Essay not to exceed two pages. The following items will be submitted by the Office of Enrollment Services along with this application: NEOMED transcripts I grant permission for the Office of Enrollment Services to provide my transcript to the Scholarship and Awards Committee for the purposes of selecting a scholarship recipient. Student Signature: Date: The recipient will be notified by May 1, 2018, and the scholarship will be applied to the student’s educational expenses for the current academic year (2017-2018). Letters of recommendation should be written to the Student Financial Aid Scholarship and Award Committee. Applicants must submit all application materials as one packet to the Office of Enrollment Services, c/o Sarah Moore. Submissions may be made in person or electronically ([email protected]). Students are in good academic standing Students are interested in a career in community Pharmacy practice The following guidelines will be used when selecting students for the awards: Application Deadline: March 15, 2018 The CVS Health Foundation has launched two scholarships for the 2017-2018 academic year. The Standard Charitable Trust Award and the Spanish Charitable Trust Award. The Standard Charitable Trust Award must have at least 25% of scholarship dollars awarded to bilingual students (please note that being bilingual is not a requirement to apply or receive the scholarship). The Spanish Charitable Trust Award will be awarded to a student that is fluent in Spanish.
