We studied the diffe rences between trac h eal deviation patterns in 23 patients with fibroatelectatic pulmonary tuberculosis and 35 patients with e longat ed and dilated aortas In cases of re traction of the trachea by fibroatelectatic pulmonary tuberculosis. the diamet er of the deviated seg- me nt was greater than that of the normal segment and deviation of th e wall adjacent to the fibroatelectasis from its normal position was greater than that of the opposite wal l. In cases of compression of the trachea by the e longated and dilaf ed aorta. the diameter of th e diviat ed segment was smaller than that of the normal segment and deviation of the wall adjacent to the aortic arch from its normal position was greater than that of the opposite wal l. We conclude that these differenc es betwee n tracheal deviation pattems are useful signs for discriminating r et raction from compression. Thus when the trachea is retracted. the deviation of the juxtalesional wall is greater than that of the lesion-free wall. and vice versa. tion may be caused either by traction toward the diseased hemithorax or by compression toward the norma l side.
Unless an obvious mass is observed radiographically. occasionally it can be difficult to decide whether the trachea
has been pushed or pulled from its normal position in the mediastinum. We studied the differences between tracheal
deviation patterns in 23 patients with fibroatelectatic pulmonary tuberculosis and 35 patients with elongated and
dilated aortas
In cases of re traction of the trachea by fibroatelectatic pulmonary tuberculosis. the diameter of the deviated seg
m ent was greater than that of the normal segment and deviation of the wall adjacent to the fibroatelectasis from
its normal position was greater than that of the opposite wal l. In cases of compression of the trachea by the elongated
and dilafed aorta. the diameter of the diviated segment was smaller than that of the normal segment and deviation
of the wall adjacent to the aortic arch from its normal position was greater than that of the opposite wal l.
We conclude that these differences between tracheal deviation pattems are useful signs for discriminating retraction
from compression. Thus when the trachea is retracted. the deviation of the juxtalesional wall is greater than that
of the lesion-free wall. and vice versa.

서 론
기관은 연골을 가진 비교적 단단한 관상 구조물로 양측벽
이 평행하며, 상종격동의 중앙선에 위치하고 있다Ol. 기관
의 전위나 변형은 여러 원인에 의해 초래되지만, 폐허탈성
결핵 때와 같이 기관이 끌리는 경우와, 종격동 종양이나 대
동맥경화 때와 같이 공간 점유성 병변에 의하여 기관이 압
박되어 전위를 초래하는 경우로 나눠진다(2, 3). 일반적으로
견인이건 압박이건 폐허탈 또는 종격동 종양 등 원인이 뚜
렷할 경우 감별진단에 문제가 없으나, 원인 병변이 애매할
경우 기관 전위의 원인 규명이 쉽지 않다. 저자들은 폐허탈
성 결핵과 대동맥 확장에 의한 기관의 전위를 비교 분석하
여 유의한 차이를 얻었으므로 이에 보고하고자 한다.
대상 및 방법
폐허탈성 결핵 환자는 23명이었으며, 이들의 연령은 32세

에서 78세로 평균 54세였고, 남자 15명 여자 8명이었다. 대
동맥 확장 환자는 35명이었으며, 이들의 연령은 43세에서
83세로 평균 60세였고, 남자 19명 여자 16명이었다. 고투과
내지는 준고투과 조건으로 촬영된 정면 흉부 X선사진상 전
위된 부위 직상부의 정상 기관의 직경과, 가장 많이 전위된
부위의 기관 직경을 각각 계측하였다. 중앙선에서 가까운
기관벽을 근벽으로 하고 중앙선에서 먼 기관벽을 원벽으로
하여, 전위되기 전 원래 위치를 가상하여 이로부터 전위된
거리를 각각 계측하였다.

폐허탈성 결핵의 경우 정상부위의 기관 직경은 1. 7:t 0. 2
cm 이었으며, 전위된 부위의 기관 직경은 2. 1::t O. 3cm 이
었다. 기관 원벽의 전위는 1. 8::t O. 7 cm 이었고, 기관 근벽
의 전위는 1.4 ::t O .7cm 이었다. 즉, 전위된 부위의 직경이 정
상부위의 직경보다 유의하게 증가하였으며, 원벽의 전위가
근벽의 전위보다 유의하게 컸다(p(O.Ol) (Fig. 1)
대동맥 확장에 의한 전위의 경우 정상부위의 기관 직경은
1.7::t O.2cm 이었고, 전위된 부위의 기관 직경은 1.5::t O.2cm
이었다. 기관 원벽과 근벽의 전위는 각각 0.5::t O.2cm 및 0

7::t O.2cm 이었다. 즉, 전위된 부위의 직경이 정상부위의 직
경보다 유의하게 작았으며, 근벽의 전위가 원벽의 전위보다
유의하게 컸다(P(O.Ol) (Fig. 2)

기관은 후두에서 용골(carina)에 이르는 연골성 및 막성
관상 구조물로「 정연 흉부 X선사진상 상종격동의 중앙선에
위치하며 관모양의 검은 공기 음영으로 보인다. 기관의 단
면 모양은 많은 변형이 있지만, 기관의 양측 벽은 내부에 있
는 공기에 의해 평행한 앓은 선상 구조물로 잘 묘출된다( 1,
4, 5). 기관의 변형이나 전위를 일으키는 원인은 다양하다. 영아
에서는 기관이 유연하고 유동적이어서 기관의 직경과 위치
에 있어서 변형이 심하다 (6). 또한 연령이 증가할수록 용골
직상부 좌측에서 대동맥에 의한 압박이 흔히 보인다(1).
이밖에 기관의 직경을 감소시키거나 증가시키는 질환을
기 관 의 전 위 유 무 에 따 라 분 류 하 연 Table 1과 같 다.
Saber-sheath trachea는 기관의 심한 관상 협착을 일으키
나 시상 확장을 동반하여, 정면 흉부X선 사진상에서는 기관
의 미만성 협착을 보이나 측면 흉부X선 사진상에서는 오히
