Introduction

Geetings to the inaugural issue of the Perak CRC's Network Bulletin.

This bulletin represents the work and achievements of the three Clinical

Research Centre's in Perak ie CRC HRPB Ipoh, CRC Taiping and CRC

Hospital Sri Manjung. The idea of having this bulletin stemmed from

comments of our State Health Director Dato Dr Ding Lay Ming and

HRPB Hospital Director, Dr Teo Gim Sian.

Situated in some corner of a hospital, CRC's are often paid scant

attention by clinicians. The research and work produced goes

unnoticed and sometimes unrecognised. The aim of this bulletin is to

create an awareness of the various research related activities conducted by the various CRC's in

Perak. A calendar of courses and workshops is also provided so that clinicians and those

interested in research can plan for their required training needs.

The bulletin will also highlight the research achievements of not only the CRC staff but also

clinicians who have produced landmark papers. Dato Dr Amar Singh has left an indelible mark

on the research landscape in Perak. Filling his shoes is an impossible task ... alone. Working

together, with the State Health Directors, Hospital Directors, Clinicians and other Allied Health

Staff, we hope the future of research in the State of Perak will continue to grow and expand.

This bulletin is a step in that direction.

Clinical Audit in Hospital

In September 2018, Hospital Raja Permaisuri Bainun successfully

organised a clinical audit competition involving participations from all

the departments. The aims of holding this clinical audit competition

were to address the findings of clinical audit and to acknowledge the

efforts of healthcare providers who have been committed in conducting

clinical audits.

Most of the healthcare providers felt a degree of trepidation about

undertaking a clinical audit. This may be because of their uncertainty

about clinical audit, its purpose and importance in health care setting. A

common recurring concern that healthcare professionals have is to

understand how clinical audit differs from research.

The purpose of clinical audit is to monitor to what extent we follow the

evidence-based standards for any given healthcare activity.

Subsequently, we need to identify the reasons of why standards are not

met and finally to ensure that any actions implemented lead to the

planned outcomes. Clinical audit is seen as the final step in evidence

based healthcare practice. Therefore, healthcare providers are

encourage to conduct a clinical audit, and the audit results should be

presented and disseminated wherever possible to share learning,

support change and celebrate good practice.

The BA/BE study obtained approval from the Malaysia Research and Ethics Committee (MREC) at the end of year 2017. The BABE activities

involved participation of healthy volunteers. The recruitment team started the recruitment activities since early year 2018. The screening activity

was carried out in October 2018 and the clinical phase was conducted in November 2018.

The BA/BE study involved personnel from multiple teams, including doctors, nurses, pharmacists and dietician. Team work is one of the most

important elements to make any BA/BE study successful. Besides, competent investigators are necessary for risk assessment and mitigation plan

during the trial. For instance, during the first week of the trial, there were difficulties in subjects’ blood withdrawal. The principal investigator

then hired medical officers from the Anesthetic Department as phlebotomist in the second week of the trial. This action has significantly

improved the blood withdrawal process and reduces the time deviation of subjects’ blood withdrawal. We have learnt that it is important to put

the right person on the right task.

After the clinical phase, the blood samples were sent to the bioanalytical site located in Bayan Lepas, Penang for analysis. The bioequivalence

report is expected to be produced in the middle of March 2019. Next, CRC HRPB Ipoh will arrange for NPRA inspection in order to be

accredited as a Bioequivalence Clinical Site. We expect to conduct more BA/BE trials after the accreditation happened.

Continuous Medical Education (CME) session

On the 25th of January 2019, Clinical Research Centre HRPB Ipoh

has participated in the Hospital Level CME session. The CME session

was held in the Auditorium of Hospital Raja Permaisuri Bainun

(HRPB) Ipoh and attended by more than 200 staffs working in the

HRPB.

The title of the CME was “BABE: Is ORI better?” The speaker was Mr.

Chang Chee Tao, pharmacists and the project manager of the study.

The aim of CME topic was to bring awareness to the audience about

the importance of Bioavailability and Bioequivalence (BA/BE) studies

in the country and the experience of the CRC HRPB Ipoh Team in

conducting the BA/BE study. The session also informed and

discussed the effectiveness of generic drug in comparison to the

original (innovator) version of the drug.

Hospital Taiping, 25 and 26 February 2019 – We, Dr. Lim Xin Jie, Ms. Chew

Chii Chii and Ms. Premaa Supramaniam, were invited as speakers for

‘Introduction to Clinical Research’ workshop. This workshop was organised

by Clinical Research Centre, Hospital Taiping and the 2-day short course

took place at Bougainvillae hall, CME building of Hospital Taiping.

Dr. Lim presented lectures about introduction and requirement for clinical

research, and Chii Chii talked about research ethics and grants application

on the first day of the workshop. On the following day, Premaa delivered

lectures on two important topics about research i.e study designs and

statistics in research. The remaining topics of the workshop were

conducted by staffs from Clinical Research Centre, Hospital Taiping.

The aim of this workshop was to introduce healthcare staffs about clinical

research. The participants attended the workshop include staff nurses,

medical officers, assistant medical officers, dental officers, lab technician,

pharmacist and specialist from different departments.

During the recent CRC Network Meeting on 21st and 22nd February, the National CRC

and 33 hospital CRCs came together to discuss the directions for research in 2019. The

1st day of the meeting was held at Holiday Villa Subang. The meeting was led by Dato’

Dr Goh Pik Pin and Dr Kalaiarasu M. Peariasamy, started off with introductions of new

CRC members, followed by 2018’s achievements, and the roles and functions of the

CRC Head and unit. Each CRC voiced challenges faced, and their plans for research in

2019. There was also a dialogue with CRM, where various issues pertaining to industry

sponsored research (ISR), including legal and financial issues, were discussed.

On the 2nd day, the meeting was moved to the National Institutes of Health (NIH) at

Setia Alam. The meeting began with a brief tour of the NIH and a group photograph.

Each unit within the Institute of Clinical Research (previously known as the CRC)

introduced their teams to the hospital CRCs, and presented their research plans for the

year. Research priority areas were identified and mechanisms to increase IIR output

were discussed.

The take-home messages from the CRC Network Meeting are:

1. Focus on investigator initiated research (IIR), and conduct research on patient-

important outcomes that will translate into policy and practice

2. Plan research projects early in the year, in line with national research priority areas, to

aim for funding opportunities through national research grants

3. Use existing resources wisely to generate research output and achieve target KPIs, to

handle upcoming budget and staff cuts.

Clinical Audit Workshop

Kolej ADTEC Taiping, 8th March 2019. A BABE Roadshow was

organised by Clinical Research Centre, Hospital Taiping. The

aims of the roadshow were to reveal the BABE study to

students, and invite them as healthy volunteers for BABE study

conducted in Hospital Taiping in the future.

Overall, there were forty students and college staffs attended

the roadshow. 15 of them registered themselves as BABE

volunteers. The roadshow was ended perfectly. We wish to

have more volunteers in the future roadshows.

Basic Statistics Workshop

Hospital Seri Manjung, 14 March 2019 – Basic Statistics Workshop was organized by

CRC Seri Manjung in collaboration with CRC Taiping and Pharmacy Department of

Hospital Seri Manjung (HSM). Speakers for this workshop were Dr. Narwani binti

Hussin (Deputy Head of CRC Taiping) and Pn. Normi binti Hamdan (Head of Clinical

Pharmacy, HSM). Ms. Chiew Shoen Chuen (CRC HSM Research Pharmacist) was the

facilitator. This one-day workshop was held at HSM Library.

The aims of this workshop were to introduce healthcare staffs about statistics concept

and Statistical Package for the Social Sciences (SPSS) data entry as well as data

analysis. This event was attended by specialists, medical officers, assistant medical

officers, pharmacists and staff nurses from several institutions in Manjung, Perak

Tengah and Hilir Perak districts.

BA/BE Roadshow

Hospital Tapah, 11 March 2019 – Nursing Clinical Audit Workshop was held at the CME hall,

Hospital Tapah. The event was organized by Nursing JKN Perak. It is a great honour for

Clinical Research Centre HRPB Ipoh to be invited as a speaker and facilitator for this event.

According to National Health Service (NHS), United Kingdom, Clinical Audit is defined as a

quality improvement process that seeks to improve patient care and outcome through

systematic review of care against explicit criteria and the implementation of change.

Therefore, Clinical Audit is one of the vital aspects in clinical governance. It is also an

important requirement in the Hospital Accreditation Program.

This event was attended by nurses from various healthcare settings. At the event, Ms.

Premaa Supramaniam and Dr. Lim Xin Jie from CRC HRPB were given the opportunity to

deliver a topic on “Introduction of Clinical Audit” and “Pub med Literature Search." At the

end of the workshop, participants were expected to have basic understanding of the

purpose and role of audits in improving the quality of health services and develop their

audit plan ready to be conducted in their local setting.

Sandakan, 29-30 March – Representative from Hospital Clinical Research

Centre (HCRC), Raja Permaisuri Bainun Hospital (HRPB), Ipoh attended

Basic Research Methodology and Biostatistics (RMB) workshop as

speaker and facilitator for the 2-days event conducted in HCRC, Duchess

of Kent Hospital (HDOK), Sandakan. Sharing of resources among

Hospital CRCs was well established when a representative from CRC

HRPB, Ms Premaa Supramaniam (Research Officer) was invited for the

hands-on training together with another representative from HCRC,

Queen Elizabeth II Hospital, Kota Kinabalu, Dr Teoh Eu Vin.

The workshop was well attended by 21

participants comprising of doctors,

pharmacists, nurses and orthodontist

from HDOK. All the participants

were exposed and successfully trained in this workshop. Among the topics covered include the elements of

research methodology, basic medical statistics, research protocol writing and National Medical Research

Registry (NMRR) online registration. The participants were also given opportunity to learn through hands-

on activities on basic statistical analysis using statistical package, SPSS.

It is a great honour to be invited by the Head of Unit, Mr. Lai Chung Ket of HCRC HDOK and his team to

speak in this workshop. Sharing of resources and collaborative efforts will be fruitful if the local trainers are

well-trained and therefore catalyst for more trainings to be conducted locally for the benefits of research

investigators.

workshop was organized

by CRC HSM and it took

place at HSM Library.

The purposes of the workshop were to discuss and refine the proposal of

each study as well as assist new researchers in producing high quality and ethical proposals. The participants who attended this workshop

included specialists, medical officers, pharmacists, staff nurses and physiotherapists from several institutions in Manjung, Perak Tengah and

Hilir Perak districts.

Good Clinical Practice (GCP)

Taiping, 1st to 3rd April 2019, Clinical Research Centre, Hospital Taiping

organised a Good Clinical Practice (GCP) workshop. The venue was in

Bougainvillea hall, CME Building of Hospital Taiping. A total number of

38 healthcare professionals including doctors, dentists, nurses and

pharmacists from Hospital Taiping and other health facilities in Perak

state were invited.

There were five invited speakers on that day. Among them were Dr.

Letchuman Ramanathan (Consultant & Head of Medical Department

Hospital Raja Permaisuri Bainun), Dr. Saiful Rijal Muhammad

The objective of the program was to provide information about clinical research and install good clinical practices to participants. It is

mandatory for all new candidates who plan to sit for GCP exam for the first time to attend and complete the GCP workshop. GCP certification is

required for all individuals involved in the conduct of clinical trials. Overall, the passing rate of the GCP certification examination was 78%.

Introduction to Clinical Research for Paramedics Workshop

IPOH, 9th & 16th April 2019 - Clinical Research Centre, Hospital Raja Permaisuri Bainun had organised a National Medical Research Register

(NMRR) workshop. Total number of 37 nurses from Health District Office (PKD) and hospitals in Perak participated in this workshop.

The workshop was held with the aim to expose the nurses in NMRR registration system. The ways of creating a personal NMRR account and

steps in NMRR registration were demonstrated by Sn. Hasni Adha bt Ibrahim from CRC HRPB. Through this workshop, 20 audit projects on

various topics related to nursing care were registered.

NMRR Workshop (Post-Basic)

On 09 April 2019 from 9am to 12 pm, Clinical Research Centre, Hospital

Raja Permaisuri Bainun had organised a National Medical Research

Register (NMRR) workshop. This workshop is jointly collaboration with

the Ministry of Health Training Centre, Sultan Azlan Shah, Ulu Kinta,

Perak. A total number of 37 Post-Basic Nephrology students participated

in this workshop.

The aim of this workshop was to expose the students on the NMRR

system and website. Hands-on tutorial was also given by registering their

research projects during the workshop. Nine research projects on

nephrology were registered during this workshop. The research protocols

and documents were reviewed by the CRC staff, and some changes or

recommendations were required to improve the validity and ethical

aspects of the proposal. The revised proposals were emailed to the

supervisor for the final check before completing the research registration.

NMRR Workshop (Clinical Audit)

Taiping, 9 - 10 April 2019- Clinical Research Centre of Hospital Taiping had

organised “Introduction to Clinical Research for Paramedics” workshop. This is the

first-time workshop ever conducted by CRC Taiping, which involved multiple units

from hospitals and district's healthcare facilities in Perak. The two-day course was

held in Bougainvillea hall, CME Building of Hospital Taiping. The total number of

participants was 37. The speakers were all from staffs of CRC Taiping. They were Dr.

Narwani Binti Hussein, Dr. Rosilawati Binti Abdul Rani, Prema a/p Muninathan, Chan

Wai Seong Christopher, and Chua Kin Wei. We assume there were good responses

from participants. We are planning to have a research camp for this target group

soon.

Learning Centre, 9th to 11th April 2019 -

Introduction to Clinical Research (ICR) Workshop

was organized by Clinical Research Centre,

Hospital Seri Manjung for 2 and a half days. The

speakers of the events included by Dr. Nga Shih

Hang (Head of CRC HSM), Dr. Chang Meng Lee

(Deputy Head of CRC HSM), Dr. Yeoh Soo Lin

(Head of Outpatient Department, HSM), Dr. Sree

Kantan Nayar (FMS), Ms. Chiew Shoen Chuen

(CRC HSM Research Pharmacist), Ms. Siti

Maisarah Mohd Muksin (Study Coordinator, CRM) and Ms. Azirah Osman (Study

Coordinator, CRM). The workshop was held with the aim of introducing healthcare staffs

about clinical research. The participants who attended the workshop included specialists,

medical officers, dental officers, assistant medical officers, pharmacists, staff nurses, lab

technicians and physiotherapists from various healthcare settings in Perak.

Mini ICR Roadshow

Gerik, 16th April 2019. Hospital Gerik was given the opportunity to conduct mini ICR roadshow

this year, which is organised by Clinical Research Centre. The aims of the program were to

encourage and engage with the potential

researchers, and install research culture in

Hospital Gerik. The program was facilitated

by four of CRC staffs which comprise of Dr.

Narwani Binti Hussein, Dr. Rosilawati Binti

Abdul Rani, Prema a/p Muninathan, and Sreevali a/p Muthuvadivelu. Among the topics

covered in roadshow roadshow included “The Research Proposal," “Literature Review,"

"Methods," “Variable & Research Instrument," and “Ethics in Research & NMRR Registration."

A total of 36 participants from various disciplines were attended the roadshow.

SPSS 2.0 for Nurses

Taiping, 17th April 2019. A workshop of SPSS 2.0 was conducted by Clinical Research Centre, Hospital Taiping. The participants were nurses

from Pejabat Kesihatan Daerah, Larut Matang & Selama. It was facilitated by Dr. Narwani Binti Hussein , Prema a/p Muninathan, and

Sreevali a/p Muthuvadivelu. The program was aimed to facilitate nurses from the health facility in their clinical audits.

IPOH, 25 & 26 April 2019 – Clinical Audit Workshop 2019 was held at Hospital Raja Permaisuri Bainun

(HRPB). CRC HRPB was taking initiative in organising this workshop for those hospital staffs that

shown interest in Clinical Audit. The aim of this workshop is to improve understanding of clinical

audit and help to develop their own audit plan ready to be conducted in their local setting. This 2

days event was well attended by over 20 participants comprising of doctors, pharmacist and nurses.

This workshop was a combination of presentations and hands-on audit projects. Dr. Hor Chee Peng

(Deputy Director of Hospital Kepala Batas) was the invited speaker and facilitator in this workshop.

Clinical Audit remains one of the important tools that can be used to facilitate change and

improvement in the healthcare system. Therefore, to initiate a clinical audit is expected to be far more

profitable and should be encouraged by stakeholder.
