AGING GRANT-FUNDED PROGRAMS AND SERVICES
ADULT PROTECTIVE SERVICES (APS) (609) 518-4793 Services to safeguard a
vulnerable adult’s rights/resources to protect them from abuse/neglect/exploitation. APS
screens and investigates reports of possible abuse/neglect/exploitation. APS identifies and
arranges for services to assist vulnerable adults. Provided by Burlington County Board of
Social Services.
ASSESSMENT/OPTIONS COUNSELING (609) 265-5069 Intake Social Worker will
complete an assessment that will determine the most appropriate services for the
individual and identify those most at risk of nursing home placement.
CONGREGATE NUTRITION (609) 702-7053 Aging office provides nutritious meals
which meet the one-third dietary reference intake to residents age 60 and over at
congregate nutrition sites. Recreational/social activities and informational/health-related
programs are available at the congregate sites.
HOMEMAKER/HOME HEALTH AIDE SERVICES/RESPITE (609) 265-5069 Support is provided to the client for personal care activities and light household tasks
related to maintaining health and allowing client to remain in their home. Provides respite
for the caregiver.**
HOME DELIVERED NUTRITION (609) 702-7053 Aging office delivers a nutritious
meal to the resident’s home which meets one-third of dietary reference intake. Clients of
the program must be age 60 and over, home-bound and unable to prepare their own meals.
INFORMATION AND ASSISTANCE (609) 265-5069 Aging staff provide information
and links residents to services, programs and opportunities available in the community.
MONEY MANAGEMENT (609) 894-9311 X 1358 Provides guidance and non-
intrusive bill paying assistance to maximize bill-paying capacity. Volunteers assist in
budgeting, the processing of routine bills, check writing, managing financial matters and/
or reconciling bank accounts. Services are provided by the Retired and Senior Volunteer
Program (RSVP) at Rowan College at Burlington County.**

OUTREACH/PUBLIC AWARENESS (609)265-5069 Aging staff provide presentations
and public awareness events. Staff is available for informational tables at community
events and senior sites. Promotes awareness of the programs, services and
information within the community.
PHYSICAL ACTIVITY/SENIOR FITNESS (856) 231-9622 Planned and structured
activities aimed at improving or maintaining physical health of residents age 60 and
over. Aging office grant funds the evidence based Arthritis Foundation Aquatic
Program provided by the YMCA which promotes the overall wellness of the client.

RESIDENTIAL MAINTENANCE (609) 265-5785 Provides services to residents age 60
and over whose health and safety are threatened and/or at risk of institutional
placement. Tasks can include replacing door locks, minor home repairs and housing
improvements which may deter crime. Provides information on home improvement
repairs, loans and weatherization. Services provided by Burlington County Sheriff’s
Department.**
SENIOR LEGAL ASSISTANCE (609) 265-5945 Provides legal advice, assistance and/
or representation to protect and secure the rights of older persons. Program provided
to residents age 60 and over and their spouses.**
SOCIAL DAYCARE/RESPITE (856) 234-5999 Provides a safe, structured, homelike
setting where older adults can socialize and participate in activities. Services are
provided by The Tender, Inc. to seniors and their caregivers.**
TRANSPORTATION TO CONGREGATE MEALS (609) 702-7053 Transportation is
provided, on a limited basis, to participants of the congregate nutrition program.
Clients receive transportation from their home to one of three congregate nutrition
sites. The program is provided by Aging grant funds and service is provided by Sen-
Han Transit.

CANCER RESOURCE CENTER Service: The education resource center, located across from Lourdes Medical
Center of Burlington County in the Professional East Building,
provides access to information related to all types of cancer. Patients
have access to hardcopy information and the ability to view DVDs,
tapes, or access the Internet.
CARING BOUTIQUE Service: The Caring Boutique is a service offered in conjunction with American
Cancer Society, and is available to all women in the community. The
Boutique offers a variety of items including: free wigs and permanent
prosthesis to those in need, free temporary prosthesis, pillows to use with
seatbelts and prosthesis bras.
LOOK GOOD FEEL BETTER PROGRAM
Service: This free public service program helps women undergoing cancer treatment
learn to cope with appearance–related side effects of treatment and regain a
sense of self-confidence and control over their lives. The program was
founded by the Cosmetic, Toiletry and Fragrance Association (CTFA)
Foundation and is offered in partnership with the ACS and the National
Cosmetology Association. Licensed volunteer cosmetologists who are
certified in the program teach women how to enhance their appearance using
makeup techniques and how to disguise hair loss using wigs, turbans, and
scarves. Each woman receives make-up donated by CTFA cosmetic industry
and samples to take home.

The Burlington County Library is partnering with the New Jersey State Library Talking
Book and Braille Center (TBBC). The TBBC provides free public library service and
materials to New Jersey residents of all ages who cannot easily read standard printed
materials due to a visual impairment, physical handicap or a learning disability. As a
service center for the TBBC the library will assist customers in downloading talking
books for their TBBC-provided devices.

THE CLEARINGHOUSE COLLECTS, DEVELOPS, DISSEMINATES INFORMATION, MATERIALS, AND
RESOURCES IN THE FOLLOWING AREAS:
HEARING--normal structure and function of the auditory system; diseases and disorders
of the ear including hereditary and other forms of congenital deafness such as Usher
syndrome, sudden deafness, noise-induced hearing loss, ototoxicity, presbycusis, tinnitus,

“Assisted Living” means a coordinated array of supportive personal and health services, available 24 hours per
day, to residents who have been assessed to need these services, including residents who require formal long-
term care.
“Assisted Living Residence” (ALR) means a facility which is licensed by the Department of Health & Senior
Services to provide apartment-style housing and congregate dining and to assure that assisted living services are
available when needed, for four or more adult persons unrelated to the proprietor. Apartment units offer, at a
minimum, one unfurnished room, a private bathroom, a kitchenette, and a lockable door on the unit entrance.
“Comprehensive Personal Care Homes” (CPCHs) provide room and board and provide assisted living services
when needed. Residential units in CPCHs can house no more than two residents and have a lockable door on
the unit entrance. CPCHs are licensed by the Department of Health and Senior Services.

CONTINUING CARE RETIREMENT COMMUNITIES
A continuing care retirement community (CCRC) provides housing, services and health care
(including long-term care) to people of retirement age. The community must provide increasing
levels of care beginning with independent living. CCRCs offer a contract that is signed when

The many duties of the Surrogate’s Court include the probating of wills, appointing of estate administrators, supervising the appointment of guardians and administering adoptions. My responsibility as Surrogate is to oversee the administration of the many services offered by this Surrogate's Court. My team and I will work to make the emotionally difficult experience that can be associated with the Surrogate’s Court a professional and sensitive experience.
A Will is a legally enforceable written document that takes effect upon death, and directs the distribution of a person’s personal property and real estate. The advantage of having a Will saves your heirs time and money during this emotional time. You may plan now for the orderly transfer of your property, both real and personal to assure that your plans will be carried out as you desire.
Guardianship of Incapacitated Individuals: You want to assist and protect your loved one in navigating the world around them, ensuring that they handle activities of daily living while exercising independence as safely as possible. We process the Guardianship of Incapacitated matters for the Superior Court, Chancery Division, Probate Part for both the person and the person and their property both personal and real estate.
Appoint Guardianship of Minors: There are several reasons that an appointment of a guardian of a minor is necessary. The most common is guardianship of the property when a minor is to receive assets, usually money, but also includes real estate, stocks and bonds, in amounts greater than $5,000 per annum. We also appoint guardianship of the person of the minor.
Adoptions: The Burlington County Surrogate’s Court serves as the Deputy Clerk of Superior Court, Chancery Division, Family Part for all adoptions. Our office processes the Complaints and support-ing documents and schedules the hearings before the Superior Court. The Surrogate’s Court also maintains the files under seal as required.
The Surrogate also acts as Deputy Clerk of the Superior Court, Chancery Division - Probate Part. In that capacity the Surrogate is responsible for estate accountings, insolvencies, actions to remove fiduciaries and similar actions brought in the probate part.

Veterans who were honorably discharged from active service during a time war may claim real
estate tax deductions of $250 on their principal place of residence. There are no income
limitations. Under certain conditions, permanently and totally disabled war veterans or their
surviving spouse may be granted a full property tax exemption on their principal place of
residence. An application for the deduction must be filed with the Municipal Tax Assessor and
will continue in force from year to year without the necessity for further claim. A qualified
taxpayer may claim both the Senior Citizens/Disabled Property Tax Deduction and the Veteran’s
