Double Check the Response Document

If the response document is not completed correctly, the student’s scores will be invalid and a zero will be reported for school accountability.

Check that “Participation Level” column has one of the three levels darkened in every box. (20 circles darkened)

Check that the “Score” column has one circle darkened in every row. (28 darkened circles under the entire column “Score”)

If any circle is darkened in the “No Performance” column, check the “Participation Level”. Only the circle for “Introductory Participation Level” may be darkened.

Check “Participation Level” and “Score” sections. There should be 48 circles darkened in these sections, combined.
