Islington’s Integrated Care Pioneer

We are proud to be one of 14 national Pioneer sites trialling new ways to integrate care

This recognises the scope and ambition of our Integrated Care Programme

We want to deliver joined up care that meets the needs and aspirations of a changing population

Underpinning this are themes of;• Supported self management• Personalised services• Collaborative care planning• Support developed around localities

Without a workforce to support this new approach we won’t be successful

– Support Workforce Planning

– Support Workforce Development

– Respond to Islington’s Workforce Needs

– Provide Education Programme Coordination

– Ensure Education Quality

Patients/users have told us their care isn’t always joined up and that they

don’t always feel listened to and heard

Staff tell us they value:

– Learning with other professionals

– Reflective practice

– Knowing who to ask when they don’t know the answer

Our priorities within the CEPN include:

Workforce analysis and development to understand where we are now

Development and implementation of Care Certificate across health and social care workforce, to support skills development and access to quality education

Increase Undergraduate Nurse Placements in general practices to give more nurses experience of working in the community

Establish a Super Hub for Community Nursing to support development into community nursing

Using to CEPN to support change

– Professionals and other care givers able to communicate with each other more easily

– Staff who know each other and understand each other’s jobs

 – More capacity in primary and community settings created by using and

developing local teams who can share skills and reduce handoffs

– A philosophy of care giving that is person centred, holistic and builds on the assets of the individual

– Clear understanding of educational needs of our staff and plans in place to deliver this.

– Systems that support shared care by providing information in real time
