-occurrence of substance abuse has played an important role in increasing the possibility of violent behavior. The severe violent offending has been inclined to oc-cur during the early period of psychotic episode before beginning the psychiatric treatment. Thus, most of the violent offending of schizophrenia patient can be prevented by prompt therapeutic interventions and treatments. (Korean J Schizophr Res 2015;18:10-15)

서 훨씬 높았다. 즉, 물질남용이 폭력의 발생에 큰 영향을 주
는 위험요인으로 나타났는데 여자에게서 더 영향이 컸다. 조
현병과 물질남용 어떤 요인이 더 영향이 클 것인가에 대한 분
석을 위해 정신병과 물질남용이 공존하는 남자 환자와 물질
남용만을 가지고 있는 남자환자들을 비교하였는데, 각각 OR
가 9.82(95% CI=5.15-18.7)와 8.88(95% CI=5.88-13.42)로 나
타나 크게 차이가 나지 않았다. 이로써, 정신병 보다는 오히려
물질남용이 폭력성에 훨씬 더 큰 영향을 준다고 추정할 수 있
다. 폭력성으로 인한 타살 행위는 조현병 환자군의 발생위험도
는 0.3%(OR=19.5)였고 일반인구에서는 0.02%로 나타나 조현
병 환자에게서 휠씬 많이 발생하였다. 하지만, 물질남용 군에
서의 발생위험도도 0.3%(OR=10.9)로 나타나 큰 차이를 보이
지 않았다. 같은 해에 이 메타분석의 결과와 관련해서 알코올
관련 환자군(n=34,653)을 대상으로 한 역학 연구결과가 발표
되었는데,32) 중증정신질환 환자에게서 폭력행동의 발생빈도
가 높게 나타났지만 이변량분석(bivariate analysis) 결과에서
는 물질남용 또는 의존이 공존할 경우에만 유의하게 나타났다.
다변량분석(multivariate analysis) 결과에서는 중증정신질환
단독으로는 폭력의 발생을 예측할 수 없었다. 이 결과는 물질
남용과 조현병 어느 것이 더 큰 영향을 주는가에 대한 결정적
인 연구결과로 이로서 조현병의 정신병리가 직접 폭력성과 관
련이 있다고 볼 수는 없게 되었으며, 물질남용이 가장 지대한
영향을 주며 더 중추적인 역할을 한다고 판단할 수 있는 근거
를 제시하였다.
다른 메타분석 논문은 정신병의 첫 번 삽화와 폭력성의 관
계에 대한 분석으로 관련된 9개의 연구결과를 체계적인 고찰
과 함께 분석하였다.33) 정신병의 첫 번 삽화에서 전체 폭력발
생 빈도는 34.5%, 심각한(serious) 폭력은 16.6% 그리고 극심
한(severe) 폭력은 0.6%로 나타나 정신병의 첫 삽화에선 극심
한 폭력은 흔하게 발생하지 않았다. 심각한 폭력의 발생과 관
련된 요인은 병전의 범죄력(OR=4.42), 정신병의 비치료기간
(duration of untreated psychosis : DUP)(OR=2.76), 증상의
심한 정도(OR=2.05)였다. 치료가 시작된 후에 발생하는 폭력
은 강제입원과 관련이 깊었다(OR=5.71).
정신병에서의 폭력발생에 영향을 주는 위험인자를 110여
개의 관련된 연구결과를 분석하여 도출하였는데, 폭력의 발
생빈도는 18.5%(전체 45,533명 중 8,439명)이었다. 분석대상은
모두 조현병은 아니었지만 87.8%의 대다수를 차지하였고, 나
머지는 양극성 장애(0.4%)와 기타 정신병(11.8%)이었다.34) 위험
인자는 적대적 행동, 최근의 약물 오용, 심리치료에 비순응, 충
동조절의 장애, 최근의 물질 오용, 최근의 알코올 오용, 치료약
물에 비순응 그리고 범죄력이었다.
결 론
1980년대 이후 다양한 대상인구에 대한 조사가 수행되었는
데 많은 연구결과에서 조현병과 폭력성의 관련성을 보고하고
있다. 더 나아가 한 연구에선 비록 전체 살인사건에서 조현병
환자가 저지르는 건수는 소수에 해당되지만 살인과 조현병과
는 통계적인 유의성이 있다고 보고하고 있다.31) 산발적인 개개
의 연구결과를 체계적으로 고찰하고 데이터를 메타분석을 하
여 결과를 종합하는 연구들이 최근 출간되고 있다. 이 연구결
과들에서 폭력성은 조현병의 병리와 직접 연관된 것은 아니기
때문에 조현병의 폭력성은 흔히 동반하는 물질관련 장애의 영
향이라는 견해가 있으며 한 연구에선 조현병과 물질장애가 동
반된 환자군과 물질장애만을 진단받은 환자군의 폭력성의 차
이는 없다고 보고하였다. 또한 폭력은 조현병의 전체 이환된 경
과에서 모두 나타나는 현상이 아닌 첫 정신병 삽화에서 주로
나타나며, 반수가 삽화의 발생에서 치료를 받게 되는 시점 사
이에 주로 발생한다고 하였다. 이 시기에 발생한 폭력에는 타
인에 대한 폭력 행사도 포함되고 심각한 경우도 일어나지만
영구적인 손상을 주는 정도의 심한 경우는 드물다고 하였다.
폭력의 발생의 위험인자는 범죄력을 비롯하여 치료를 거부하
는 행동, 물질남용과 관련된 요인들이 포함된다고 하였으며,
이런 요인들은 정신병에서 발생하는 폭력을 줄이기 위한 개입
에서 중점적인 사안이 되어야 한다고 주장되고 있다. 더불어,
조현병 환자에서 폭력성의 행동화에 대한 조기선별과 예방을
위해 DUP를 최소화 하기 위한 정신의학적 중재가 적극적으
로 이뤄져야 할 것으로 추정된다.
