Martin County Water District {"Martin District"), a water district organized pursuant to KRSChapter 74, owns and operates facilities that treat and distribute water to approximately 3,533customers in Martin County, Kentucky.
Questions I-17 are related to ongoing Management Audit recommendations steps.

a. Since August 2015, found 2 cheater bars and turned over tothe County Attorney for prosecution.

Step 5. Provide an updateof MartinDistrict's progress in replacingor repairingall excessively leakingmains and services as outlinedin Step 5. Identify anydecreases in water loss resulting from it efforts.
a. Reference Step 2 and 3 above. The leaks mention abovehave been repaired. Our storage tanks in these areas aremaintained at the appropriate levels. Water loss in theseareas will be determined at the end of tlie month.

Steps 1-4. Provide Martin District's formal, written preventive maintenanceprogram.
a. The Disti-ict has a Water Loss and Leak Detection Programfor Preventive Maintenance.

a. After consulting with the Health Inspector, concerning thewater shed, he is aware of the situation and monitors thearea where residents live. No complaints at this time.

Steps 1, 2, 3, and 6. Other than working with the electric providers to reducepower outages, explain how Martin District is addressing this recommendation.
a. The District has purchased a 300kw mobile generator thatcan run the plant or any other pump station in the system.The District has spent approximately $23000.00 for thegenerator. This has and will continue to assist the Districtin covering our power outages.
b. Also, the power company has been clearing trees fromoverhead power lines.

Other than working with the Big Sandy ADD, explain what internal or otherexternal steps Martin District is taking to address this recommendation.

C. For each fire department that made a withdrawal from the MartinDistrict system for the purpose of extinguishing fires or trainingpurposes during the period January 1, 2013, to present, provide a copyof the fire department's estimate of the withdrawal.
d. Estimates were made by the District's Office since noreports were present for withdrawals and placed on thewater loss report as estimates.
D. For any instance in which a fire department failed to provide anestimate of a withdrawal during tlie period January 1, 2013, to present,state the actions that Martin District took concerning the failure.
e. A meeting was held with the County Judge Executive theChairman of the Board and Business Manager, concerningthe lack of water withdrawal reports from the FireDepartments and was informed that the Judge Executivewould withhold funds from the Fire Departments.
E. Provide a copy of each record kept by Martin District pursuant to 807KAR 5:006, Section 26(3), for the period January 1, 2015, to date.

The water supplied toDistrict's customers is metered by the outgoing meter at thepump station. The difference in water going to the HoneyBranch Tank and water supplied to the Martin Countycustomers is purchased by Prestonsburg Utilities at $3.05per thousand. Ifwe do not pump sufficient amount ofwater to supply our businesses and customers mention, wemust pay Prestonsburg Utilities $3.05 per thousand.Prestonsburg Utilities has the Contract with the UnitedStates Prison

Water Loss and Leak Detection Program
Steps to follow:
Martin County Water DistrictWater Loss Plan
February 2016
Read the master meters daily and at the same time each day. At aminimum, they should be read Monday through Friday. This willminimize water loss due to a large leak that can go undetected for aweek or month.
Divide the distribution system into zones or subsections wherepossible and calculate water loss for eachzone. This will allowforthe prioritizing of work basedupon the severity of the problem in aparticular zone.
Utilize computer billingsoftware to generate water loss reports forsections or zones as well as to generate an overall water loss report.
Install bypass monitormeters as needed to isolate lines withpotentialleaks. A 5/8- x 3/4- inch meter will suffice for each 100 customer
section of line.
Install a 2 - inch bypass monitor meter at water storage tanks toisolate sections of the line with potential leaks during the night (1:00a.m. to 4:00 a.m.)
Utilize pressure recordings to detect fluctuating pressures andabnormally low or high pressures in distribution systems lines.
Test and change-out all meters according to Kentucky Public ServiceCommission (PSC) regulations. PSC regulations require residentialmeters to be tested and changed-out in 10-year intervals. Four-inchand larger meters are to be tested annually.
It may be feasible to hire a part-time operator to utilize leak detectionequipment to search for leaks. A portion of the distribution systemcould be covered each month.
Identify sections of pipe in the distribution system with the mostfrequent line breaks. Budgeting for infrastructure replacement isimperative in any water utility.
Having a main transmission line from the master meter to a waterstorage tank will reduce pressure fluctuations in the distributionsystem and result in fewer line breaks.
Provide the necessary resources for manpower and equipment toproperly maintain the distribution system appurtenances such as gatevalves, pressure reducing valves, and hydrants.
COMPONENTS OF A
WATER LOSS PREVENTION PLAN
How do the terms "water loss" and "weight gain"relate? Can it be that apathy, procrastination, or plainidleness might apply to both situations? Definitely, goodintentions abound with either problem. No one wants tobe overweight, just as no one who is responsible for themanagement and operation of a water utility wants tohave excessive water loss, How do we attack these
problems? In either case, we must identify the root of theproblem, focus on a solution and stick with it! How manytimes have we heard the phrase, "If you fail to plan, youplan to fail." A good plan is the key to any long-termsolution.
A person's physical and psychological make-up hasa huge impact when attempting to solve the weightproblem. Heredity affects us In many ways, butespecially in regards to our ability to lose weight. Whatabout the person who accepted the position of managerand soon leamed that they had inherited an inadequatelyoperated water system?
Age is another common factor in both problems. Awater distribution system that was installed in the WPAdays of the 1930's will most likely experience problemsthat newly installed water lines will not possess. Theolder that 1get, the harder it is to keep the weight off! Wecan list other analogies such as our body shapes andsizes compared to the geographical terrain of our utilitiesand our distribution system sizes. However, let's getgoing with a proactive approach to the problem of waterloss.
Accurate records are vital to any water lossprevention plan. Howdo we know the status of our waterloss if we do not keep records? There is a water losstemplate that is available for download from the KentuckyRural Water Association website
fwww.krwa.oroL This Excel spreadsheet, or a similarrecord keeping system, can be utilized in a preliminarywater audit. The initial step in water loss prevention isthe water loss calculation. Secondly we must locate andeliminate all water leaks. Sounds easy, doesn't it?
The following steps can be utilized to prevent orreduce water loss and should be incorporated into awater loss prevention plan:
1. Read the master meters daily and at the same timeeach day. At a minimum, they should be read Mondaythrough Friday. This will minimize water loss due to alarge leak that can go undetected for a week or month.
2. Read all meters in the distribution system within a 3-to 5-day window. When the meters are read over a
2-week period, this will cause fluctuations of monthlywater loss numbers. However, these numbers willaverage out over a year's period of time.
3. Divide the distribution system into zones orsubsections where possible and calculate water lossfor each zone. This willallow for the prioritizing of workbased upon the severity of the problem in a particularzone.
4. Utilize computer billing software to generate water lossreports for sections or zones as well as to generate anoverall water loss report.
5. Install bypass monitor meters as needed to isolatelines with potential leaks. A 5/8- x 3/4-inch meter willsuffice for each 100-customer section of line.

Today's advanced technology can certainly enhance ourwater loss prevention plan. Computers not only utilizesoftware for spreadsheets to caloulate water lossexpediently, but can be used ina variety of ways to identifyareas of the distribution system with potential leaks. Bothmaster meters and customer meters can be read bysatellites or other automated meter reading mechanisms.
Telemetry/SCADA systems operated with computers canproduce trend charts for water flows, water pressures, andwater levels in storage tanks. This kind of data is valuablein determining where leaks are or are not prevalent.Computers analyze hydraulic data to determine iftheoretical and actual water flows and water pressures inthe distribution system match. Computerized maps withGPS and CIS data are beneficial when used properly. Awater utility's budget is the major limiting factor as to whytechnology is not used more frequently.
By industry standards, more than 15 percent waterloss in a rural system in unacceptable. Probably, no onerealizes this more than the managers of water districts,water associations, and investor-owned utilities under thejurisdiction of the Kentucky Public Service Commission.Just as we should be concerned with our health due to
being overweight, the PSC is concerned with the financialhealth and well-being of water utilities under theirjurisdiction in Kentucky. PSC inspectors routinelydiscuss water loss during their inspections. When awater system exceeds 15 percent water loss on theirannual report to the PSC, a deficiency Is issued.Numerous water systems' response to the PSC'sDeficiency Tracking Reports (DTR) has been deemedunacceptable by PSC. A common request from PSC tothe water system with a deficiency due to water loss is fora water loss control plan. A good water loss control planshould include the above-mentioned components with atime frame to implement the Improvements and follow-upevaluations to measure the success of the plan.
Whether we are weighing in or wading in, we shouldalways do so with a goal in mind. We cannot continue toignore our problem and hope it resolves itself. Just asthere are various diets to control an individual's weight,there are various methods for controlling water loss.

*We are required to monitor your drinking waterforspecific contaminants on a regular basis. Results ofregular monitoring are an indicator ofwhether ornot our drinking water meets health standards. During2/1/2013 - 2/28/2013, we did not complete all monitoring ortesting for chlorine and turbidity, andtherefore cannot besure ofthe quality ofyour drinking water during that time. *
There is nothing you need to do atthis time. You may continue to drink the water. If a situation ariseswhere the water isno longer safe to drink, you will be notified within 24 hours.
By the 10^ of each month we must submit a Monthly Operation Report (MOR) to the Division of Water.The MOR for February 2013 did not arrive by the required deadline and this caused three violations. Oneviolation for not submitting the MOR by the required deadline, another for not submitting dally chlorineresults, and another for failing to submit the daily turbidity readings. (The chlorine results and turbidityreadings are contained within the MOR.) We have modified our procedures to assure that the MORarrives by the required deadline.

We routinely monitor for the presence of drinking water contaminants. Testing resultsfrom 7/1/2014 through 9/30/2014 show that our system exceeds the standard, ormaximum contaminant lave! (MCL), for trihalomethanes (THM) and haloacetic acids(HAA). The standard for THM is 0.080 mg/L and the standard for HAA is 0.060 mg/LThese are determined by averaging ail samples collected at each sampling location forthe iast 12 months. The level of THM averaged at one of our system's locations for7/1/2014 to 9/30/2014 was 0.101 mg/L and HAA was 0.081 mg/L.
o There is nothing you need to do. You do not need to boilyour water or take othercorrective actions. If a situation arises where the water is no longer safe to drink,you will be notified within 24 hours.

We routinely monitor for the presence of drinking water contaminants. Testing resultsfrom 10/1/2014 through 12/31/2014 show that our system exceeds the standard, ormaximum contaminant level (MCL), fortrihalomethanes (THM) and haloacetic acids(HAA). The standard forTHM is 0.080 mg/L and the standard for HAA is 0.060 mg/L.These are determined by averaging all samples collected at each sampling locationforthe last 12 months. The level of THM averaged at one of our system's locations for10/1/2014 to 12/31/2014 was 0.096 mg/L and H/V\ was 0.105 mg/L.
• There is nothing you need to do. You do not need to boil your water or take othercorrective actions. If a situation arises where the water is no longer safe to drink,you will be notified within 24 hours.
• Ifyou have a severely compromised immune system, have an infant, arepregnant, or are elderly, you may be at increased risk and should seek advicefrom your health care providers about drinking this water.

Ourwater system recently violated a drinking water requirement. Although this was notan emergency you have a right to know what happened.We routinely monitor your waterfor turbidity (cloudiness). This tells us whetherwe areeffectively filtering the water supply. On 2/23/2015 we had a turbidity result of 1.195NTU. This was above the standard of 1.0 MTU. Water samples for February 2015showed that 6.58 percent of turbidity measurements were over 0.3 turbidity units - thestandard is that no more than 5 percent of samples may exceed 0.3 turbidity units permonth. The turbidity levels are relatively low. However, their persistence is a concern.Normal turbidity levels at our plant are 0.12 units. This is notan emergency. If it hadbeen, you would have been notified within 24 hours.

Ourwater system recently violated a drinking water standard. Although this incident wasnot an emergency, as our customers, you have a right to know what happened andwhat we did (are doing) to correct this situation.
We routinely monitor for the presence ofdrinking water contaminants. Testing resultsfrom 7/1/2015 through 9/30/2015 and 10/1/2015 through 12/31/2015 show that oursystem exceeds the standard, or maximum contaminant level (MCL), fortrihalomethanes (THM) and haloacetic acids (HAA). The standard for THM is 0.080mg/L and the standard for H/V\ is 0.060 mg/L. These are determined by averaging allsamples collected at each sampling location for the last 12 months. The level ofTHMaveraged at one of our system's locations for 7/1/2015 to 9/30/2015 was 0.081 mg/Land HAA was 0.070 mg/L. The level of THM for 10/1/2015 to 12/31/2015 was 0.100mg/L
• There is nothing you need to do. You do not need to boil your water or take othercorrective actions. If a situation arises where the water is no longer safe to drink,you will be notified within 24 hours.
• if you have a severely compromised immune system, have an infant, arepregnant, or are elderly, you may be at increased risk and should seek advicefrom your health care providers about drinking this water.
Some people who drink water containing trihalomethanes in excess of the MCL overmany years may experience problems with their liver, kidneys, or central nervoussystem, and mayhave an increased n'sk ofgetting cancer. Some people who drinkwater containing haloacetic acids in excess of the MCL over many years may have anincreased n'sk of getting cancer.

We are working to minimize the formation of disinfection by-products while ensuring wemaintain an adequate level of disinfectant.We have taken steps to change disinfectantlevels, remove natural organic matter, and increased flushing of water lines. Weanticipate resolving the problem within the next six months.

Comments: Wolf creek, Coldwater, Inez 1&2 tanks are not in service. The system does not usethese tanks at this time. The Systemhas switched from gas chlorine to liquid bleach disinfectant.Thesystem is using a Thornsorb activated carbon system. The water treatment planthas a 100,000gallon clearwell at the plant and t^vo 500,000 gallon clearwell tanks on thehill above theplant.
Thesystem needs to perform a complete system flush however approximately 50% water loss andlow storage dueto distribution leaks in the Warfield area make it difficult. Failureto properly flushthe system will make meeting future DBF limits a difficult task.

Dear Mr. Alley,
The Division of Water has considered your request and will grant an extension to the required repeatpublic notice for the ongoing disinfection byproduct MCL exceedance or a reduction in frequency of therepeat public notice. This will depend on the Locational Running Annual Average that is calculated forthe4'" quarter of 2015.
If the water system continues toexceed the MCL for disinfection byproducts for the 4*"^ quarter, thewatersystem will use the 4'** quarter results in the repeat public notice. If the water system comes back intocompliance inthe4' quarter, the water system will be required to perform a public notice using the3^*^quarter results.
The public notice shall be performed no later than February 10th, 2016.
It isnot required for the water system toreceive a 4*^ quarter Notice ofViolation before issuing the publicnotice. The water system may issue the notice assoon as they receive 4^quarter TTHM analysis resultsand can make a determination whether ornot they arein compliance for the4"* quarter.
Please note all violations shall be listed in the 2015 Consumer Confidence Report.
