DESMOND - Self Management Program For Type Two Diabetes Diabetes Help Tauranga is proud to announce the arrival of DESMOND in the Western Bay of Plenty. This is the result of a close collaboraon between Diabetes Western Australia (WA) and DESMOND UK. DESMOND UK launched the first newly diagnosedand ongoing programsin 2011 and have since empowered thousands of people living with diabetes to take control of their own diabetes. DESMOND, an acronym for Diabetes Educaon Self - Management for Ongoing and Newly Diagnosedis supported by a dedicated quality development and training team based in Australia who support healthcare professionals facilitang the program in New Zealand. DESMOND WA has trained over 130 Registered Healthcare professionals to become educators in Australia and New Zealand. Evaluation results in Australia have shown: A significant decrease in systolic blood pressure A significant decrease in diabetes distress Downward trends in HbA1c and cholesterol Increase in physical activity A significant increase in the perceived impact of diabetes Working with GPs, Pracce Nurses, Diabetes Nurse Specialists and other Healthcare Professionals is an integral part of the DESMOND pathway. During the program the person with diabetes is provided with the opportunity to set their own diabetes management goals which the referrer is then encouraged to support them in achieving. Diabetes Help Tauranga are licensees for DESMOND in the Western Bay of Plenty and its programs are facilitated by Registered Diabetes Nurse Debbie Cunliffe and Danelle Stevens, Registered Nutrionist. We focus on quality of delivery & program content to ensure positive outcomes Trained facilitators are guided through a mentoring and assessment process to ensure they incorporate DESMOND learning theories and philosophies in their delivery DESMOND is an evidence based program incorporating results from a randomised controlled UK trial, where educator behaviour was found to be vital in ensuring positive client outcomes Quality development processes are in place to ensure delivery is consistent and appropriate The 6 hour or 2 half day program ensures accessibility to those seeking a positive start to diabetes management What Healthcare professionals are saying: Working as a DESMOND educator is very em- powering. DESMOND - Self Management Program For Type Two Diabetes

Diabetes Help Tauranga is proud to announce the arrival of DESMOND in the Western Bay of Plenty. This is the result of a close collaboration between Diabetes Western Australia (WA) and DESMOND UK.
